In our effort to provide seniors, adults with disabilities, and their families and caregivers with accurate and comprehensive information regarding available community resources, we are pleased to present the annual Community Resource Directory produced as a collaborative effort between our department and the Stevens Point Buyers Guide. We thank the Stevens Point Buyers Guide and the advertisers in this directory for helping to make this publication possible. Our mission is to support seniors, adults with disabilities, and their families and caregivers by offering easy access to services and by fostering a caring community that values lifelong contributions, maximum independence, and individual dignity. 6 Abuse/Adult Protective Services Portage County Health and Human Services Ruth Gilfry Center 817 Whiting Ave. (715) Contact: Intake Services Services: Department of Health and Human Services is the lead elder adults-at-risk (age 60 and older) and adults-at-risk (age 18-59) agency in Portage County and respond to and investigates reports of elder and/or vulnerable adult abuse. Abuse is defined as physical, emotional, sexual, treatment without consent and unreasonable confinement or restraint. Abuse can also refer to financial exploitation, neglect, or self-neglect. The Department of Health and Human Services works with other local agencies (Law Enforcement, Aging & Disability Resource Center, Family Crisis Center, etc.) to investigate allegations and offer services to elders or other vulnerable adults. 10 Safe & Sound Program Assisted Living Facilities - Community Based Residential Facilities (CBRFs)/ Residential Care Apartment Complexes (RCACs) Community-based residential facility" means a place where 5 or more unrelated adults reside in which care, treatment or services above the level of room and board but not including nursing care are provided to persons residing in the facility as a primary function of the facility. Residential care apartment complex" means a place where 5 or more adults reside that consists of independent apartments, each of which has: an individual lockable entrance and exit a kitchen, including a stove individual bathroom, sleeping and living areas and that provides, to a person who resides in the place, not more than 28 hours per week of services that are supportive, personal and nursing services. "Residential care apartment complex" does not include a nursing home or a community-based residential facility, but may be physically part of a structure that is a nursing home or community-based residential facility. Each learner is interviewed and assessed in the areas of reading, math or spoken English and a specific curriculum is designed to address identified needs. The Literacy Council is especially sensitive to those who have found weak English and math literacy skills to be a barrier to the achievement of personal and family goals. Learners with many different literacy needs are being served, including those who speak very little English, people with learning disabilities, and people with cognitive disabilities. Assessment, program development and instruction are provided at little or no cost to learners.
