4 NURSING HOMES Definition: A health care facility that provides around the clock care and services for people who need more than seven hours a week of nursing or personal care. Services: Must provide sufficient staff (nurses and Certified Nursing Assistants) to meet the needs of all residents. Conduct comprehensive, ongoing assessments by an interdisciplinary staff to determine the needs of each resident. Develop, with the resident s input, a plan of care that identifies the needs and wants of that individual, and sets goals with timelines, indicating the services to be provided to meet those goals, focusing on quality of care and quality of life. Provide social services to meet the psychological, social and emotional needs of each resident, in accordance with the comprehensive assessment. The social worker should be the resident s advocate. Provide and promote in-house and community leisure time activities to meet the interests and needs of the resident, in accordance with the comprehensive assessment. Nursing services must be provided to ensure that each resident stays as independent as possible, providing assistance with daily living skills as needed, and providing skilled care, in accordance with the comprehensive assessment. Manage and administer medications, according to physician s orders, unless the resident chooses to and is capable of doing this her/himself. Provide three meals a day, plus snacks, including providing therapeutic diets as ordered by the physician. Provide rehabilitation services identified in the plan of care, such as physical, occupational and speech therapy, and mental health services. Regulatory Oversight: Licensed by the Wisconsin Department of Health Services, renewed annually. Onsite inspections approximately annually. Requirement to investigate complaints. Resident Rights protections in the regulations.

5 Advocacy: The Wisconsin Board on Aging and Long Term Care Ombudsman Program provides advocacy services to residents who are 60 years of age and older. The Wisconsin Coalition for Advocacy is the designated agency to advocate for clients with Developmental Disabilities or Mental Health Concerns. Considerations for Dementia: Does the nursing home indicate they provide services for people with dementia? Does the facility call itself dementia specialists? Do they have a special care unit or in-house program designed for people with dementia with a trained coordinator? What makes them so special? Does the facility seek guidance from the Alzheimer s Association? What type of training has the staff received concerning dementia? Have all staff members been trained? Is staff able to handle difficult or challenging behaviors? Is staff trained on the most effective and humane approaches? How many staff members are there? How many residents have dementia? (staff to resident ratio) Are physical or chemical restraints used? Is the environment safe for a person with dementia? Are the activities appropriately structured for individuals who have dementia and who experience sundowning or are awake at night? At what point will the nursing home refuse to keep the person?
