4 PROVIDERS BENEFIT SUMMARY CHC IS PROUD TO OFFER OUR ARNP RESIDENTS... Medical coverage is provided through Group Health. Community Health Care employees have the choice of enrolling in an HMO or POS plan. Employee only coverage is $26.84 or $61.43 for the HMO or $62.30 for POS. Dependent coverage is also available for additional cost. Dental coverage is provided by Delta Dental. Employee only coverage is $2.43. Dependent coverage is available. Flexible Spending Account - Community Health Care offers a pre-tax savings program for dependent care and/ or health care related expenses. Life Insurance paid for by Community Health Care. Benefit is 2 times the annual salary with a maximum paid benefit of $400,000. Voluntary Life Insurance Employees can elect to purchase additional life insurance for themselves and family members. Community Health Care offers voluntary life insurance and accidental death and dismemberment insurance. Short Term Disability Insurance Long Term Disability Insurance Loan Repayment Community Health Care is a qualified site for both the Washington State Loan Repayment Program as well as the National Health Service Corp Federal Loan Repayment Program. This program is for qualifying nursing, dental, and provider positions (available if employed post residency). Sick Leave - Full time employees accrue 3.69 hours of sick leave per pay-period. Vacation 10 days per year based on a 40 hour clinic week. Holidays - Community Health Care recognizes 11 holidays. 403b Thrift Savings Plan All employees are eligible to contribute. After 1 year of employment, part-time and full-time employees are eligible for a 4% employer contribution and, if contributing, an additional 1% employer match. Continuing Education $300 per year. 5 RESIDENT ARNP JOB DESCRIPTION OVERVIEW: The Resident Nurse Practitioner will become an autonomous and highly effective member of CHC through graduated independence and leadership experiences. Provides full scope of preventive and ambulatory medical services under the direction of the NP Residency Director. This position provides general primary care including chronic care management preventive services, health maintenance and urgent care treatment in an outpatient setting. ESSENTIAL FUNCTIONS: Exercises judgment in interpreting health and illness status, performing a comprehensive assessment and diagnosis, integrating diagnostic findings, and interpreting the need for referrals. Obtains a health history and, using the problemoriented format, develops a problem list and initiates a plan of treatment. Performs a physical examination on the patient, orders, performs and evaluates basic laboratory procedures. Prescribes and/or dispenses prescribed legend drugs and treatments in treating acute and chronic health problems. Provides health education and counseling for a wide variety of patient conditions. Establishes a collaborative relationship with primary care providers within Community Health Care and the community, co-managing or referring case management according to scope of practice. QUALIFICATIONS: Current Washington State licenses for RN and ARNP with prescribing authority. Must be eligible for malpractice insurance. 3 years experience as a registered nurse involved in direct patient care. Graduation from an approved family practice/ nurse practitioner program. Board certified by appropriate NP certifying body in primary care/family practice. Ability to be an effective team member. Ability to communicate effectively with patients, their families, and to other professional health practitioners. Basic knowledge of methods and practices of medical examinations, diagnosis and treatment, particularly pertaining to ambulatory medical care. Basic knowledge of mechanisms of prevention and control of communicable diseases. Knowledge of health promotion/disease prevention, health education, and psychosocial aspects of health care. Preferred Skills: Bilingual Capacity Community Health experience Supervisory experience Managed care experience OTHER REQUIREMENTS: Maintain current CPR. Maintain board certification. Provide translation when capable and able. Attend meetings as requested. Participate in quality improvement projects as assigned. Work as a team with other staff for continuity and efficient, quality care. Requires adherence to all policies, standards and procedures included in the Administrative, Clinical and HR Manuals and the agency Code of Conduct.

Tacoma s story spans more than two centuries from the time Captain George Vancouver anchored off Tacoma s north shore in In 1870, Tacoma s natural deep-water port became an attraction that the Northern Pacific Railroad couldn t pass up, when it made Tacoma a stop on its transcontinental line. The railroad platted New Tacoma and the growth of the city began. Old Tacoma and New Tacoma merged in 1884 and incorporated as Tacoma. By 1890, the population reached 36,000 people. Sawmills, coal mines, flour mills and a smelter turned raw materials into exportable goods. Tacoma continues to make use of both its land and sea resources. It is home to the Port of Tacoma, the seventh-largest container port in the United States, and it is within 20 miles of the Seattle-Tacoma International Airport and 36 miles of the city of Seattle. Today Tacoma is a thriving city with a revitalized downtown that caters to residents and visitors alike with its shopping, dining, theaters and awardwinning arts and architecture. It is a diverse cultural melting-pot that is filled with a multitude of exciting events, activities, and entertainment.
