3 Summary of Benefits This brochure is a brief summary of current benefits. It contains information about who is eligible, when benefits are effective and the amount of the organization s contribution. These benefits represent a significant portion of the overall compensation plan extended to employees. Insurance Long-Term Disability Insurance Benefits will equal 50% of employee s monthly salary, should employee become disabled for more than six months. Short Term Disability Pays 50% of base salary up to $1,000 per week in the event of a disability Life Insurance Life insurance and accidental death and dismemberment coverage, equal to the employee s annual salary up to a maximum of $150,000. Life insurance, $4,000, includes accidental death dismemberment coverage Dependent Life Insurance Option to purchase term life insurance for spouse and dependents Additional Life Insurance Option to purchase up to two times annual salary in additional life insurance. Up to a maximum of $500,000. Critical Illness Pays a tax-free lump sum benefit directly to the employee. Guaranteed issue benefits amounts of $15,000 and $30,000 for employee. 6 Insurance continued Whole Life (with Long-Term Care Rider) Retirement Provides a death benefit to your beneficiaries if you pass away, but can also build a cash value that you can use while you are still alive. 7 Other Benefits continued Birthday Meal All Coupon for meal in hospital cafeteria Workers Compensation All Insures employees against accidents on the job under the provisions of the Workers Compensation Act. Military Leave All Permits employees to take military leave for active duty in the Armed Forces of the United States. Military leave is granted without loss of continuous service and provides the difference between regular pay and military pay up to one year. Sick-Child Care All provides sick-child care for employees while they work (based on availability). Hospital Bill Discount All employees, their spouses and dependents under the Vidant Health Medical Plan or other comparable medical plan May be eligible for a discount of up to 50% toward the deductible or coinsurance. You must request the discount from the business office within 60 days of insurance payment. This benefit covers inpatient, outpatient and diagnostic services. discounts up to 50% of the deductible and/ or coinsurance after insurance has paid. 7
