1 Placement: LVN to RN Application Checklist I understand that to be c considered for admission to the Registered Nursing Program at Hartnell College, I must have all of the following: Application for Admission: Submit if necessary, an online application to Hartnell College Admissions Completion of all Prerequisite Courses Course-to-Course Evaluation Form: Courses completed at other colleges or institutions must be evaluated by a Hartnell College counselor. Foreign transcripts must evaluated by an outside credential evaluator before a request for a Course-to-Course Evaluation form can be completed b y a Hartnell College counselor. Official Transcript(s): Submitted with th e application packet. o One (1) sealed official Hartnell College transcript o Two (2) sealed official transcripts from other colleges and/or institutions Official High School Transcript and/or a GED Report: Proof of US high school through 12 th grad e as documented by an official high school transcript and /o r a GED report. ATI Test of Essential Academic Skills (TEAS) Version V Score: A computer generated print-out of a minimum final comprehensive score of 62% must be obtained in order to be considered for acceptance. Scores within 2 years of the application submission date are acceptable. You may take the ATI TEAS more than once, the highest score will be considered. All required documents must be submitted in-person or mailed to the Nursing and Allied Health office by Friday, October 31, 2014 at 4:30 pm. Mailed documents must be post-marked no later than the deadline date.

2 P lacement: LVN to RN Application Instructions Admission to Hartnell College: Students not yet enrolled at Hartnell College must complete an online application for admission. Refer to Hartnell College s website at artnell.edu/admissions/apply.html Required Prerequisites: Prerequisites taken at Hartnell College: An official Hartnell College transcript must be included with the application packet. A list of required prerequisites is listed on the Nursing & Allied Health s website at artnell.edu/nursingandalliedhealth/ Prerequisites taken at any other college/university/institution(s): An authorized Hartnell College Course-to- Course Evaluation Form must be included with the application packet. The Nursing and Allied Health counselor will evaluate course descriptions and official transcripts provided by the student for each course completed at other colleges and universities. Once reviewed, the Nursing and Allied Health counselor completes and signs the form that must be included when the application is submitted. Appointments with the counselor are made by calling the Hartnell College Counseling Office at (831) Official Transcripts.
