Letter Over 18 Neuro 6 weeks maximum According to clinical need, once or twice a week Goal based, 9 sessions on average No OT is through social services. If patients need home therapy, need community physio. Sunderland also has inpatient neuro rehab. Transport to the service is bad.

6 Sunderland community OT- part of social services Single point of access Tel: See patients once, assess for equipment needs All of Sunderland Telephone to single point of access and ask to be referred to Duty OT. An OT will contact patient within 10 days. All All including palliative for community OT. Can see palliative patients urgently. One-off - within Sunderland wall with city hospital OT and happy to work closely with other hospitals. Undergoing change to become a re-enablement team
