1 Community Based Services AREA AGENCIES ON AGING Area Agencies on Aging were established in 1973 under the Older Americans Act to respond to the needs of Americans age 16 and older in every local community. AAA s plan, coordinate and offer services to help older adults remain in their home, if that is their preference. Services include Meals on Wheels, homemaker assistance and whatever else it may take to make independent living a viable option. AAA services fall into five categories: information and access; community-based services (such as senior centers); inhome services; housing; and elder rights. To find out about services to older adults in your community, the best first call you can make is to your AAA. For our area, the AAA is the Council on Aging of Southwestern Ohio. Bayley PLUS Community Outreach Services Farrell Ct., Staff members help individuals obtain information about community services for mature adults. Information may be obtained by telephone with our 24-hour hotline, onsite visit or over the internet. A variety of religious and other books, magazines, tapes and health materials are available for general use in the Resource Center, which offers a wide range of information for continued learning and support. The Caregiver corner provides information about related health care issues to caregivers. Professional Resource Area stocks periodicals, videos and audio tapes for health care and social service professionals. Butler County Elderly Services Program or Fairgrove Ave. (Rt. 4), Hamilton Helps older adults remain safe and independent in their own homes. Services are available to people age 65+. Home delivered meals are available for people age 60+. Services include adult day services, companion, home delivered meals, independent living assistant, medical equipment, medical transportation, personal care, respite, homemaking, and home repairs and modifications. Catholic Charities SouthWestern Ohio , or E. Eighth St., Caregiver Assistance Network provides support, education, information and referral to family caregivers. Our services extend to those who reside in Butler, Warren, Clinton, Clermont and Hamilton co s. Caregiver CAREline is an information and referral phone line available M F, 8:30 A.M. 4:30 P.M. 2 Homecare Program provides the necessary supportive services to help restore and strengthen adults and their families, and provides the care needed to prevent institutionalization. The program includes light housekeeping, errands, personal care and independent living assistance to assist with personal business needs such as banking, bill payment and mail. Availability depends on area of Hamilton County and current staffing. In-home counseling helps homebound clients cope with isolation, depression and anxiety. Family Consultations assists with communication and addresses caregiver issues (limited to Hamilton Co.) For all faiths. Senior Centers Senior Centers...are places where older and disabled adults can gather and make new friends. While programs vary by center, most offer outreach, health screenings and counseling, planned group tours, cards, crafts, recreational activities, and a variety of educational, fitness and volunteer programs. Some have congregate lunches or home delivered meals and volunteer or Federally funded transportation services. Some centers may charge a small annual membership fee. Congregate Meal Sites This program is funded by the Older Americans Act. It is intended for persons 60 years of age or older to have one nutritionally balanced meal on weekdays. Advance meal order reservations are usually required. Most Programs encourage a monetary donation. Congregate Meals or nutrition sites may be in senior centers, in senior apartments, churches, temples or community centers. Transportation to these meals is available. 5 Home Delivered Meals (Meals on Wheels) Persons 60 years of age or older who are homebound and unable to shop for or prepare meals may be eligible for a home delivered meal. The meal, which may be hot or frozen, is balanced to provide one-third of the daily recommended nutritional requirements. In some cases, two meals may be delivered daily, either frozen or a brown bag supplemental meal. Special dietary restrictions such as low cholesterol, diabetic and low salt are followed. These programs also provide some monitoring of the homebound person. Home Delivered meals Meals on Wheels are funded by a variety of sources including federal funding from Title III-C-2 of the Older Americans Act and local tax levies. Some are privately funded. Most programs encourage contributions. Some assisted living and residential retirement facilities have private home delivered meals service for set fees. CAREGIVER TIP Did you know that several social service agencies and home healthcare agencies offer homemaker services? CAREGIVER TIP Older adults have special medication needs and issues. Many times they take several medications for more than one medical condition. It is important to note that the body disposes of drugs differently as it ages, causing a person to become more sensitive. Make sure you know: The name of all your drugs and ask your pharmacist or doctor all about what you are taking. How many times a day and when to take the medication. Should you take it with or without food? What to do if you miss a dose. How long you need to take it and if it needs refilling. Any and all possible side effects and if the drugs interact with any foods, activities or other drugs. Any special storage instructions. Use your prescription medications wisely. 7 Transportation Title III of the Older Americans Act provides for some transportation to congregate meal sites and senior centers within a three mile radius of the site. Title XIX of the Social Security Act (Medicaid) provides for transportation of Medicaid patients for medical services. Curb to curb transportation requires a rider to be able to walk from house to curb to enter vehicle. Escort service is when a person accompanies an older adult or a small group who needs help getting to and from medical appointments, shopping or banking. See Primary Outreach Agencies and Senior Centers for referral and matching with an escort program near you. Home to door service assists or carries the rider from the home to the vehicle through the door of the destination. A person who needs this should call and confirm distance to be carried, number of steps and allowable weight.
