This, along with the questionnaire overleaf which we would ask you to complete and return to us, is designed to assess your general level of health. Any of our practice nurses or the doctor will carry out the screening. If they should find any problem they will then arrange for further follow up. 5 Welcome to Alness/Invergordon Medical Group and Primary Health Care Team With the help of the patients we have prepared this leaflet to give you information about our services. The Medical Practice, Primary Health Care Team and NHS The Medical Practice is made up of the Doctors, the Practice Nurses, the Practice Managers and the Receptionists. The Primary Health Care Team consists of the Medical Practice plus the nursing staff, physiotherapists, chiropodist, dentist and visiting specialists. All members of the team have specific skills and experience, which they bring together to provide the help you need. We have given detailed information about medical services and brief details about services provided by everyone in the Primary Health Care Team. These can be found on the chart at the back of the booklet. If you require care from the other members of the team they will give you more details about their services. The Practice Managers and Receptionists The Practice Managers manage the administration of the Practice and the Receptionists deal with appointments, repeat prescriptions, results and general enquiries. Registrars and Students At certain times a GP registrar (trainee medical practitioner) and/or medical students, or students with any member of the Primary Health Care Team may be working under supervision. You may be asked for your permission for a student to be involved in your care. 5

Each doctor is qualified to provide a full range of medical services General medical consultations, treatments, advice and referral to specialists if required. Maternity care Contraceptive services and cervical screening Child health check ups and immunisations Travel immunisations Minor surgery and treatment for minor injuries. Health Promotion The Primary Health Care Team encourages all patients to adopt a healthy lifestyle by stopping smoking, drinking alcohol in moderation, eating a well balanced diet and taking regular exercise. We offer Individual counseling to help you stop smoking and lead a healthy lifestyle Regular checks if you have heart disease, stroke, asthma or diabetes Preconceptual counselling information and advice for people considering pregnancy Private medical services with costs - Some services are not provided on the NHS, for example, pre-employment medicals, private medical insurance certificates and some travel immunisations. Details of these and their costs are available from the receptionists. Health Centre Opening Times Monday Friday, 9am 5.30pm 6 consultations are by appointment.

7 Making an appointment To make an appointment to see a doctor please telephone Alness or Invergordon or call at the Health Centre during opening hours. The receptionist will try to give you a time that is convenient to you and with a doctor of your choice, if this is not possible she will suggest an alternative time or another doctor who is available. The Practice also offers telephone consultations where appropriate. Please ask the receptionist if you feel this type of consultation would be suitable. Each appointment is for one patient and one problem only. Home Visits by Doctors If your illness prevents you from attending the surgery, you can ask for a home visit by telephoning Alness or Invergordon Please telephone before 10am. Our reception staff may ask for further information to help the Doctors assess the urgency of your call. When the Practice is closed If you need a Doctor outside normal opening times please contact NHS 24 on If you telephone the surgery you will be given the contact number via an answer phone. Please restrict these calls to urgent problems that cannot wait until the next surgery. Emergency Consultations During Surgery Opening Hours Every effort will be made to see patients with urgent medical matters during surgery hours. Please telephone Alness or Invergordon and the Doctor will either ask you to attend the surgery, or if he/she feels it more appropriate, arrange a visit. Arrangements for Repeat Prescriptions You will have been given a repeat prescription form with a list of your medicines with your last supply. Tick on the form which medicines you need and where you want to collect your prescription; surgery or chemist. Give the form to the receptionist and your prescription will be ready for collection form the surgery 48 hours later. Another 48 hours should be allowed if you wish to collect the prescription from the pharmacist. Alternatively you can post your form to the surgery with a stamped selfaddressed envelope and we will send you your prescription. 7

8 Facilities at the Health Centre Car Parking Parking spaces are limited; please only use the car park when attending the Health Centre. Access for Disabled Patients Both Health Centres are all on ground floor level with a ramp for wheelchairs. There are also disabled toilet facilities within each building. Mothers and Babies There are facilities for baby changing and nursing mothers. Fire Safety In the event of the fire alarm ringing, members of staff will advise you what to do. Smoking and Alcohol The Practice operates a No Smoking and No Alcohol Policy. Please be aware that if you are under the influence of alcohol, your appointment may be cancelled. Courtesy to Staff Staff is required to be courteous to the public and courteous behaviour towards staff is appreciated. Abusive or violent behaviour causes offence to both staff and patients; the Practice will refuse to provide services, or will prosecute, any person whose behaviour is disruptive. You can help us provide an efficient service by: Informing us if you change your name, address or phone number Arriving on time for your appointment and letting us know as early as possible if you want to cancel an appointment Telephoning between am only for home visits and same day appointments and after 1.00pm for non-urgent appointments and results. Please be concise when telephoning as others may be trying to call. 8

9 Patients Rights and Responsibilities How to make comments, suggestions or a complaint - We welcome comments, suggestions or any complaints about any aspect of the service or your care. In the first instance, please address these to the Practice Managers. For issues relating to the medical group The Practice operates an in-house complaints procedure. If you wish to make a formal complaint please contact the Practice Managers or one of the Doctors. 12 Freedom of Information (Scotland) Act 2002 came into force on 1 January 2005 and enables any person requesting information from a public body to receive that information, subject to certain exemptions. This is to encourage public authorities, (including GP practices) to be more open and accountable and organize their information in an efficient and accessible way. This excludes personal data. The Practice Publication Scheme Document is available to view, please contact the Practice Managers for more information. Data Protection Act 1998 you are entitled to access your clinical records held by the Practice. This should be done in writing stating exactly what information you require. A charge may apply for this request of between maximum. Confidentiality of Records Practices undergo regular visits by external assessors whose purpose is to verify the process of the Practice s quality of care to the patient, therefore they may need to view records of a random number of individual patients. The visiting team adheres to a strict code of confidentiality. If you do not wish your records to be inspected then please inform the Practice Managers. 12
