3 WELCOME TO THE PRACTICE. Our Practice is based at two premises, built for the purpose in 1980 and With the expansion of the village the Holmeon Spalding Moor surgery has been further extended in 1990 and Both sites are suitable for wheelchair access and have car parking facilities. The surgery at HolmeonSpalding Moor has offices for the community nursing team and health visitor. The Bubwith surgery was extended early 2010 through a training grant and has doubled in size and enables us to offer a full range of services to the community at both sites. This guide sets out the services we are able to offer, gives details of the staff and clinical team, opening hours and surgery times for both sites. It will help you to make the best use of the services we provide. THE SERVICES WE PROVIDE The Practice is contracted by the East Riding of Yorkshire Primary Care Trust to provide what are termed essential services for patients registered with the Practice. In broad terms this means looking after the acutely and chronically ill. We also offer additional services which include cervical screening, contraceptive services, vaccinations and immunisations for adults and children, child health clinics, maternity medical services and minor surgery. In addition to the services we provide we also host a wide range of other services meaning easy access to podiatry, dietician, midwife, primary care counselling and physiotherapy. PRACTICE CLINICAL TEAM AND HOW THEY CAN HELP YOU When making an appointment it is helpful to book with the clinician best skilled to deal with your needs. The information below should help you to book an appointment with the right person. If you are unsure who to book with please ask any member of the reception team. The Doctors. Dr B White, MB, BS, MRCGP, DRCOG Senior Partner Dr K A Richardson MB, ChB Dr R Horsley MBBS BSc (Hons) DRCOG, MRCGP GP Specialist Trainee Registrar The practice is a GP training practice and takes qualified doctors who wish to specialize in general practice on six month rotations. Hull York Medical Students The practice provides placements on four month rotations for final year medical students. Their training depends upon the opportunity to consult with patients and gain practical experience of a wide range of conditions. You may be offered an appointment with the student, followed by an appointment with one of the partners. We would ask that you help us to support their development. You can however decline a student consultation if you wish.

4 Other Clinical staff, Nurse Practitioner/Prescriber SRN, BSC (Honours) She has undertaken extended nurse training and prescribing. Janet can deal with and prescribe for most minor ailments, coughs, colds, sore throats, ears. Janet also advises on contraception and HRT and can prescribe for these. Janet is able to refer patient s to hospital consultants should the need arise. She runs the practice recall system for medication reviews to ensure that patients are having their asthma, diabetes and other long term conditions monitored. Janet Almgill RGN, ENB Practice Nursing Diplomas in Advanced Asthma Care, Coronary Heart Disease prevention and treatment, cervical cytology and women s health. Christine Wood RGN, ENB Practice Nursing Diploma in cervical screening and women s health, basic and intermediate studies in diabetes care. Janet and Chris provide adult and children s vaccinations, diabetic and asthma reviews, dressings and wound care including suture removal, ear syringing, smears, ECG s and spirometry, along with a range of other tests including Glucose monitoring and anticoagulation monitoring. Sue Oxtoby, Healthcare Assistant. REGISTERING WITH THE PRACTICE To register with the practice please call in to either of our surgeries. You will be required to complete a registration form for each family member and may be asked to provide some additional identification. You will also be asked to complete a new patient questionnaire which assists us in gaining information regarding your general health. If you are on any regular medication please ensure that you have the details of this. Please remember that it can take 6 8 weeks for your records to come from your previous GP. MAKING APPOINTMENTS We undertake to provide every patient who needs an appointment, with an appointment. What we can t promise is that the GP that you wish to see is available at all times and you may be offered an alternative. There are many reasons for this including GP leave and study, the time already booked by another patient, and the GP at the other surgery. We can however work together to make the appointment system as effective as possible. The following information should help you to see your chosen GP in the majority of instances. Please tell the receptionist what type of appointment you require. Sue will provide new patient checks, blood pressure monitoring, spirometry, ECG s and routine blood taking. Jane Slater, Phlebotomist Jane will provide routine blood tests and blood pressure monitoring.

5 TYPES OF APPOINTMENT Medication reviews Most of these appointments are now been made with the Nurse Practitioner, however if you have more then 10 repeat medications you may be booked with a doctor. Routine/Non urgent These appointments can be made up to four weeks in advance, to secure the clinician of your choice please book as much in advance as possible. Urgent Same Day Make these as early in the day as possible. You may not be able to see your preferred Doctor urgently. You may be offered an appointment with the nurse practitioner if appropriate. Extended Hours Surgery Routine Non Urgent There are a limited number of late appointments for patients who have difficulty accessing the surgery during normal working hours. These are available on Monday s between 6.30 and 7.10pm. These are not suitable for nondispensing patients who require immediate medication as the local pharmacy will be closed. Patients requiring urgent treatment should contact the out of hours service on Telephone Consultation You may be offered a telephone consultation with GP or Nurse practitioner. If you believe that your condition can be dealt with over the phone please ask for a GP call back. Home Visits Home visits are for patients too ill to come to the surgery. Requests should be made before 10.00am where possible. HOW TO BOOK AN APPOINTMENT There are a number of ways that you can book an appointment Online booking via the practice website You will need a password for this service which you can get from reception. By telephone to the surgery you wish to attend. In person at either surgery. Children under 14 years should be accompanied by an adult at their appointment. Please book a separate appointment for each member of the family wishing to be seen. Please, please remember to cancel your appointment if you no longer need it. RESULTS INFORMATION Blood test results can take up to 2 weeks for the results to be available. If you have results which indicate immediate treatment the doctor will contact you directly. You can now ring reception to ask about your results, if the doctor has viewed the results and they state no further action but you are still experiencing symptoms please book a routine GP appointment to investigate further. If your result indicated a change to treatment, further treatment or investigation is required, you will be contacted to make a routine appointment. If we can not contact you by telephone, you will receive a letter asking you to contact reception. We will not contact you if all your results are normal. If you haven't heard anything or you wish to confirm your results are back please call the surgery you usually attend after 1.00pm 14 days following your blood test. Some tests do take longer then others. OUT OF HOURS ARRANGEMENTS If you require urgent medical attention when the surgery is closed Contact the Out of Hours Service on or call NHS Direct, the 24 hour nurseled help line, on

6 REPEAT PRESCRIPTIONS AND DISPENSING The practice can dispense medications to all patients who live more than 1 mile from a pharmacy. For patients who live within 1 mile of a pharmacy they must collect their prescription from reception, or arrange with a chemist to have these collected. Holme Pharmacy is able to provide a collection service to our nondispensing patients. Please complete a form at the chemist if you wish them to collect your prescription. Acute Medication These will be ready after your consultation in most circumstances. Regular Medication If you are taking regular medication the doctor will have set up a repeat prescribing record for you. This sets out the medication you have been prescribed and enables you to get further supplies, without a consultation, for a limited period. You should note when your medication review is due and make an appointment before you run out. This can be found on your prescription counterfoil. HOW TO ORDER REPEAT MEDICATION Please allow 2 working days (48 hours) between ordering and collecting your prescription. Counterfoil Complete the prescription counterfoil, ticking the items you require and deliver to either surgery, stating where you would like to pick up your medication in the space provided. 24 hour telephone answering service You can leave a message on the answering service. Please state your name and date of birth and telephone number where we may contact you and clearly list the items you require. YOUR MEDICAL RECORD To make sure you receive the best possible care we keep information about you. In addition to basic information such as address, date of birth and NHS number it may include details about your consultations, treatment and results of investigations such as blood tests. You can help to keep this accurate by informing us of any changes in your circumstances. Some of the information we hold may be shared with other health care agencies such as hospital doctors or other agencies within the NHS. Information about you will not be shared with any agency outside of the NHS without your written consent. The practice fully complies with the strict legislation under The Data Protection Act 1998 which controls the way we look after data. It also gives you the right of access to information we hold about you. If you want to see your records you should make a written application to the Practice Manager. The Practice is permitted to make a reasonable charge if you require copies of any information. Confidentiality All information held at the practice is highly confidential. If you wish to discuss any aspect of your care, please feel free to request this is done in private. Please don t ask us to give out information regarding a family member or relative over the age of 14, as we cannot do this without their express consent.

7 NONNHS SERVICES Some services are not covered under the NHS contract. The practice will charge for these services in accordance with British Medical Association (BMA) recommended fees. These include holiday vaccinations and travel cancellations, HGV and taxi medicals, private prescriptions, insurance reports and letters. For full details of our charges please ask any member of staff. This work is not high priority therefore we ask patients to allow at least 14 days for this work to be completed. FITNESS TO WORK NOTES These are issued by the GP after the initial 7 days of absence from work. Your employer should provide you with a self certification form for the first 7 days of any illness. If you are on long term sickness absence you may be able to request a certificate by telephone. OCCUPATIONAL HEALTH SERVICES It is the employer s responsibility to ensure that adequate services are in place to cover occupational health. We are unable to offer these services unless confirmation of payment is received from the employer. GENERAL HEALTH CHECKS If you are under 65 without a long term condition and have not been seen by a Doctor for five years you are entitled to a general health check. This can be booked with the nurse. Blood tests will only be carried out if there is a clinical indication that these may be necessary. If you are over 75 you are eligible for an annual health check. FLU AND PNEUMONIA VACCINATIONS The Pneumonia vaccination is a single vaccine for all over 65 s, and those with underlying health conditions. The seasonal flu vaccination is an annual vaccine for all over 65 s and those under 65 in an at risk disease category. The practice offers these vaccines from mid September through to December, through a range of special clinics. Please contact the surgery for further details. We can only vaccinate those patients eligible for a vaccine. CHILD HEALTH CLINICS The health visitor, Liz Boville runs baby clinics each Tuesday afternoon at Holme On Spalding Moor, and on the second and fourth Tuesday morning in each month at Bubwith. You can arrange to have baby vaccinations and child health checks done at these clinics. DISTRICT NURSES The district nurses Claire, Wendy, Vicky and Jenny work closely with the doctors to deliver care to our elderly and sick population. The single point of contact number is MIDWIVES The community midwives, hold clinic s at Holme on Spalding moor every Wednesday morning and at the Bubwith surgery on alternate Friday afternoons. If you've recently found out you are pregnant you should contact the direct access line first on CARERS The practice is keen to support anyone who cares for a patient over the age 18 that is unable to look after themselves. We can direct patients to specialist services and support networks. Please complete a form at reception if you have a carer or are a carer.

8 WORKING TOGETHER NOTES For us all to get the best out of the services we offer we must work together. As patients you have a right to expect: NHS treatment delivered locally where possible and without discrimination. To be allowed to make informed choice. To be treated with courtesy and consideration by all staff. For all information about you to be treated with confidentiality. For our doctors, nurses and staff to pursue appropriate training and development to ensure we deliver the highest standard of care. In return we expect from our patients: To attend appointments at the arranged time or to cancel in advance so that this appointment can be offered to another patient. To make a separate appointment for each person wishing to be seen. Only request home visits for those too ill to attend the surgery. Remember that many problems can be resolved without a prescription. Treat all doctors and staff with courtesy and respect. The Practice has a zero tolerance policy and if any patient displays aggression or abuse to staff or other patients then you may be removed from the practice list.
