2 Welcome to Seymour Medical Practice This leaflet is a guide to our services for both existing patients and those considering registering with us. It tells you about our services, how to access them and some general information about how our practice operates. REGISTERING WITH THE PRACTICE If you would like to register with the practice and live within our practice boundary, you will be asked to provide photograph identification (normally a passport or driving licence) and proof of your address in the form of a utility bill or bank statement which bears your name and address and which is not more than three months old. You will also be asked for the name and address of your previous GP. All new patients will be asked to fill in a health questionnaire, giving details of medical history and lifestyle and all adult patients will be offered an appointment for a new patient medical check. PRACTICE HISTORY Seymour Medical Practice was formed in May 2005 by the merger of two practices, Dr Norman & Partners and Dr Price & Partners. 3 GP REGISTRARS AND MEDICAL STUDENTS Our practice is a training and teaching practice. Our GP registrars are fully qualified doctors and have a great deal of hospital experience. Registrars are attached to our practice for a number of months in order to complete their specialist training in general practice. We are also fortunate to have medical students attached to our practice from time to time. We are grateful to patients who agree to allowing a medical student to be present during consultations in order to assist in their training. NURSING TEAM Led by our Nurse Partner, Mrs Elizabeth Turner, our team of Nurse Practitioners, Practice Nurses, Treatment Room Nurses and Health Care Assistants offer acute care, treatment room nursing and chronic disease management services to our patients. ADMINISTRATION TEAM Ms Sarah Forde, our Practice Business Manager, leads the administrative team and manages the practice. Mrs Laura Baker, our Reception & Access Manager, manages the reception services provided to our patients both on the telephone and at our front desk. SUPPORTING HEALTHCARE TEAMS Additional clinical services are provided to our patients by the Primary Care Trust funded teams of Community Nurses, Midwives and Health Visitors based at the Charlotte Keel Health Centre. These teams can be contacted through the Charlotte Keel Health Centre main reception on The Health Links service, which is also based at the Charlotte Keel Health Centre and funded by the Primary Care Trust, provides interpreting and advocacy services to patients who have difficulty accessing healthcare services because of cultural or language barriers. The Health Links team can be contacted on

4 OPENING TIMES The practice is open for telephone calls Monday to Friday The health centre is open Tuesday to Friday and and on Monday. APPOINTMENTS Planned care appointments can be booked in advance by telephoning , by visiting the practice or on our web-site if you have registered for online access. Patients who contact the practice with an urgent care need will be offered a call-back from a GP under our telephone triage system. The GP will normally call you within two hours to discuss your condition and to determine the most appropriate way to meet your healthcare needs. We also offer pre-bookable planned care appointments on Wednesdays and on alternate Saturday mornings. At these times, the health centre is only open for those patients with booked appointments. HOME VISITS Our GPs and Nurse Practitioners can typically see four patients in the health centre in the time it takes to do a single home visit. For this reason, we ask our patients to come to the practice if at all possible. Patients are requested to telephone before if a home visit is required. Please give the receptionist as much information as possible to enable us to allocate priority to home visits. OUT OF HOURS If you need urgent medical attention outside of our opening hours, please telephone the NHS 111 by dialling 111. EMERGENCIES Whatever the time of day, if you are have a serious problem such as severe chest pain, loss of blood or suspected broken bones, go to the nearest accident and emergency department or call NHS 999 by dialling 999.

5 SELF CARE Remember that you can treat many minor ailments, such as colds, coughs and indigestion, by keeping a well-stocked medicine cabinet at home. We suggest you keep the following: Paracetamol and aspirin (children under 16 and people with asthma should not take aspirin) Mild laxatives Anti-diarrhoeal medicines Rehydration mixture Indigestion remedy (for example, antacids) Travel sickness tablets Sunscreen SPF15 or higher Sunburn treatment (for example, calamine) Tweezers and sharp scissors A thermometer A selection of plasters, non-absorbent cotton wool, elastic bandages & dressings Remember to keep your medicine chest in a secure, locked place. Always read the instructions and use the suggested dose and do not keep any medicines which are past their expiry dates. CONSULTING YOUR LOCAL PHARMACIST Your local pharmacist will be able to give you free health advice at any time without an appointment. Under the Minor Ailments Scheme, pharmacists can also prescribe some medication to certain patients. If you are eligible for free prescriptions, there will be no charge for the prescriptions generated by the pharmacist. CONSULTING NHS 111 NHS 111 offers health information and advice 24 hours per day dial 111.

6 CLINICS Post Natal Clinic Thursday AM By Appointment Patients are seen by a GP. Appointment bookings will be automatically sent to patients who have given birth. Baby Clinic Monday & Wednesday PM By Appointment Patients are seen by a GP and Nurse to assess your child s developmental progress and administer immunisations. Appointment bookings will be automatically sent to parents. Well Woman Clinic Tuesday PM By Appointment A GP and a Nurse provide women s health services including smear tests. Chronic Disease Management Clinics By Appointment Daily appointments are offered by Practice Nurses for chronic disease management including diabetes, COPD, heart disease, asthma and epilepsy. Travel Vaccinations Clinics By Appointment Please make an appointment at least eight weeks prior to your date of travel to ensure adequate cover. A charge will be made for certain vaccinations which are not covered by the NHS. A list of charges are available on our web-site and at our reception. REPEAT PRESCRIPTIONS Repeat prescriptions will be issued at the GP s discretion and are normally for patients receiving long-term treatment. Requests can be made through your pharmacists, by visiting the health centre, in writing or via our web-site if you have registered for online access. Requests received before on one day will be available after two working days later. Please ensure that you order your repeat prescriptions in good time and, if you are ordering the repeat yourself, we would ask that you always use the tear-off slip from your last prescription as this ensures that we have the accurate drug names when processing your request. TEST RESULTS Please telephone for results after when the phone lines are less busy. The time taken for testing varies according to the test; your GP or Nurse should be able to tell you how long we would normally expect to have to wait for results.

7 PATIENTS WITH SPECIAL NEEDS The health centre has disabled car parking spaces adjacent to both entrances and there is a wheelchair-accessible toilet in the waiting area. We can arrange interpretation services for patients who have a profound hearing loss. Interpreting for patients who do not speak English is available via a telephoneinterpreting service or in person through the Health Links team. PATIENT CONFIDENTIALITY We respect your right to privacy and keep all your health information confidential and secure. To enable the multi-disciplinary team to provide you with healthcare that meets your individual needs, you should be aware that it will sometimes be necessary for us to share medical information with our fellow healthcare workers. You have a right to know what information we hold about you. If you would like to see your records, please telephone the practice and ask to speak to the Practice Business Manager. COMPLIMENTS, COMMENTS & COMPLAINTS We are always pleased to receive comments about our service so that we can understand what you appreciate and where you think we could make improvements. We aim to give a professional service to all our patients. In the majority of cases any concerns you may have about our service can be resolved quite easily. However, if you feel we have not dealt with the issues you have raised as you would wish, please ask for a copy of our complaints leaflet which provides you with the information required to assist you in making a formal complaint. ZERO TOLERANCE OF VIOLENCE OR ABUSE The practice supports the NHS policy of zero tolerance with regard to violence or abuse to our staff. Patients abusing this policy may be reported to the police and removed from our patient list.
