4 Page INTRODUCTION 1.1 General This Fire Safety Plan has been prepared for the Scarborough Chinese Baptist Church located at 3223 Kennedy Road in Scarborough, Ontario. This plan has been developed in accordance with Section 2.8 of the 1997 Ontario Fire Code (OFC, O.Reg.388/97 revised to 144/06). This Fire Safety Plan is designed to provide occupant safety in the event of fire, to provide effective utilization of the fire safety features of the building and to minimize the possibility of fires. This plan discusses what occupants are to do in the event of fire, fire safety, supervisory staff and related duties, and other related issues. The Fire Safety Plan will also assist firefighters in the performance of their duties, by providing floor plans, building, tenant and occupant information, if an emergency every occurs. In order for this plan to be effective, Supervisory Staff must know the Fire Safety Plan and be able to implement it in the event of fire. The Fire Code requires the owner to be responsible for carrying out the provisions for fire safety, and defines owners as any person, firm or corporation controlling the property under consideration. Consequently, the owner may be anyone of, or a combination of parties, including church administrators, clergy, congregation and tenant groups. The Superintendent has been appointed as the Chief Fire Warden to oversee the implementation of the Fire Safety Plan. The Superintendent will be responsible for all aspects of the plan including appointing and training sufficient subordinate Supervisory Staff (i.e., Fire Wardens, Event/Activity Fire Wardens) to successfully implement the plan. The Chief Fire Warden is considered as part of the Supervisory Staff and plays a key role in the effectiveness of the Fire Safety Plan because of his/her involvement and familiarity with the mechanical services, piping and general layout of the building. As such, the Chief Fire Warden or alternate is able to provide the Toronto Fire Services with valuable information pertaining to access and conditions within the building during a fire situation. To ensure that a high level of life safety is maintained in the building at all times, it is imperative that the Fire Safety Plan be implemented and rehearsed in regularly staged fire drills. This document is for use by Scarborough Chinese Baptist Church. RBAL relied in part on information provided by others in preparing the original Plan dated April 20, This updated Plan dated April 11, 2011 has been modified by the Owner. It is not intended for use by any third party or subsequent owners.
5 Page 4 The Fire Safety Plan has been approved by the Toronto Fire Services, but this does not in any way relieve the owner, the lessee, the Superintendent, tenants or the clergy of their responsibilities as set out under Ontario Fire Code. The Fire Protection and Prevention Act states that every person who contravenes any provision of the Fire Code and every director or officer of a corporation who knowingly concurs in such contravention is guilty of an offence and on conviction is liable to a fine of not more than $50, for an individual or $100, for a corporation or to imprisonment for a term of not more than one year or both. The Toronto Fire Services may require this plan, or parts thereof, once approved, to be resubmitted if there are any changes to occupancy or use, if there is any change in Standards, if the Fire Safety Plan has not been kept current or up-to-date, or because the Chief Fire Official judges the current Fire Safety Plan as no longer being acceptable. The Chief Fire Official is to be notified regarding any subsequent changes in the approved Fire Safety Plan.

Multi Purpose Rooms / Gymnasium (Group A, Division 2 occupancy). Offices (subsidiary Group D occupancy). Number of Storeys Two storeys and a basement. 2.2 Building Resources Fire Department Access Via a private fire access route accessed from Kennedy Road to the West of the building (see Attachment A). Barrier to Access None. Access to the Church Via main entrance located to the West of the building (see Site Plan). A second entrance is located to the East of the building adjacent to the parking lot. 2.3 Fire Alarm System Type This building is provided with a Simplex 4100-U singlestage, zoned supervised fire alarm system. Detection Fire alarm manual pull stations. Smoke detectors within service rooms, vestibules, and outside select stairwells. Duct smoke detectors. Smoke detectors at top of stairs. Sprinkler flow switches. Remote Fire Alarm Annunciator Located in the West entrance vestibule (see Attachment A). Fire Alarm Control Panel Located in the Northeast section of the Ground Floor (to North of circulation stairs).

13 Page EMERGENCY PROCEDURES 4.1 Signage The actions to be taken by occupants in emergency situations are required to be posted at elevators, exits and above the fire alarm manual pull stations throughout the building. The signage will read similar to the following: IN CASE OF FIRE UPON DISCOVERY OF SMOKE OR FIRE Leave fire area immediately, closing all doors behind you, if safe to do so. Actuate the closest fire alarm manual pull station. DO NOT USE ELEVATOR. Call the Toronto Fire Services: UPON HEARING FIRE ALARM Leave building via the nearest exit, closing all doors behind you. DO NOT USE ELEVATOR. REMAIN CALM The building is equipped with a single-stage fire alarm system. The fire alarm system is to be activated to alert occupants of an emergency and to put into operation the approved Fire Safety Plan. The Toronto Fire Services is to be notified by telephoning giving the correct address (3223 Kennedy Road) and the exact location of the fire. 4.2 Procedure Description The single-stage fire alarm system installed in the building provides emergency signals as follows: 1. Actuation of any fire alarm device in the building causes an alarm signal throughout the Church. 2. The alarm lamp will illuminate at the fire alarm control panels and remote annunciator. 3. Signal sent to ULC listed monitoring company.
14 Page OWNER The Owner is responsible for overseeing the implementation of this Fire Safety Plan. The Owner is not required to participate in supervisory responsibilities assigned under this Fire Safety Plan but is required to follow the occupant emergency instructions specified in the Fire Safety Plan if a representative of the Owner is in the building during an emergency. The owner is defined as any person, firm or corporation controlling any portion of the building or the property under consideration and includes the persons in the building or property. Please note the responsibilities of the Owner have been assumed by the Board of Directors (see Section 6.0). 5.1 Owner Responsibilities 1. Establishment and implementation of emergency procedures to be used in case of a fire emergency. 2. Appointment and organization of designated supervisory staff (including Event/Activity Fire Wardens) to carry out fire safety duties and specific responsibilities as per the approved Fire Safety Plan. 3. Training of supervisory staff (including Event/Activity Fire Wardens) and other occupants so that they are aware of their general and specific responsibilities for fire safety. 4. Holding of fire drills in accordance with the Fire Code, incorporating emergency procedures appropriate to the building. 5. Control of fire hazards in the building. 6. Maintenance of building facilities provided for the safety of occupants (keeping records of same) (Property Management responsibilities may be contracted out at which point a revision will be required to the Fire Safety Plan). 7. Provisions of alternate measures for safety of occupants during shutdown of fire and life safety systems. 8. Ensure the information in the Fire Safety Plan is current, and notify the Chief Fire Official regarding any changes in the approved Fire Safety Plan. 9. Designate and train sufficient alternates to replace supervisory staff (including Event/Activity Fire Wardens) during any absence. 10. Post and maintain on each floor area emergency procedures for occupants.
15 Page Training of Supervisory Staff Ongoing inspections, training and fire drills are necessary to ensure an effective fire safety program. 1. The training of all designated supervisory staff (including Event/Activity Fire Wardens), including new personnel members, is the responsibility of the Owner. 2. Training will be scheduled at the time of the fire drills and as often as necessary to ensure the supervisory staff know their responsibilities. 3. All supervisory staff (including Event/Activity Fire Wardens) must know their duties when an emergency occurs and the location and operation of all building fire and life safety systems.
16 Page CHIEF FIRE WARDEN The Superintendent has been appointed as the Chief Fire Warden for the Church and is responsible for overseeing the fire emergency procedures outlined in this Section of the Fire Safety Plan. The Superintendent is periodically on site Monday to Sunday between the hours of 9:00 a.m. to 5:00 p.m. In the event of an after hour emergency, supervisory staff may be contacted using their alternate telephone numbers (i.e., home, cell) which have been included in Section 3.1 of this report. 6.1 Chief Fire Warden Emergency Procedures 1. Leave the fire area taking any persons in the immediate vicinity with you, if safe to do so. 2. Close all doors behind you. 3. Sound the fire alarm by activating the nearest fire alarm manual pull station, if safe to do so. 4. Telephone the Toronto Fire Services by dialing from a safe location, (never assume that this has been done). Give the correct building address (3223 Kennedy Road), the location of the fire, and your name. 5. Supervise the evacuation of occupants as necessary. Advise evacuees of the congregating area in the parking lot at the East side of the Church. 6. Proceed to the main entrance to meet the Toronto Fire Services with a copy of the approved Fire Safety Plan and a master set of building keys. 7. When the Toronto Fire Services arrives, inform them of the conditions in the building and any known missing persons. 8. Assist the Toronto Fire Services as requested. 9. Do not permit occupants to re-enter the building until authorized to do so by the Toronto Fire Services. UPON HEARING A FIRE ALARM: 1. Call the Toronto Fire Services by dialing from a safe location (never assume that this has been done). Give the correct building address (3223 Kennedy Road), the location of the fire (if known), and your name. 2. Supervise the evacuation of occupants as necessary. Advise evacuees of the congregating area in the parking lot at the East side of the Church.
17 Page Proceed to the main entrance to meet the Toronto Fire Services with a copy of the approved Fire Safety Plan and a master set of building keys. 4. When the Toronto Fire Services arrives, inform them of building conditions and any known missing persons. 5. Assist the Toronto Fire Services as required. 6. Do not permit occupants to re-enter the building until authorized to do so by the Toronto Fire Services. 6.2 Chief Fire Warden Responsibilities 1. Provide alternate measures for life safety of occupants during the shut-down of fire protection equipment. 2. Be in charge of implementing the Fire Safety Plan and notify the Chief Fire Official of any changes in the plan. 3. Have a copy of the Ontario Fire Code (latest edition). 4. Establish emergency procedure to be used in case of a fire emergency. 5. Hold and participate in at least one fire drill annually in accordance with the Ontario Fire Code incorporating emergency procedures appropriate to the building (see Section 13.0). 6. Control fire hazards in the building. 7. Distribute copies of the Fire Safety Plan as outlined in Section 15.1 of this Fire Safety Plan, including one approved copy of the Fire Safety Plan which is required to be kept in the Fire Safety Plan Box located in the main entrance vestibule (see Attachment E). 8. Designate and train sufficient assistants to act in the position during any absence of personnel from the building. 9. Post in each floor area emergency procedures for occupants. 10. Complete a list to be readily available of emergency contacts (i.e., fire alarm, sprinkler contractors). This list will be updated on a regular basis (as necessary) and maintained with the Fire Safety Plan. 11. Ensure that a diagram showing the location and type of all building fire emergency systems (location of fire alarm annunciator panel, fire detection devices, etc.) is maintained. 12. Have a complete master set of building keys at all times.
18 Page Provide adequate training to other Supervisory Staff (including Event/Activity Fire Wardens). 14. Check, test, inspect and maintain all fire protection equipment as required in the Ontario Fire Code and outlined in Section 12.0 of this Fire Safety Plan. Ensure that such work is completely on schedule and that all records are retained for a minimum duration of 2 years. (This responsibility may be to a property management company which will require revision to the Fire Safety Plan). 15. If the Assembly space, or any part of the building is to be rented out for non-church usage, the renter is to be designated as an Event/Activity Fire Warden, and is to be provided with a copy of the Fire Safety Plan and be adequately trained in his/her duties. When no Fire Wardens are present for Church activities, a suitable replacement must be identified and adequately trained in his/her duties.
19 Page FIRE WARDENS The Supervisory staff outlined in Section 3.1 have been appointed as Fire Wardens and are responsible for the safe evacuation of their respective areas in a fire emergency, when at the Church. Any one Fire Warden is normally on site Monday to Saturday 9:30 a.m. to 10:30 p.m. and Sunday from 7:00 a.m. to 9:00 p.m. Fire Wardens (or delegates) will assist elderly persons or people who require assistance evacuating the building. 7.1 Fire Warden Emergency Procedures UPON DISCOVERY OF SMOKE OR FIRE: 1. Leave the fire area taking any persons in the immediate vicinity with you, if safe to do so. 2. Close all doors behind you. 3. Sound the fire alarm by activating the nearest fire alarm manual pull station. 4. Call the Toronto Fire Services by dialing from a safe location (never assume that this has been done). Provide the Toronto Fire Services with the correct building address (3223 Kennedy Road), the exact location of the fire, and your name. 5. Supervise the evacuation of occupants from your area. Advise evacuees of the designated congregating area. Assist elderly persons or persons requiring assistance in evacuating. Check washrooms. 6. Proceed to the designated congregating area (the parking lot located at the East side of the Church) (see Attachment A). 7. Assist the Toronto Fire Services as required. 8. Appoint and train someone to replace you in your absence. 4. Know the fire alarm audible signals for the building's fire alarm system. 5. Participate in fire drills.
21 Page EVENT/ACTIVITY FIRE WARDENS If the Assembly space, or any part of the building is to be rented out (or loaned out) for non- Church usage, the Chief Fire Warden or one of his alternate will be on site. The renter/event organizer or the project manager is also designated as an Event/Activity Fire Warden, and is to be provided with a copy of the Fire Safety Plan and be adequately trained in his/her duties. 8.1 Event /Activity Fire Warden Emergency Procedures UPON IDSCOVERY OF SMOKE OR FIRE: 1. Leave the fire area taking any persons in the immediate vicinity with you, if safe to do so. 2. Close all doors behind you. 3. Sound the fire alarm by activating the nearest fire alarm manual pull station. 4. Call the Toronto Fire Services by dialing from a safe location (never assume that this has been done). Provide the Toronto Fire Services with the correct building address (3223 Kennedy Road), the exact location of the fire, and your name. 5. Supervise the evacuation of occupants from your area. Assist elderly persons or persons requiring assistance in evacuating. Check washrooms. 6. Proceed to the designated congregation area (the parking lot located at the East side of the Church) (see attachment A) 7. Perform a head count of persons from your specific area. Proceed to the main entrance and inform the Chief Fire Warden, his alternate or the Toronto Fire Services of any known missing persons. 8. Assist the Toronto Fire Services as requested. 9. 23 Page OCCUPANTS 9.1 Occupant Emergency Procedures UPON DISCOVERY OF SMOKE OR FIRE: 1. Leave the fire area immediately taking any persons in the immediate vicinity with you, if safe to do so. 2. Close all doors behind you. 3. Activate the fire alarm using the nearest fire alarm manual pull station. 4. Telephone the Toronto Fire Services by dialing from a safe location (never assume that this has been done). Provide the Toronto Fire Services with the correct address of the building (3223 Kennedy Road), the exact location of the fire, and your name. 5. Use the exit doors to leave the building immediately (DO NOT USE ELEVATOR), and proceed to the designated congregating area (the parking lot located at the East side of the Church). 6. Do not return until it is declared safe to do so by the Toronto Fire Services. UPON HEARING A FIRE ALARM: 1. Leave the building immediately using the closest exit door (DO NO USE ELEVATOR), taking any persons in the immediate vicinity with you, if safe to do so. Proceed to the designated congregating area (the parking lot located at the East side of the Church). 2. Do not return until it is declared safe to do so by the Toronto Fire Services. 9.2 Occupant Responsibilities Occupants shall: 1. Control fire hazards. 2. Avoid unsafe cooking practices. 3. Do not use unsafe electrical appliances, frayed extension cords, overloaded outlets or lampwire for permanent wiring. 4. This is a non-smoking facility. Smoking is not permitted in the building. 5. Do not leave articles such as shoes, rubbers, mats, etc. in the building hallways.
24 Page Know where the fire alarm manual pull stations and exits are located. 7. Call the Toronto Fire Services by dialing whenever you need emergency assistance. 8. Know the correct address of the building address (3223 Kennedy Road).
25 Page FIRE EXTINGUISHMENT, CONTROL OR CONFINEMENT 10.1 General This is primarily the responsibility of the Toronto Fire Services. The production of toxic fumes in buildings makes firefighting potentially dangerous, particularly if a large amount of smoke is being generated. Only after ensuring that the fire alarm has been activated and the Toronto Fire Services notified, a small fire can be extinguished by experienced person(s) familiar with fire extinguisher operation, if safe to do so. If it can not be easily extinguished, leave the area and confine the fire by closing the door Fire Hazards In order to avoid fire hazards in the building, occupants are advised to: 1. Keep stairways, landings, hallways, passageways, corridors and exits clear of obstructions and refuse at all times. 2. Refrain from using unsafe electrical appliances, frayed extension cords, overloaded outlets and lampwire for permanent wiring. In general, occupants are advised to: 1. Know where the fire alarm manual pull stations and exits are located. 2. Call the Toronto Fire Services immediately whenever you need emergency assistance by dialing Know the correct address of the building, 3223 Kennedy Road. 4. Know the audible fire alarm signal and the procedures established to implement safe evacuation. 5. Report any condition which may be a fire hazard to the Chief Fire Warden or a member of the supervisory staff.
26 Page FIRE ALARM OPERATING, TESTING & RESETTING INSTRUCTIONS 11.1 General Alarm Alarm signal will sound throughout the building upon operation of any detection device. The alarm lamp on the fire alarm control panel will illuminate. Reset Reset operated detection device. Depress reset button for two seconds. All alarm indicators should extinguish. Troubles Check that the A/C power is on and operational switches are in normal position. If this is the case, depress "trouble silence" momentarily to silence the trouble zone. Call for service. Notes 1. Resetting of fire alarm panel shall only be done when authorized by the Toronto Fire Services. 2. Only qualified Fire Alarm Technicians are to work on the fire alarm system.
27 Page MAINTENANCE SCHEDULE The following list describes the required checks, inspections and tests to be performed on equipment and facilities in accordance with the Ontario Fire Code (latest edition). A written record of the tests, maintenance and corrective measures will be kept on the premises for review by the Chief Fire Official for a period of two years. The Chief Fire Warden shall be jointly responsible for ensuring that the following tests, maintenance and corrective measures are completed and recorded. Note: Maintenance may be designated to a property management company which will require revision to the Fire Safety Plan. The following definitions of check, inspect and test have been developed by the Ontario Fire Code: Check: Inspect: Test: means visual observation to ensure the device or system is in place and is not obviously damaged or obstructed. means physical examination to determine that the device or system will apparently perform in accordance with its intended function or operation. means operation of device or system to ensure that it will perform in accordance with its intended function or operation. DAILY 1. Check fire alarm system A.C. power lamps, trouble lights and check for trouble conditions ( and ). 2. Check exit lights to ensure that they have not been damaged and that they are illuminated ( / ). MONTHLY 1. Inspect all portable fire extinguishers ( ). 2. Test all fire alarm components including standby power ( ). 3. Inspect all doors in fire separations ( ). 4. Emergency lighting system, unit and lamps to be inspected and tested ( ). 5. Test sprinkler alarms using alarm test connection ( ).
28 Page 27 EVERY 2 MONTHS 1. Test sprinkler supervisory transmitters and water flow devices ( ). EVERY 6 MONTHS 1. Inspect the commercial cooking suppression system as detailed in NFPA 96, Standard for the Ventilation Control and Fire Protection of Commercial Cooking Operations". Service the equipment as required. 2. Test gate valve supervisory switches and other sprinkler and fire protection system supervisory devices ( ). ANNUALLY 1. Conduct maintenance on fire extinguishers ( ). 2. Test the fire alarm system in accordance with ULC Standard S536 by persons acceptable to the authority having jurisdiction ( ). 3. Inspect all fire dampers and fire stop flaps ( ). 4. Inspect chimneys, flues and fluepipes and clean as necessary ( ). 5. Inspect disconnect switches for mechanical air conditioning and ventilation ( ). 6. Test emergency lighting units for 30 minutes under simulated power failure conditions. Test charging conditions for voltage and current and the recovery period against the manufacturer's specifications ( ). 7. Check hoods, filters and ducts in ventilation systems that are subject to the accumulation of combustible deposits ( ). 8. Inspect exposed sprinkler system pipe hangers ( ). 9. Check all sprinkler heads ( ). 10. Test water flow on wet sprinkler systems using most remote test connection ( ). 11. Test flow of water supply of sprinkler system using main drain valve ( ). 12. Inspect auxiliary drains to prevent freezing ( ). 13. Inspect and flow test all private fire hydrants ( ).
29 Page Remove plugs or caps on fire department connection and inspect for wear, rust or obstructions. Lubricate connections ( ). 15. Conduct an annual fire drill. EVERY 6 YEARS 1. Replace the extinguishing agent in all dry chemical extinguishers ( ). EVERY 12 YEARS 1. Hydrostatically test dry chemical extinguishers ( ). AS REQUIRED 1. Recharge extinguishers after use or as indicated by an inspection or when performing maintenance. 2. Check all doors in fire separations to ensure they are closed. 3. Maintain exit signs to ensure they are clear, and legible. 4. Maintain exit lights to ensure they are illuminated and in good repair. 5. Maintain corridors free of obstructions. 6. Ensure streets, yards and private roadways provided for Fire Department access are kept clear. 7. Conduct checks, inspections and tests of special extinguishing systems (such as kitchen extinguishing system) in accordance with manufacturer's instructions.
30 Page CONDUCTING FIRE DRILLS A fire drill is required to be held at least once every 12 months for the Supervisory Staff. The following procedures shall be followed before conducting the fire drill. 1. Supervisory Personnel are required to participate. Select a group of occupants (optional) to participate in the fire drill. 2. Employees are to be notified of the exact time and date of the fire drill 48 hours in advance. 3. The Toronto Fire Services is to be notified by dialing DO NOT USE EMERGENCY NUMBER 911. Notify the Central Monitoring Station by dialing The Chief Fire Warden and Supervisory Personnel meet 1/2 hour prior to the drill for a briefing meeting. Provide copies of the fire drill report to the Supervisory Staff (attached) and have the Supervisory Staff complete and return the form. 5. Activate the fire alarm system by actuating a fire alarm manual pull station or other fire alarm initiating device. 6. Supervisory Personnel are asked by the Chief Fire Warden to return to the meeting area after the drill to discuss any deficiencies noted. 7. Once all participating personnel have successfully evacuated, the fire drill is deemed to be completed. Reset the fire alarm. Notify the Toronto Fire Services by dialing and indicate that the fire drill is complete. DO NOT USE EMERGENCY NUMBER 911. Notify the Central Monitoring Station, by dialing The Chief Fire Warden and Supervisory Personnel will meet to discuss any noted deficiencies. 9. Request Supervisory Personnel to advise the Chief Fire Warden if they experienced difficulty in hearing the alarm or if they have any suggested improvements. 10. Maintain a record of fire drills for a minimum duration of 2 years.

32 Page ALTERNATE MEASURES FOR SAFETY OF OCCUPANTS The following alternate measures will be taken by the Chief Fire Warden when any part of the fire protection equipment is inoperative. If any life safety equipment is required to be shut-down for more than 24 hours, the Toronto Fire Services shall be notified in writing and notified immediately when the equipment is operational. ALTERNATE MEASURES FOR THE FIRE ALARM SYSTEM Post notices at building entrances. Notify the Toronto Fire Services at (DO NOT USE EMERGENCY NUMBER 911). Notify the Central Monitoring Station, by dialing Fire Wardens are required to perform an hourly fire watch and will notify occupants verbally upon discovery of smoke or fire. Notify the Toronto Fire Services by dialing when repairs have been completed and systems are operational. Notify the Central Monitoring Station, by dialing Remove posted notices. ALTERNATE MEASURES FOR EMERGENCY LIGHTING Post notices at building entrances. Notify the Toronto Fire Services at (DO NOT USE EMERGENCY NUMBER 911). Notify occupants when defective system or equipment has been repaired or is operable. Notify the Toronto Fire Services by dialing when repairs have been completed and systems are operational. Remove posted notices. ALTERNATE MEASURES FOR FIRE EXTINGUISHERS Replace the fire extinguisher with a spare.
33 Page DIAGRAMS AND RECORDS 15.1 Distribution of Fire Safety Plan The following persons shall receive a copy of the Fire Safe Toronto Fire Services, Owner(s), Superintendent (Chief Fire Warden), Fire Wardens, Event/Activity Fire Wardens, and Fire Safety Plan Box (located in Church main entrance vestibule). Distribution and implementation of the approved Fire Safety Plan is the responsibility of the Chief Fire Warden. A copy of the Fire Safety Plan will be kept in the Fire Safety Plan Box located at the main entrance vestibule, as required by the City of Toronto By-Law (see Attachment E) Schematic Diagrams The following pages contain the following schematic diagrams: Site Plan, Basement Floor Plan, Ground Floor Plan, and Second Floor Plan. The drawings will be posted on each respective floor area by the exit doors. This is the responsibility of the Chief Fire Warden Records A written record shall be kept of all tests and corrective measures taken for a period of 2 years after they are made. The Chief Fire Warden is responsible for maintaining all records for the building as required by this Fire Safety Plan. These records shall be made available upon request to the Toronto Fire Services.
