2 Oral and Maxillofacial Surgery Pearls IV The OMS Pearls IV Conference faculty features six outstanding educators and practitioners in oral and maxillofacial surgery and pathology, and hours of CME credit over two days. Both days of CME also include free continental breakfasts and breaks. A special Louisiana cocktail/hors-d oeuvres reception event will be held on Thursday evening, April 9 at 6:00pm. OBJECTIVES: Describe, discuss and surgically manage the following: 1) consideration of the biopsy and biopsy techniques, 2) immune-related disorders and human papillomavirus, 3) medications-related osteonecrosis of the jaws, 4) significant odontogenic cysts and tumors, 5) significant odontogenic epithelial tumors, 6) skin tumors and residual defects, 7) sleep apnea, 8) oral carcinomas, 9) fibro-osseous lesions, and 10) salivary gland tumors. PEARLS IV HIGHLIGHTS: Three Oral and Maxillofacial Surgeons (OMS) with 125 combined years of practice and two Oral and Maxillofacial Pathologists, all outstanding in their fields, will integrate their collective and individual clinical experience in the treatment of Surgical Pathology and Reconstruction of Oral and Head/Neck area. They will discuss common and uncommon entities which are known to be problematic in diagnosis and treatment. This is not a textbook lecture course, but rather an approach to surgical pathology which includes practical considerations, mistakes, misadventures, as well as illustrations of gratifying surgery and reconstruction of oral and head/neck lesions. In addition we are pleased to have two outstanding clinicians review the current trends in diagnosing and surgically managing 1) Sleep Apnea Disorders and 2) Skin Tumors and correction of their residual defects. The Jack Kent Oral and Maxillofacial Surgery Foundation The Louisiana Society of Oral and Maxillofacial Surgeons and The LSU Oral and Maxillofacial Surgery Alumni Associations from the programs in New Orleans and Shreveport are proud to join the Jack Kent Oral and Maxillofacial Surgery Foundation (JKOMSF) in sponsoring OMS Pearls. The American Association of Oral and Maxillofacial Surgeons is also sponsoring and providing CME credit. The LSU OMS Alumni Association, through the efforts of a core group of concerned alumni, has formed the Jack Kent Oral and Maxillofacial Surgery Foundation (JK OMS Foundation) for the purpose of supporting the teaching, research, and service goals of the LSU OMS department in New Orleans. The foundation is a 501(c)(3) organization, and all donations are tax-deductible. INTENDED AUDIENCE: This activity is specifically directed towards the Oral and Maxillofacial Surgeon, but it is open to all other interested physicians, residents, medical and dental students, and allied health professionals. Neary, DDS, MD, FACS 8:15 am - 8:45 am Pearls and Perils of the Biopsy Lionel Gold, DDS The biopsy is never a casual procedure ( we ll just take a snip of tissue ), it must be carefully planned, and may be the most important surgery performed by a surgeon for his patient, himself, and the pathologist. The do s and don ts, the when,where, and if will be discussed. The professional relationship between the surgeon and the pathologist will be emphasized. Some familiarity with the pathology laboratory and especially the role of the pathologist assistant in the preparation of tissue slides is necessary. 8:45 am - 9:35 am Lesions that Surgeons Would Rather Not Treat: Immune-mediated Disorders Thomas P. Williams, DDS, MD and Kitrina Cordell, DDS, MS Patients with an array of immune-mediated, and potentially genetically linked disorders, are regularly seen for diagnosis and treatment. The variegated members of this group extend from relatively innocuous, but persistent, to life-threatening. Treatment is often difficult and unsatisfactory and offers limited success or failure. Some of the lesions that may be discussed include Lichen Planus, Lichenoid Mucositis, Pemphigus, Pemphigoid, Reiter s Syndrome, Aphthous Stomatitis, Herpes and Sjogrens s Syndrome. History, clinical features, and treatment will be discussed. 9:35 am - 10:05 am Oral and Oropharyngeal Manifestations of the Human Papillomavirus Kitrina Cordell, DDS, MS Over 100 different subtypes of the human papillomavirus (HPV) have been identified. Infection is frequently seen on the skin, genitals and within the oral cavity. A spectrum of lesions have been identified from benign to malignant. These entities include verrucas vulgaris, squamous papilloma, focal epithelial hyperplasia (Heck s disease), condyloma accuminatum and most significantly HPV-positive oropharyngeal carcinoma. Clinical features, histopathologic findings including when to request specific HPV testing from the pathologist and treatment will be discussed. 10:05 am - 10:20 am Discussion Drs. Cordell and Williams 10:20 am - 10:40 am Break 10:40 am - noon Odontogenic Keratocyst and Other Aggressive Odontogenic Cysts of the Jaws with Discussion Drs. Williams and Gold noon - 12:45 pm The biologic and clinical behavior of the relatively common cystic lesion newly termed KOT (né odontogenic keratocyst ) Nevoid Basal Cell Carcinoma Syndrome will be discussed with emphasis on management of the primary and secondary forms. Several other interesting and aggressive, less frequently occurring, cystic lesions that are not well recognized by general pathologists or surgeons will be illustrated. They may be very destructive, persistent, and capable of neoplastic changes. Lunch 12:45 pm - 2:15 pm Surgical Management of Several Significant Odontogenic Epithelial Tumors with Discussion Drs. Gold, Carlson, and Williams The evolution of the odontogenic clear-cell tumor to its recognition as odontogenic clear-cell carcinoma is a prime example of the importance of case presentation, pathologic study and clinical evidence. Unfortunately this cannot be said of the relatively common ameloblastoma which continues to be labeled a benign tumor after more than 100 years of reporting and study. Tradition and the absurd classification of this lesion by the WHO has contributed to misunderstanding and underestimation of this lesion with sometimes tragic clinical results. Treatment of these lesions will be illustrated. Despite their slow growth, these odontogenic tumors are capable of significant growth, destruction, and metastasis. Clinical malignant behavior and biologic malignancy does not always require metastasis.
5 Surgical Pathology and Reconstruction 2:15 pm - 3:30 pm Surgical Management of Several Significant Odontogenic Mesenchymal and Mixed Mesenchymal/Epithelial SATURDAY, APRIL 11 Tumors with Discussion Drs. Gold and Carlson The lesions of this group are interesting examples of faulty embryologic development and growth mechanism of teeth and their supporting tissues resulting in a spectrum of anomalies from hamartomas to benign and malignant neoplasms that produce great destruction of the jaws. Management of these lesion are illustrated. 8:00 am - 9:30 am Surgical Pearls for Skin Tumors and Residual Defects Michael F. Zide, DMD Frozen histological sections are antiquated, so the office OMS can save patients significant financial resources compared to other specialists. In office surgery, with or without intravenous sedation, and the techniques presented will benefit your patients and bolster your practice when you navigate with these soft tissue skills. 9:30 am - 10:15 am Update on Sleep Apnea for the Oral and Maxillofacial Surgeon John P. Neary, DDS, MD, FACS This presentation will focus on the latest thoughts on the pathophysiology of Obstructive Sleep Apnea and review the latest diagnostic criteria. A review of medical management, alternative management, and surgical management will be presented. Techniques of Maxillomandibular Advancement and defining a cure versus management of this chronic disease will be discussed. 10:15 am - 10:35 am Break 10:35 am - 11:45 am Atypia/Carcinoma in situ and Oral Carcinoma with Discussion Drs. Gold and Carlson 11:45 am - 12:30 pm Lunch The hyperplastic epithelial lesions, known clinically as leukoplakia, demonstrate a continuum of histologic change that are frequently reflected in clinical appearance. The microscopic alterations extend from atypia to carcinoma in situ and usually eventuate in overt carcinoma. The challenge is to halt the process before the carcinomatous stage. Management of occult disease of the cervical lymph nodes as part of early oral squamous cell carcinoma represent one of the more important aspects of the diagnosis and management of patient with oral cancer. This lecture will review the principles of neck dissection in patient with NO and N+ TNM designations. 12:30 pm - 1:30 pm Surgical Management Pearls of Fibro-Osseous Diseases with Discussion Drs. Gold and Carlson Fibro-osseous lesion is not a diagnosis. The term loosely represents a group of bone diseases that include lesions of specific etiology such as Paget s disease and hyperparathyroidism, excepting cranio-facial osseous dysplasia and others. This lecture will concentrate on fibrous dysplasia, ossifying fibroma and cemento-osseous dysplasia. The distinction between these lesions is not always recognizable or histologically specific, making imaging and growth pattern important guides to treatment. Several other lesions with perhaps more specific histologic appearance such as osteoid osteoma, benign osteoblastoma, and non-osteogenic fibroma may be illustrated. 1:30 pm - 2:30 pm Surgical Management of Oral and Extraoral Salivary Gland Tumors with Discussion Drs. Gold, Carlson, and Williams The management of salivary gland tumors is predicated on the understanding of the growth pattern, both clinical and histologic, and the exercise of sound surgical principles. The discussion also includes specific and general diagnostic and surgical management approach for patients with masses of the parotid gland. This includes a particular focus on neoplastic processes of the superficial lobe of the parotid gland. 2:30 pm - 3:15 pm Current Thinking and Treatment Pearls of Medications-Related Osteonecrosis of the Jaws (MRONJ) Eric R. Carlson, DMD, MD, FACS Osteonecrosis of the jaws associated with bisphosphonate and other anti-resorptive medication (MRONJ) has historically been a poorly understood disease process in terms of its pathophysiology, prevention, and treatment. Non-evidence based speculation have been published in the international literature and is currently being challenged. This lecture will review MRONJ with a specific focus on treatment, and specifically includes a discussion of the controversy regarding the stages of the disease that requires resection. 3:15 pm - 3:30 pm Discussion Faculty

Krahwinkel Chairman, Department of Oral and Maxillofacial Surgery, Director of Oral and Maxillofacial Surgery Residency Program and Oral/Head and Neck Oncologic Surgery Fellowship Program, University of Tennessee Medical Center, Knoxville and the University of Tennessee Cancer Institute Dr. Carlson is a graduate of University of Pennsylvania School of Dental Medicine and University of Miami School of Medicine. He completed OMS residency at Allegheny General Hospital in Pittsburg, a fellowship in Oral/ Head and Neck Tumor Surgery at University of Miami/Jackson Memorial Hospital, and a General Surgery Internship at University of Tennessee Medical Center. His expertise is in oral/head and neck tumor surgery and research interests are in molecular aspects of oral/head and neck cancer diagnosis and treatment. Dr. Carlson is Section Editor, Surgical Oncology and Reconstruction, Journal of OMS, Chair, Clinical Cancer Committee, University of Tennessee Cancer Institute, and Former Chair, Special Committee on Oral and Maxillofacial Surgery Parameters of Care, AAOMS. Kitrina Cordell, DDS, MS Associate Professor of Oral and Maxillofacial Pathology Interim Chair of the Department of Diagnostic Services LSU School of Dentistry Dr. Cordell received her DDS and MS in Oral and Maxillofacial Pathology from The Ohio State University. Prior to joining the faculty at LSUSD in 2010, she was as member of the faculty at the University of Michigan School of Dentistry for eight years where she also served as director of the Oral Pathology Biopsy Service. At LSUSD Dr. Cordell is responsible for educating dental, dental hygiene and advanced education students in clinical and didactic settings and also serves as director of LSU Oral Pathology Service, providing histopathologic diagnosis of biopsy specimens from practitioners in the dental and medical fields. Dr. Cordell is a Diplomate of the American Board of Oral and Maxillofacial Pathology and a Fellow of the Academy of Oral and Maxillofacial Pathology and was elected to a three year term as Councilor for that organization from Lionel Gold, B.A., D.D.S.
