1 Albion House offers high quality, person-centred care for men and women A unique centre of care with highly experienced staff Albion House is a 24-bedded residential care home for both adult men and women over the age of 18 years. It provides dedicated support for people with mental health issues. We accept anyone with mental health problems including those on a Community Treatment Order (CTO) and conditionally discharged 37/41 of forensic background. Albion House is located in the heart of Lewisham in south east London. We provide 24 hour care and supervision for our service users based on the principles of rehabilitation and recovery and the belief that everyone has the capacity for both. This philosophy is underpinned by the need to provide the opportunity for independent living, social inclusion and robust risk management. Our ultimate aim is to provide improved quality of life for service users and enable them to move on to more independent living accommodation and sustain their independence by providing access to flexible staff support that meets their changing needs. We strongly believe in promoting our clients rights and choices through person-centred planned care. Albion House is conveniently situated 100 yards from Lewisham High Street. It is close to all amenities including post office, bus and train links, banks and shopping centre.

Ann Perera - Manager Elizabeth van den Heever - Deputy Manager This philosophy is underpinned by the need to provide the opportunity for independent living, social inclusion and robust risk management. We strongly believe in promoting our clients rights and choices through person-centred planned care, ensuring the sustenance of their independence and providing access to flexible staff support that meets their changing needs. Our ultimate aim is to provide improved quality of life for service users that enable them to lead healthy, fulfilling lives in the wider community. The Therapy team based at nearby Bromley Road Hospital provide in reach support to Albion House, delivering a range of Occupational Therapy and Social Inclusion Interventions. If service users have transitioned to Albion House from Bromley Road, this facilitates the continuation of their individual treatment plans and therapeutic timetables as well as helping the service user feel more at ease in their new placement through having regular contact with familiar staff. Service users have commented on how this has given them a sense of continuing involvement in their recovery journey and made the transition from hospital to residential service easier to manage. For service users new to Choice Lifestyles, the Therapy team are able to offer the same range of initial assessments, treatment planning expertise and day to day input at Albion House as one might expect to be available in higher level services or rehabilitation hospitals, but within the more homely, residential setting of Albion House. 4 ADMISSION CRITERIA We accept anyone with mental health problems including those on a Community Treatment Order (CTO) and conditionally discharged 37/41 of forensic background.treatment resistant disorders/difficult to treat illness Personality Disorders A forensic background Challenging behaviours such as aggression, risk of self-harm and risk of suicide History of emotional, physical and sexual abuse Mild learning difficulties and other causes of cognitive impairments Co morbid, eating disorders, substance misuse REFERRAL PROCESS Each patient will be assessed by two members of the clinical team, if it is agreed that the patient would benefit from the service, a pre-admission report, initial care plan and social inclusion report will be generated and sent to the referring professional/care co-ordinator. Referrals can be made directly to the unit manager or via the referral form found on our website. Or contact our referral Hot-line on
5 TREATMENT Following comprehensive risk and needs assessments, service users are offered individualised programmes based on recovery and rehabilitation. We believe that the recovery model enables individuals to look beyond their mental health problems and encourages them to set new goals and develop relationships that bring a whole new meaning to their lives. Other treatment programmes include planned 1:1 sessions with their Key Workers as well as weekly Planning group. Individual and group cookery sessions and planned shopping trips are encouraged in promoting independent living. We strongly believe in delivering person-centred care. Service users are offered a choice, their autonomy is respected and encouraged to participate in decision making. FAMILY & FRIENDS The team at Albion House recognises the importance of family and friends and the role they play in recovery and encourage their involvement in their care, with the agreement of the client.
6 CLINICALLY LED TEAM The service is led by a strong clinical multi-disciplinary team who work corroboratively with the patient and community team to achieve the best outcome for the patient. The care follows the social inclusion recovery-focussed and psycho-social models of care. Outcomes measures such as the Recovery Star and HoNOs are used. Albion House takes into account specific needs around gender, ethnicity, sexuality and religion. A dedicated team will support the client to achieve their individual goals and targets by encouraging structured daily activities. The clinical team at Albion House includes: Keyworkers Nivya Jose - Shift Co-ordinator Occupational Therapist Social Inclusion Workers RMN Nurses HCA team and activity co-ordinator OUR MDT APPROACH We provide bespoke packages of care to individual service users ensuring they have regular, meaningful activities on a weekly basis. Albion House is keen to ensure all service users engage in at least 25 hours of therapy or meaningful activities per week. Service users are given an individualised activity planner and are encouraged to attend specific, targeted activities to aid their recovery. Each service user at Albion House will be offered approximately 40 hours of meaningful activities and therapies per week. We offer a wide range of Psychological Therapies, Occupational Therapy Interventions and Social Inclusion Activities. All team members work collaboratively to ensure the best possible treatment is provided. Rohan Perera
7 OT Occupational Therapy focuses on supporting service users to engage in the activities and life roles that are important and meaningful to them. Occupational Therapy works with service users to develop the skills they need to live independently and successfully move on to supported accommodation or living in the community. Examples of OT interventions may include: Independent Living Skills Assessment (ILSA Rehab) Unique assessment tool developed by Choice Lifestyles staff. Cooking skills development Communication and discussion groups Individual budgeting plans Karl Sieber - Lead Occupational Therapist Personal and environmental care support. Ward based exercise and physical activity groups Gardening Vocational skills development Working towards obtaining recognised ASDAN qualification. Computer Skills Music Appreciation Arts and Crafts Lower key ward based social activities such as Bingo, Garden Games, BBQs and Table Tennis. Emmanuel Illevbare- Asst. Occupational Therapy As part of the service philosophy of maintaining close links with therapeutic services provided by Bromley Road Hospital, residents at Albion House also have the option to engage in educational activities as part of the ASDAN programme, which has been recently developed at Bromley Road Hospital. This enables service users with a wide range of needs to work towards achieving individual tailored, externally recognised qualifications. Further information on ASDAN qualifications can be found at
8 SOCIAL INCLUSION Social Inclusion is based on the principle of the service user being part of their local community, making productive use of leave and re-establishing a balanced occupational routine. Examples of social inclusion activities include: Cinema Cooking and independent living skills Breakfast clubs and coffee mornings Bowling Engaging in college course or other educational activities. Support to attend Church, Mosque or meet any other religious/spiritual needs. Visiting areas/events of interest. Gym Swimming Snooker Walking club Many other activities are also offered based on the individual wishes and needs of the service users.
