They will liaise and work in partnership with the relevant statutory, voluntary and independent agencies, ensuring that the work of Penumbra is promoted and publicised in a positive manner. Responsibilities Ensure that holistic person centred support is delivered. Be responsible for the assessment, planning, implementation and evaluation of programmes of support. Use Penumbra s Recovery tools (e.g. I.ROC, HOPE toolkit) and NHS A.C.E. III Act as named nurse for designated people ensuring that needs are prioritised and flexible and individual preferences are given due importance. Support people in their re-integration within the local and wider community..

2 Co-ordinate and p a r t i c i p a t e in m u l t i -disciplinary meetings regarding the assessment and review of individual support plans. Ensure the m a intenance of r e l e v a n t documentation and data gathering systems. Work with people, their carers and partner agencies to ensure the provision and development of the service reflects their needs and aspirations. Responsible for the implementation of the ARBD Step Down Service operational policies. Contribute to the development of the ARBD Step Down Service by providing advice and information to partners on all areas related to operational policy. Adhere to the Nursing and Midwifery Council Codes of Conduct and Professional Accountability Ensure that the service meets National Care Standards and complies with Care Inspectorate requirements. May include outreach work across Edinburgh, Midlothian, East Lothian and West Lothian.

5 ARBD Step Down Service Information Introduction Penumbra is a leading Scottish voluntary organisation working in the field of mental health. Penumbra provides an extensive range of inclusive, recovery-focused services to support people s health and wellbeing. Penumbra has been delivering support for over 20 years to people with a diagnosis of Alcohol Related Brain Damage (ARBD) in a range of settings including housing support and supported accommodation in Glasgow, Aberdeen and Edinburgh. Following a review of the care pathway for service users of Edinburgh and the Lothians who have ARBD and who are in hospital, we are delighted to be awarded the contract to work in partnership with NHS Lothian, City of Edinburgh Council (Department of Health and Social Care) and Waverley Care to open a new, innovative service in Edinburgh - an alcohol-free 10-bedded step-down unit to provide short-term care for vulnerable men and women, aged 18+, who have a diagnosis of ARBD. This is a newly configured service for Lothian and Penumbra will work with NHS Lothian, the City of Edinburgh Council and other partners to develop and evaluate this model of service provision. Approximately 100 patients per year with ARBD are admitted into an NHS Lothian hospital and approximately patients will be transferred to the ARBD step down unit. Patients will predominately be under 65, have moderate to severe cognitive impairment and may have challenging behaviour. There will be male and female patients although the majority will be male. The emphasis on the unit is to maintain capacity to reduce delay in acute hospitals whilst providing an environment conducive to rehabilitation and recovery from the patient s cognitive impairment. Location of Service The step-down service will be provided from Milestone, 113 Oxgangs Road North, Edinburgh. Milestone provides a shared site with four bungalows, two of which deliver a blood-borne virus (BBV) respite service. The ARBD service will be provided within a self-contained unit of two bungalows separate from the BBV respite service provided within the building. The site has recently undergone considerable refurbishment.

6 Admission Criteria Admissions to the Service will be through a multi-disciplinary panel comprising of representatives of Health and Social Care, NHS Lothian and Penumbra. The service will prioritise people who; require nursing care but not in an acute setting; require convalescent care prior to their return home; have had a period of hospital admission and require further assessment in order to decide future care arrangements; have had a period of hospital stay and require further care, treatment, rehabilitation, recuperation or discharge arrangements to be made; require a residential re-ablement and rehabilitation service to facilitate a return to home; are in need of planned post-detox care and support to enable the Service User to access treatment, rehabilitation or recovery orientated services; are awaiting onward referral or care and support at point of hospital discharge and whose needs the service can meet. Service Description Purpose of Service To provide a short-term care and support service in a place where there is: Time to recover: where a person s short term needs are more than can be met at home but which do not require hospital care, the unit will provide a setting where care and support is provided in a way that assists a person to achieve outcomes important to them. Time to assess: where people require further assessment of their needs, the service will contribute to the assessment through multidisciplinary working. The service will provide monitoring and review of progress towards outcomes and the care and support the person requires. The service is not responsible for decisions over future care arrangements, but will actively contribute to assessment and decision making. Time to prepare: the service will provide the accommodation, care and support a person requires to ensure that transitions are planned. For example, transitions may include facilitating a staged return home. A flexible service: the service will work flexibly to meet the needs of people with ARBD who have short term care needs that cannot currently be met at home. For example, while house repairs are being carried out. In order to achieve its purpose, the service will provide: 24 hour care and support for an average length of stay intended to be no longer than weeks. In many cases, a short stay of 1-2 weeks may be appropriate for some patients;

7 a staff team who, in conjunction with other professionals and disciplines, will be trained to deliver short-term support to increase the person s abilities, reduce high risk behaviour and increase the person s self-efficacy; staff who are supported to deliver care which demonstrates a high level of skill, understanding and empathy in meeting the specific needs of people who use the service; a range of activities which will meet needs and outcomes identified in the Personal Care and Support plan. a flexible service which contributes to the assessment of a person s ongoing care needs through regular monitoring and review in order to achieve the outcomes important to them. high levels of consistency of staff deployment as a key objective; opportunities for occupants to access a range of community based services, allowing comprehensive assessment and review of their needs; opportunities for meaningful activity, exercise, social interaction and diversion from distress or distressing behaviour enabling people to achieve an optimum quality of life; partnership working with family, friends and unpaid carers to share information and learning in order to improve the experience of both the person and their carer on return home. The service will ensure that the carer s views and knowledge inform the delivery of care and support and allow an occupant to be seen as a whole person.
