2 Introduction Pennine Care NHS Foundation Trust is a leading provider of mental health and community services predominantly for people who live in the boroughs of Bury, Oldham, Rochdale, Stockport, Tameside and Trafford. However, some services are provided to people who live outside these areas, where appropriate. Our vision is to deliver the best possible care to patients, people and families in our local communities by working effectively with partners, to help people to live well. The vision clearly sets out our values to deliver the best possible care and looks at how we work with patients, carers, the public and other partners to develop and improve care in the communities we serve. In support of the vision, we have established a set of strategic goals which steer the organisation in the right direction: Put local people and communities first Strive for excellence Use resources wisely Be the partner of choice Be a great place to work About our rehabilitation and high support services Our Rehabilitation and High Support Directorate (RHSD), part of the Trust s Specialist Services Division, provides a range of low secure and step-down services, along with a Psychiatric Intensive Care Unit for men. These aim to support service users to reach their full potential and optimum level of independence and to enhance their life skills and community functioning. The ultimate aim is to enable them to safely integrate back into their local communities, where appropriate. Our RHSD services are delivered at nine bespoke units, which are age and gender specific (with the exception of one mixed sex unit) and are located across the Pennine Care footprint either within the community or on a hospital site. Year after year, our recovery success rates continue to be high the majority of our service users are successfully supported towards recovery and discharged into units of lesser security, supported living accommodation or to live independently We successfully deliver a reduced length of stay against the prescribed care pathway Our re-admission rates are low for all units We receive excellent feedback from our service users, their families and carers and our staff We consistently perform well during Care Quality Commission inspections We provide care closer to people s homes and families which evidence shows can improve outcomes We have significantly invested in a number of our units over the years to improve the environment and the care and support that is offered Every single one of our units is staffed by an experienced, knowledgeable and skilled multi-disciplinary team of staff Our staff turnover is low, which supports continuity of care, improved relationships between service users and staff and better outcomes overall We are also a forward thinking Trust evidenced by the many innovative schemes we have implemented. This includes: A self-harm reduction group for women: This aims to teach service users the skills to manage overwhelming thoughts and feelings and to equip our staff with the skills to better support them. The evaluation of this project highlighted a range of successful outcomes, including improvements in emotion regulation and a reduction in the number of self-injuries and unit destroying behaviour The Backdoor Music Project: This was developed directly in response to feedback from our service users. It involves them working with a music mentor to develop their singing, dancing and other creative skills, to aid their rehabilitation. This annual project, which is now in its sixth year, culminates in a show held at a community venue. The initiative was shortlisted for an NHS England Excellence in Participation Award and was highly commended for its community partnership working and innovation Buddy and Peer Support Worker Scheme: People arriving, or waiting to be admitted, to our low secure units are paired with an existing service user who acts as a buddy. The buddy can answer any questions or concerns they have and offer guidance about life on the ward. Those who have been discharged are then offered the chance to become a Peer Support Worker, which involves visiting service users before and during admission to provide them with information about what to expect. Feedback shows that this initiative offers new admissions increased hope about their recovery Finally, commissioners can be reassured that we are committed to offering transparent care packages, with no hidden costs. This supports our commitment and passion for providing our service users with the very best care and support, which relies on offering a whole person approach.

3 Pennine Care s units at a glance Our low secure units: Prospect Place 45 beds For men aged between 18 and 65 years Provides a pathway of recovery for people who have a diagnosed psychotic disorder, for example schizophrenia, and have been detained under the Mental Health Act. Many will also be misusing drugs and/or alcohol Three distinct wards supporting a clearly defined and structured pathway of recovery 2 yr Two year care pathway Provides an innovative Buddy and Peer Support Worker Scheme 96% Since 2012, the unit has supported 96 per cent of service users to be discharged to lower secure services 54% Of these, 54 per cent achieved discharge prior to the end of their prescribed care pathway Tatton Unit 16 beds For men aged 18 and 65 years admissions can be accepted for those older than 65 years, where appropriate Provides long-term care and support for people who have a severe and enduring mental health condition, such as paranoid schizophrenia, which is complicated with other mental health issues 3-5 Newly opened in 2013 providing a longer pathway for people who require low secure care Since opening, 100 per cent of service users have been discharged to placements of lesser security Of these discharges, 100 per cent achieved an early discharge from their prescribed care pathway

4 Our step-down units: Beckett Place 3-5 Bevan Place % 10 beds For women aged 18 to 65 years Provides care and support for women who have a primary diagnosis of schizophrenia, but may also have other difficulties Since the unit opened in 2010, 84 per cent of service users have been discharged into independent or supported living accommodation In the last three years, 100 per cent of service users achieved early discharge from their prescribed care pathway 16 beds Heathfield House 2 yr 90% For men aged over 50 years (average age of 64 years) Provides rehabilitation and high support to men who have a severe and enduring mental illness, such as schizophrenia Two to five year care pathway Committed to supporting ongoing learning recognised with an Excellence in Practice award from Manchester University Since 2012, 85 per cent of service users have been discharged to placements of lesser security Of these discharges, 100 per cent were supported to achieve early discharge from their prescribed care pathway 19 beds and 3 self-contained studio flats For men aged 18 to 65 years Provides intensive mental health rehabilitation and recovery in a community-based step-down environment for men who have a severe mental illness, such as schizophrenia or bi-polar disorder, and who have been difficult to engage in the past Two year care pathway 50 per cent of service users have been stepped down from secure units More than 70 per cent of service users have been successfully discharged to community placements and 20 per cent to independent living Hurst Place beds Rhodes Place 3-5 For men aged between 18 years and 65 (average age of 35) Provides step down care for people who have a severe mental illness, significant forensic history, drug use, who are Ministry Of Justice restricted or who have been detained under the Mental Health Act Receives regular positive feedback from the Care Quality Commission about the homely environment Since 2012, the unit has supported 100 per cent of service users to be discharged to placements of lesser security 8 beds Stansfield Place 2 yr For women aged between 18 years and 65 years (average age of 35) Provides step down care for people who have a severe mental illness, significant forensic history, drug use, who are Ministry Of Justice restricted or who have been detained under the Mental Health Act Receives regular positive feedback from the Care Quality Commission about the homely environment Since 2012, the unit has supported 100 per cent of service users to be discharged to places of lesser security 12 beds six male and six female located seperately For men and women aged between 18 and 65 years Provides rehabilitation for people who have a severe and enduring mental health condition, such as schizophrenia or bi-polar affective disorder Two year care pathway Situated in a quiet residential area, close to local amenities offering a large garden area with stunning views over the Pennine hills Since the unit opened, 73 per cent of service users have been successfully discharged into the community or supported accommodation

5 Our Psychiatric Intensive Care Unit: Cobden Unit 10 beds For men aged 18 to 65 years Provides a referral and repatriation service for men and women who are admitted to out of area beds Provides assessment for prison, court and police referrals 42 day 42 day care pathway Staffed by a skilled multi-disciplinary team, including a Consultant Psychiatrist, Psychologists, Doctors, Nurses and Occupational Therapists 80% 20% 80 per cent of admissions are successfully discharged back to an adult acute ward and the remaining 20 per cent are discharged to low secure services, or other specialist services Bespoke brochures A bespoke brochure is available for each unit, which provides detailed information about the recovery interventions offered, the environment, outcomes and feedback.
