2 Our Philosophy Our History The Nelson Trust believes in the possibility of change. We believe with the right care, support and encouragement, everyone has the ability to change and improve the quality of their lives. Everyone comes to us with the capacity to overcome addiction, put their past behind them, enjoy lifelong recovery, and transform their futures. Our aim is to help people gain the understanding, skills and tools they need to live independently, manage the stresses and strains of everyday life, build positive healthy relationships and progress to study, work and achieve their life s goals without any further need for mood-altering substances. In 1985, Mary and Roy Brash invested their life savings in purchasing the former Nelson Inn to establish one of the UK s first abstinence-based residential treatment centres. From these humble beginnings the Trust, an independent charity, has created a therapeutic community with a national and international reputation as a centre of excellence in addiction treatment. Today we welcome around 150 clients each year from all over Britain and beyond. We believe no-one overcomes their problems alone: peers, house-mates, buddies, mentors and volunteers with experience of recovery all work together to provide mutual support it is important for people to know they are not alone in their journey to overcome addiction. 2

4 Who is the programme for? Residential Treatment Individuals seeking an abstinence-based residential treatment programme to achieve lasting recovery from drug or alcohol addiction. Who can access this service? Adults (17+) who have a drug or alcohol addiction and who 1) Have been assessed and referred by their local specialist Substance Misuse service, for example their Drug & Alcohol Action Team (DAAT); or 2) Can fund their own treatment programme. 4
5 What do we provide? Outcomes & successes A residential addiction treatment programme combining a holistic, clientcentred approach and cognitive therapy within a therapeutic community setting where we can address the physical, mental and emotional wellbeing of people seeking to recover from addiction. The treatment service includes an integrated Education Training and Employment (ETE) programme and continuity of care through our resettlement housing service. We can work with people who have a dual mental health diagnosis, people in the criminal justice system, and we have a dedicated Women s Service for those women who would benefit from treatment in a female-only environment. The Nelson Trust reports to the National Drug Treatment Monitoring System (NDTMS) and National Alcohol Treatment Monitoring System (NATMS). Our data is analysed by the National Drug Evidence Centre (NDEC) and covers top level outcomes such as waiting times and the impact of treatment on individual clients through the completion of Treatment Outcome Profile (TOP) questionnaires. At a recent annual reunion we interviewed a number of former clients who described achieving peace and contentment, reconciliation with their families, structure and purpose, health and energy many said they never expected to enjoy life and look forward to each day until they achieved their recovery with us. Our former clients contribute to the community in a variety of ways, including their work in supporting newcomers to recovery. 6 What are my first days like? Residential Treatment On arrival our Admissions Team will welcome you. Then you will be shown around your house, where you will meet your housemates, your chosen Buddy (a I think it really helped for me, I came from a really fast drug induced haze of flashing lights and the city and it was really good for me to be able to sit in a calm environment in the country... and yes it really helps 6
7 peer who will support and show you around for the first few days), and your Recovery Worker to help you get to grips with the programme and stay focussed on your recovery. With the Recovery Team you will develop a timetable with a wide range of structured activities. The support, user friendliness and the structure around the shared housing are second to none As each day went on it became easier and easier... you re made to feel extremely welcome Central to your treatment experience is sharing a house with peers. You ll be eating together, sharing responsibility for shopping, cooking and keeping the house clean and tidy. There is a wealth of support around at all times and above all, you will find that your house and the Nelson community is a safe and friendly place to be. 7
8 Residential Treatment How does the programme work? Our programme is holistic and clientcentred. A personal recovery programme, therapeutic groups and workshops are interwoven with education, recreational activities, practical support and the day-today routines of a therapeutic community. Treatment is based on firm clinical evidence and is in line with Department of Health guidance on best treatment practice. We use a range of therapeutic techniques including cognitive behavioural therapy (CBT), motivational interviewing (MI) and mindfulness. 1 to 1 counselling is provided at an appropriate stage usually when clients have adjusted to being drug and alcohol free and their concentration span is sufficient to engage meaningfully with their counsellor. 8
9 How long does the programme last? The initial care plan is designed for a period of twelve weeks. However, individual circumstances and needs vary a lot, and some clients will want to extend their stay beyond twelve weeks, whilst others may be ready to complete earlier. Progress is reviewed regularly and clients are involved in planning their move-on in a way that meets their personal needs. They taught me a lot here, I would recommend this to anybody who wants to change their life around... if you re going down the wrong path. It put me on the right path. 9
10 Where will I stay? Residential Treatment We have four residential houses in the village of Brimscombe, near Stroud in Gloucestershire, all of which are within 5-10 minutes walk of our treatment and training centres. New clients begin treatment in Nelson House and after two or three months move on to Stonebank or East Wharf Cottage. Sports, country walks, tai chi and gardening provide an opportunity to relax and reflect. As soon as I got here, I realised how quiet it was and I found a bit of peace, first night I stayed here I remember looking up at the sky and I could actually see the stars... Stroud is readily accessible by bus and throughout the year, clients have the opportunity to take part in an outward bound day. 10

12 What else is on offer? Education Training & Employment The therapeutic programme can be emotionally demanding and tiring. To maintain a healthy balance we provide a wide range of recreational activities which include creative and artistic workshops, a seasonal Pantomime, occasional Talent Shows, and a whole programme of education and training opportunities and taster sessions at our ETE Centre. As a registered National Open College Network (NOCN) centre, we can design and deliver relevant, bespoke training courses leading to recognised qualifications. I think that was the thing that gave me the hope and the confidence that I could succeed out there, living a normal life... before I didn t really have a belief of being able to learn or get any qualifications or a job. 12

14 Women s Programme Other Services Many women come to treatment with a history of trauma and complex needs. We therefore provide a contained therapeutic environment with a higher ratio of staff to clients, where women can work together to address their substance misuse and these underlying issues safely. The Women s House has an all female staff team experienced in counselling, recovery and trauma. We provide a children s visiting facility in a self-contained flat so that women can have their children to stay overnight. Supervised contact can be accommodated. For women who resettle in Gloucestershire after completing treatment, continuing support with complex needs can be accessed through our ISIS Women s Centre in Gloucester. 14
15 Dual Diagnosis Dual Diagnosis refers to individuals who have both substance dependence and also a diagnosed mental illness. If a client is admitted for our dual diagnosis programme, we design a treatment plan appropriate to their needs. We know from experience and the evidence base that any attempt to treat either condition in isolation from the other usually results in failure. Dual diagnosis clients are not treated separately but fully integrated into the same residences, therapeutic groups and other activities alongside other clients. We provide additional resources such as staff with specialist mental health training, a horticultural therapist and close liaison with mental health professionals should medical intervention, a further assessment or medication review be required. 15
16 Resettlement Housing Other Services The Nelson Trust offers supported resettlement housing for clients as they complete their stay as a stepping stone between treatment and fully independent living. We have four houses in the local area, three are mixed gender and one is for women only. Our resettlement houses provide a safe and supportive space where clients can consolidate and develop confidence in their recovery. They can get day-to-day support from their house mates, peers in the community, local services and drug and alcohol agencies and through attending AA, NA and SMART recovery meetings. And they can feel reassured that if there are unforeseen problems along the road in that first year of recovery, expert support will be close at hand. 16
17 Family Services Our treatment programme recognises the central importance of close relationships - both in addiction and recovery. We aim to help clients to improve their understanding of family relationships, their awareness of family dynamics and communication, and to develop their confidence and their skills in managing their responsibilities as a parent, partner and family member in their recovery. Skills learned in assertiveness workshops and communication skills practice sessions help clients communicate with their family. Often clients will practice and rehearse what they want to say to family members by role playing scenarios with their group. In Family Therapy, we work with clients, their families and friends to open an honest dialogue, improve communication and develop plans for ongoing support and the future. 17
