Rights to Regional STEMI team and meeting Overview under each bill of right defining the rationale for a perfect
3 Southern Regional Update 2. Right to pre-hospital EKG 3. Right to Rapid recognition 4. Right to EMS/STEMI Provider Education 5. Right To STEMI Protocols (please see VHAC website for full presentation) Bedford Janet Blankenship reported - EMS Agencies are now transmitting at least 50% EKG. The number continues to increase annually of EMS driving to LGH ED. Peter Martin discussed the availability of Centra One if time is greater than 45 min. STEMI system. Continuation of education for EMS staff continues and appropriate use of EKG equipment. Farmville A. Cassada reported the following 3 agencies are equipped with pre hospital EKG equipment: Prince Edward, Charlotte county ad Pamplin. EKG s are transmitted by cell phone to a fax machine. In the ED with CP EMS calls Aug transmissions and Sept there were 25 transmissions Nottoway County has new monitors and plan to educate and use the equipment. Halifax J. Slabach reported EKG equipment was available and was waiting to hear from Rep from ProComm to move forward with implementation. Peter Martin informed J. Slabach of the Centra One single call process and she said they did not have the STEMI phone, but would like this for the ED to simplify processes. J. Slabach ed and had heard from the Procomm Rep and he plans to test equipment of Fir. 10/26/2012 will keep us posted. 10/ /2012
4 John Dugan recommended a meeting with a Halifax to assist in the education and purchase of equipment as needed. 11/2012 Dr. O Brien volunteered to present the STEMI Road show to EMS and ED staff. Peter Martin will work with Janelle to install the phone and review the Centra One single call process 10/2012 Dr. Srinivasan attended and is the Interventional Cardiologist at Halifax. The Cath Lab operating hours are 8:00 a.m. to 3:00 p.m. They will continue to transfer patients after or before those times. Plans to assist with education and pre hospital EKG transmissions. Dr. O Brien to notify Cardiologist of the operating hours.
5 Review Protocols Protocol changes were made as follows: To change purple phones to say STEMI Phone Halifax changes: - delete Lovenox and add Apply stat pads for Centra One. Bedford/Southside add Apply stat pads for Centra One. C. Cole will update protocols. 10/2012 Centra One. Peter Martin discussed the hot load process and its success in Halifax and Bedford. Suggested implementing this process at Centra Southside. Aileen Cassada will present this to the Southside ED staff. 11/2012 Mission Lifeline/AHA Door In/Door Out DI/DO Peter Martin asked for Gretna Medical Center to have a STEMI protocol per their request. John Dugan announced possible founding for Pre-hosp EKG equipment. Also, discussed ongoing necessity of education for first responders. Dr. O Brien and John discussed the hospital who participates in the ACTION Registry. The importance of this participation in order to show pre-hospital EKG and AMI quality data and patient outcomes for the state. This will assist in benchmarking national C. Cole and Dr. O Brien presented results for Door In/Door Out times for 2012YTD: * Bedford = 49 min * Farmville = 50 minutes * Halifax = 62 minutes Peter will provide the contact person to C.Cole Virginia has 26 PCI faculties participating in the ACTION Registry. In the future the plan is to have transfer hospitals participate in ACTION. Dr. O Brien explained the CMS goal time as 30 minutes, but for the Southern Region the goal time is 45 min., due to
6 Transfer Times C. Cole presented 2012 YTD transfer times for: * Bedford = 90 minutes * Farmville = 96 minutes * Halifax = 125 minutes geographical areas for EMS Providers. Each facility will continue to look at ways to decrease times for patients. Pre Hosp EKG D2B Times C. Cole reported that Pre hosp EKG utilization has increased each year: * 2008 = 62% * 2009 = 87% * 2010 = 92% *2011 = 94% C. Cole presented D2B times for 2012YTD NON EMS arrival and EMS arrival: * EMS = 34 min/median * Non EMS = 61 min/median 2012YTD pre hosp EKG utilization 98% Continue to track this monthly LGH D2B 2012 YTD median time is + 38 min.
