2 ADDENDUM MARCH 2015 ARTICULATION AGREEMENT Between DELAWARD COUNTY COMMUNITY COLLEGE and TEMPLE UNfVERSITY Of The Commonwealth System of Iligher Education for Associate in Science for Eealth Professions to Bachelor of Science in Eealth Information Management Delaware County Community College (DCCC) and Temple University (Temple) enter into this agreement in order to facilitate the transfer of students who have earned the Associate in Science for Health Professions at DCCC into the Temple Bachelor of Science in Health Information Management (HIM) progrun. Once students transfer to Temple University they shall be able to complete the HM program within four regular semesters of Full-time enrollmen( assuming that no remedial or repeated courses are needed, that the recommended courses are successfully completeq and all applicable Temple grade and other requirements are met to the satisfaction of Temple. Students who complete the Delaware County Community College curriculum specified in the required course equivalencies table on page 4 ofthis agreement are guaranteed the transfer credit for courses indicated in that section. Couses not listed there, but brought into Temple in transfer by a student covered by this agreemeng will be granted credit according to standard Temple-Delaware County Community College equivalencies. This agreement does not change the regular Temple policy that gants transfer credit to courses taken elsewhere only if they were completed with a grade ofc or higher. However, HIM requires a C or higher in all HIM prerequisite courses; see notes 2, 3 and 5 on page 4 ofthis agreement. Students who transfer under the terms of this agreement are responsible for meeting requirements in their major field of shrdy, and any other curricular requirements applfng to all Temple students, that were in effect at Temple at the time of their admittance to Temple. These students will abide by the policies and procedures, and any revisions thereof which apply to all Temple students. This agreement does not cover students who transfer from Delaware County Community College to Temple without an Associate in Science for Health Professions. Students who take advantage ofthis agreement are also encouraged to meet with a DCCC Transfer Advisor to veriry if they qualiff for Dual Admissions and sign the Intent to Enroll Form. TERMS AND CONDITIONS OF ADMISSION TO TEMPLE l. Students entering Temple under the terms of this agreement must meet all regular Temple admissions requirements and all applicable Temple requirements and deadlines pertaining to application for admission and payment oftuition and fees. 2. Evaluation oftransfer credits is based on a studenfs major, and the student's Temple schooucollege makes the final determination of which transfer credits are applicable to the student's degree program. Transfer evaluations may change for students who change majors. Wlenever possible, credit is allowed for courses completed more than 10 years before the date of admission. 3. Because the Delaware County Community College Associate in Science for Health Professions, covered by this program-to-program articulation agreement is also approved for GenEd-to-GenEd Transfer to Temple, students under this program agreement satisry Temple General Education (GenEd) requirements according to the terms of the Delaware County Community College-Temple GenEd-to-GenEd Transfer agreement. Their GenEd requirements are met by receipt ofthe Delaware County Community College A.S. degree. Developmental courses and courses for which a grade below C was received do not transfer to Temple, but having taken such courses or

3 received such grades does not affect an otherwise eligible student's GenEd-to-GenEd Transfer status. Students who completed the program prior to Fall 2011, must get certified by DCCC's Transfer Office. JOINT PESPONSIBILITIES FOR MAINTENANCE OF THIS AGREEMENT Delaware_County Community College will maintain and distribute information about the current form ofthis agreemen! and any changes relevant to it, to students, counselors, and faculty advisors. Temple Universi!y's College of Health Professions and Social Work will keep appropriate Temple offices and staff (including the Office ofthe Senior Vice Provost for Undergraduate Studies and ihe Admission,s office) informed ofthis agreement and any modifications thereto, will post needed DARS exceptions specified in the agre men! and will be responsible for distributing information conceming any changes. Both institutions will encourage Delaware Couty Community College students to meet with representatives of Temple University to discuss issues related to admissions and financial aid well in advance oftheir expected date ofentry, Faculty from each institution teaching in the curriculum for which this ageement has been developed will be encouraged to meet at least once anaually to discuss issues of curriculum and student development. Temple University and Delaware County Community College will collaborate when possible on research activities that will help assess the effectiveness ofthis transfer program in meeting the needs of stuients transferring from Delaware County Community College to Temple University. The Temple University College of Health Professions and Social Work and the Transfer Office at Delaware County Community College are responsible for identifuing, and communicating to each other, changes in their respective ;ourses, requirements, or policies that affect this agreement. This agreement will be in effect as ofthe Fall 2012 Semester, and will be reviewed annually by The Temple University College of Health Professions and Social Work and Transfer Office at Delaware County Community Coliege, and wili automatically be renewed rmtil superseded by a new agreement or formally tenninated. Revisions in the co-uise equivalency lists may be made with theverbal consent ofthe representatives ofthe two schools; major revisions to this agreement require their signatures on a letter of agreement or a revised version ofthe agreement. Ail revisions, however, require that documentation is sent and approved by the Office ofthe Senior Vice Provo-st for Undergraduate Studies at Temple University to ensure that updates are made to the appropriate systems in order to effectively-implement this ".geel"n!. Either institution mayterminate this agreement at any time by written notice at least one year in advance of the effective date oftermination. Should this agreement be terminated, ii is understood that the termination will not apply to students already accepted to Temple University under the terms of this agreement, but not yet enrolled in classes at Temple. Delaware County Community College_and Temple University understand that they shall act in an independent capacity in the performance of this agreement, and shall not be considered as employees, agents, affiliates, or subsidiaries ofiach other. Neither Temple nor DelawareCounty Community College have the righi to bind or obligate one another in any manner inconsistent with or unrelated to this agreement. This agreement is govemed by the lais ofthe Common*""lth of Pennsylvania.

Studenls sfuould select AHM 233, DPR 100 and either a Humanities or Social Science elective for the three open electives. AHM 233 firlfills a rnajor requirement at Temple. Students transferring without these courses will need additional semesters at Temple to complete the degree. Temple's Health Information Management Deparment will accept by DARS exception DCCC's DPR 100, as fuifilling the CE 1055 requirernent for the major. A gnde of C or higher is required for all HM prerequisites. Therefore a C or higher is needed in DCCC's DPR 100 (or a passing score on HM's Computer Competenry Exam). Shrdents who complete the A.S. for Health Professions are included in the DCCC-Temple GenEd-to-GedEd Transfer Agreement and therefore bave satisfied all of the GeDEd requirements at Temple. Students should work with theirdccc tansf,er advisorto select approved cources to fotell the DCCC degree rcquircmene Studenr who completed the program prior to Fall 201l, mustbe certified by DCCC's Transfer Offrce.
