1 EMERGENCY MANAGEMENT PLAN The following criteria are to be used when developing a comprehensive emergency management plan for your location. These criteria are not intended to limit or exclude additional materials you may decide to include and to satisfy special issues deemed important. I. 3 III. DEFINITION OF POLICIES, PROCEDURES, RESPONSIBILITIES AND ACTIONS

4 At a minimum, the location plan needs to address all of the topics covered under this section. A. Community Coordination It is extremely important that your location be actively involved with your local community in preparing and planning for emergencies/disasters that may occur in your local area. Involvement with planning should extend to your state agencies as well. City, County, and State Emergency Management Services can be an invaluable resource for your location when putting together your emergency/disaster plan, as well as a valuable partner in the event a disaster or emergency should occur. Develop that partnership with your local and state EMS agencies before, during and after the development of your Emergency Management Plan. 1. Describe the role your location will play in your local community emergency/disaster plan 2. Describe the resources that will be available to your location through your local/state EMS agencies, and how you will keep abreast of any changes with these identified resources (transportation, shelters, supply caches, volunteers, etc.). 3. How will your location communicate with these agencies during the emergency/disaster event? 4. If resources are available through these agencies, describe the process necessary to access them. 5. Provide a plan for communication touch points of key stakeholders within this area of focus. If your local community has meetings on a frequent basis please identify how frequent those meetings are. If your local community does not meet on a frequent basis to discuss emergency/disaster planning please provide details of how your location will maintain an active role with community involvement. B. Direction and Control Direction control provides the basis for decision making and identifies who has the authority to make decisions for the location. 5 2. Identify the chain of command to ensure continuous authority in key positions: Work Work Work 3. State the procedure to activate staffing in emergency situations. For example: Posting of call list. Who is in charge of initiating emergency call list? List must be updated every time there is a change in staff/leadership. Department heads should call their own staff. Include volunteers on your list. C. Notification

6 Procedures must be in place for the location to receive timely information on impending threats and the alerting of decision makers, staff and clients of potential emergency conditions. 1. Define how the location will receive warnings, to include off hours and weekends/holidays. Examples: National Weather Service radio with battery backup, sheriff's office notifies location, etc. 2. Identify the location's 24-hour contact number, if different than number listed in the introduction. Example: Manager s home phone. 3. Define how key staff will be alerted. Example: Calling tree initiated by director, manager, etc. 4. Define the procedures for reporting to work for key staff. Example: Calling tree workers present identification. 5. Define how residents/clients will be alerted and precautionary measures that will be taken. Examples: Alarm, announcement and staff. 6. Identify alternative means of notification should the primary system fail. Examples: Cell phone list, citizen's band radio, civil defense, call tree, sheriff's office. D. Evacuation Describe the roles, responsibilities and procedures for the evacuation of residents/clients from the location. & 1. Identify the key person and backups responsible for implementing the evacuation procedures. 2. Identify evacuation routes that will be used and secondary routes should the primary route be impassable. 3. Specify the amount of time it will take to successfully evacuate. Keep in mind that in hurricane evacuations, all movement should be completed before the arrival of tropical storm winds (40-mph winds). 4. Establish procedures for ensuring all clients, employees and visitors are accounted for and are out of the building. 5. Secure the building. Call local law enforcement. E. Re-Entry Once a location has been evacuated, procedures need to be in place for allowing staff/clients to reenter the location. 1. Identify who is the responsible person(s) for authorizing re-entry. Identify whether this authority comes from management, or another entity. 2. Identify procedures for inspecting the location to ensure it is structurally sound.

7 IV. INFORMATION, TRAINING AND EXERCISE This section shall identify the procedures for increasing employee awareness of possible emergency situations and provide training on their emergency roles before, during and after a disaster. Employee participation must be documented in employee's continuing education file. A. Identify how key emergency management plan staff will be instructed in their emergency roles during non-emergency times. B. Identify the disaster training for employees regarding their disaster-related roles. C. Identify a schedule for conducting exercises/drills of all or portions of the disaster plan on an annual basis or other applicable schedule per service line and governing regulations. It is recommended that drills be conducted at least annually for those disasters that are relevant to the region. D. Establish procedures for correcting deficiencies noted during training exercise, i.e., through the safety committee. E. Describe how your location performs mandatory testing of its emergency generators. Include frequency, and duration of your generator testing, as well as how/where the testing is documented. Attach copies of this documentation to this plan (Section VI). V. APPENDIX The following information should be included here as part of the appendix, if not included within the body of the plan or is in addition to what is in the body of the plan. A. Roster of employees and companies with key disaster roles: 1. List the names, addresses and telephone numbers of all staff with disaster-related roles. 2. List the name of the company, contact person, telephone number and address of emergency service providers such as transportation, emergency power, fuel, food, water, police, fire, Red Cross, etc.

8 B. Agreements and Understandings Provide copies of any mutual aid agreement entered into pursuant to the fulfillment of this plan. This is to include reciprocal host facility agreements, transportation agreements, current vendor agreements, or any other agreement needed to ensure the operational integrity of this plan. C. Evacuation Route Map A map of the evacuation routes. Plan an alternate route in the event the primary route is not passable. D. Support Material 1. Any additional material needed to support the information provided in this plan. 2. Copy of the location's fire safety plan that is approved by the local fire department. E. Copies of Emergency Plan A copy of this plan should be kept at a location onsite that is easily accessible by key personnel. A copy should be shared with local police, fire and civil defense and with any organization designated to assist in evacuation. VI. ATTACHMENTS
