Central Avenue Marshfield, WI Project: Additional Asbestos Sampling City of Marshfield 111 W. 3rd Street 110 W. 2 nd Street NorthStar No: NorthStar Environmental Testing, LLC (NorthStar) was requested to perform additional sampling and material confirmation within two commercial buildings prior to demolition. A summary of additional sampling and analysis results follows this report. A copy of the City of Marshfield s Addendum #1 (RFI Q&A) with answers to questions that were submitted by one of the bidding abatement contractors (KPH) is also attached to this report. Follow-Up Site Summary for 111 W. 3rd Street: The following building materials should be presumed ACM, and abated prior to demoltion: - boiler room fire door (south wall, exterior door) - vault door in room electrical buss bar components located in room 102 The following is a summary of items questioned for 111 W. 3rd Street during the walk through: 1. Exterior stucco window panels were sampled and confirmed non-acm. 2. Room 103 had been previously abated. Subsequent sampling of the remaining plaster ceiling showed for asbestos. A total of 7 samples have been collected and analyzed. 3. No vermiculite was found within exterior wall concrete block cores. 4. No asbestos was found on metal ductwork located in the closet of room No suspect asbestos was found on or behind any univent heat units. 6. Quantity of acm ceiling spray within Hall 110 may appear incorrect only because of how the various adjacent rooms were quantified. Overall quantity is within normal limits (subject to verification by bidding contractors). 7. Crawlspace does not appear to have any visible or accessible pipe insulation.

2 Follow-Up Site Summary for 110 W. 2nd Street: Addendum #1 The following is a summary of items questioned for 110 W. 2nd Street during the walk through: 1. It does not appear that floor tile extends underneath partition walls (though additional destructive investigation will be required by abatement contractor while under regulated area or containment). 2. Window caulking locations were confirmed. Abatement is limited to the two windows on the north wall of the building. 3. One fire door should be included in the abatement quantification (boiler room). 4 ADDENDUM #1 RFI Q & A Issued 7/3/ Is this a prevailing wage project and if so what is the prevailing wage? There are no Prevailing Wages required for this project. 2. Is the floor tile and mastic being demolished in place? No, these need to be removed under this project. 3. Is the Roofing material being demolished in place? Yes, the roofing materials will be removed by the demolition contractor. 4. Is the Joint compound material being demolished in place? No, this material needs to be removed under this project. 5. Is the window seam caulk being demolished in place? No, this material needs to be removed under this project. 6. Is the black adhesive under carpeting and behind cork baseboard being demolished in place? No, this material needs to be removed under this project. 7. Is there any floor tile and mastic located in basement areas? No flooring material in basement areas. 8. Does the floor tile and mastic go beneath any fixed cabinets or interior improvements? This is unknown, contractor should verify. 9. Is there any black mastic or positive adhesives beneath floor leveling compound I thin set? This is unknown. 10. Are we able to wrap pipe insulation and cut out piping? Yes, the Contractor awarded the contract for this project may use this method of abatement. 11. Will the pipe be drained prior to abatement activities? This is unknown. 12. Survey limitations state: sample results, quantities and recommendations are limited to areas that were accessible to us during the investigation. Additional presumed-acbm that may have been located in spaces not accessible during our investigation, hidden from view, or not sampled at the client's request may require additional sampling prior to disturbance activities. Instructions and Specification (12): The ACM quantity estimates listed in the testing reports that are supplied as part of the bid documents are for information purposes only. Should additional asbestos be found (including asbestos which the owner did not notify the contractor to remove during any walk-through, concealed asbestos, or asbestos not listed in bid documents) Contractor shall do so as part of this project. Contractor is responsible to determine all actual ACM quantities in both buildings that are needed to be removed. Contractor shall include cost for all ACM abatement and removal in their bid based upon their own observations and testing. Questions: If NorthStar Environmental was not able to identify all materials during their inspection how do you think the abatement contractor is going to be able to do what they were not able to do? That is up to you to determine if there is more ACM than the Owner s supplied reports. Prior to bids being due, if you think there is an ACM issue large enough or different enough that may need to be addressed, then an addendum may be in order. Contact the Owner at least 5 business days prior to bids being due so that an addendum may be issued if it is deemed necessary to do so by the Owner.

5 13. Will the abatement contactor be allowed to conduct a destructive investigation at the bid walk through or after? Yes for both. Please contact the Building Services Secretary, Cheryl, for an appointment. 14. Will the abatement contractor be allowed to conduct an inspection above the ceiling tile and wall cavities to verify quantities of pipe insulation? Yes 15. During the site walk though, if we identify a suspect material that has not been tested is it our responsibility to test prior to our bid and include in our base bid? Yes, with the exception of material noted in the NorthStar report of additional testing (performed 6/27, reported 6/30). 16. Is E-verification required for this project? We are not sure what is meant by this question. 17. Is there an access panel to view above the asbestos containing acoustical spray-on ceiling coating to see what the MEP looks like above and quantify pipe insulation and/or other suspect materials? This is unknown. 18. Will the electrical components be de-energized prior to abatement activities? The electrical supply to these buildings has already been disconnected. 19. Is there anything with in the structures that has to be protected or salvaged? In the northwest corner of the 111 W. 3 rd St. property known as the News Herald Building there is a dock lift assembly that shall not be damaged and needs protection from such. 20. Were the structure's occupied when inspection was conducted or vacant? Both structures were vacant at the time of the environmental testing. 21. In the inspection report for 110 West 2 nd St., under assumed ACM, lists a fire door but under presumed ACBM there is no mention of fire doors. If fire doors need to be assumed are we to assume every door with in the building and how many of them are there? Or are we allowed to conduct destructive inspection on all doors? 110 W 2 nd St. apparently has a single fire door of the old steel clad type. It is labeled and was the access to the furnace and electrical room. No other doors are needed to be assumed to have ACM within 110 West 2 nd St. 22. In the inspection report for 111West 3rd Street the doors were tested and results were negative. One door is presumed to be ACM 111 West 3 rd St. (boiler room exterior door, and vault door). 23. Is power and water available on the site? Per the Instructions and Specifications that were part of the bid documents, NO. Contractor shall supply all required electrical, water, and sanitary facilities as needed to complete the project. 24. If exterior windows or doors need to be removed to complete removal of ACM caulks or other materials, is the Contractor responsible to board up or enclose these openings? Yes, the Contractor is responsible to install plywood or OSB on wood support framing made out of 2 or larger nominal dimensional lumber. Other suitable means may be used. The intent is to enclose the opening so that access to the building(s) is limited. Owner needs to approve of all such enclosure methods and materials. 25. Can the Contractor use a hydrant for supply of water? Such use is only with the approval of Marshfield Utilities. Contact Dave Wasserburger at to see if this is possible. Please note that backflow prevention is required and a plumbing permit and inspection may be required. 26. The 111 W 3 rd St. has a walk in vault. Has the door been tested for ACM? The vault door has not been tested and will be assumed to be containing ACM. The Contractor shall be responsible for testing or proper disposal of this door.
