1 1st Revised Sheet 1 Delaware LLC Cancels Original Sheet 1 A. DIAL TONE LINE CONNECTION CHARGES 1. DESCRIPTION The Dial Tone Line Connection Charge is that charge that applies for arranging an exchange line to provide voice grade service between the Central Office and the Rate Demarcation Point (RDP). The Dial Tone Line Connection Charge includes, but is not limited to, making or changing connections in a Central Office, and making or changing connections in distribution facilities, necessary cross connections, normal placement of the Network Interface Device (NID) if necessary and line transfers between the Central Office and the RDP. The definitions of RDP and NID are specified in Section 20B of this Product Guide. The Dial Tone Line Connection Charge applies for the connection of the following: a. Announcement Lines b. Attendant Lines c. Customer Direct Lines d. Time and Charge Special Access Trunks e. Central Office Multiple Terminations f. Exchange Dial Tone Lines including Intrastate and Interstate Foreign Exchange g. Trunk Lines h. Network Controlled Inmate Line (NCIL) i. Network Controlled Coin Line (NCCL) j. Network Controlled Non-Coin Line (NCNL) 2. REGULATIONS a. One Dial Tone Line Connection Charge applies for each line connected or changed, i.e., coin to non-coin, Foreign Exchange to local exchange, etc. This charge includes the normal placement of a NID and/or a protector if necessary. Exceptions are listed in b. following. b. Dial Tone Line Connection Charges do not apply under the following conditions: (1) When dial tone lines or trunks are temporarily suspended or restored. (2) When dial tone line numbers (telephone numbers) are changed within the same Central Office. (See Paragraph B.l.a) (3) When Touch-Tone and/or Custom Calling services are added to existing service. (4) When exchange dial tone lines are changed to trunk lines and vice versa. c. Customers requesting rearrangements of outside wiring, including the Dial Tone Line, protector or NID, shall be billed for such work in accordance with Sections 16 or 20B of this Product Guide. 3. CHARGES The Dial Tone Line Connection Charge for each line is as follows: Residence Business NCIL and Service Service NCCL NCNL and COCOT $35.96 $60.94 $75.00 $60.94
2 1st Revised Sheet 2 Delaware LLC Cancels Original Sheet 2 B. MISCELLANEOUS NONRECURRING CHARGES 1. NONRECURRING CHARGES APPLY TO THE FOLLOWING: a. Customer Requested Toll Restriction Voluntary Toll Restriction At the customer's request, Verizon Delaware LLC will restrict an individual residence or business line, except Centrex customers, from access to the Verizon Delaware LLC toll network as well as the interexchange carrier toll network, where facilities permit. The nonrecurring charge will apply to each line at the time of restriction. A separate charge will apply at the time of restoral. A restriction charge will also apply when a customer whose toll service was suspended for nonpayment of toll charges pays all outstanding charges and requests the Total Toll Restriction option. Any Verizon I.Q. Calling Cards associated with the customer requested toll restricted line will remain valid billing vehicles for both local and toll calls. Two County Calling Plan customers are not eligible for Customer Requested Toll Restriction. b. Lifeline Voluntary Toll Restriction Lifeline customers may request Lifeline Voluntary Toll Restrictions which will restrict the Lifeline customer from access to Verizon Delaware s toll network as well as the interexchange carrier toll network, where facilities permit. A nonrecurring charge will not apply to establish Lifeline Voluntary Toll Restriction. Any Verizon I.Q. Calling Cards associated with the customer-requested Lifeline Voluntary Toll Restriction will be suspended. When a Lifeline customer requests restoral of toll services, a charge will apply, as outlined in 1.c. below.
3 Delaware LLC Original Sheet 3 B. MISCELLANEOUS NONRECURRING CHARGES (Cont d) 1. NONRECURRING CHARGES APPLY TO THE FOLLOWING: (Cont d) c. Toll Service Restoral (M) When a customer whose access to Toll Service has been voluntarily restricted, a nonrecurring Toll Service Restoral Charge will apply to each line when the customer requests that toll service be restored. The Telephone Company may require payment of the Toll Service Restoral Charge in advance of the actual restoral of toll service. This charge is not subject to the Optional Payment Plan for nonrecurring charges described in Section 1B of this Guide. d. Telephone Number Change Charge - Customers requesting a change of their Dial Tone Line telephone number will incur a Telephone Number Change Charge unless a Dial Tone Line Connection Charge is applicable as specified in A.2. This charge applies for each telephone line number changed. (M) (N) e. Temporary Suspension Charge - Customers requesting temporary suspension of their service will incur this charge at the time of the suspension. There will be no charge upon the subsequent restoral of service. In addition, Premises Work Charges will also apply when a premises visit is required in connection with the suspension of service or equipment. f. Transfer of Billing Name Charge - This charge applies when one party contracts for the service that was previously contracted for by a second party for the use of the first party; or when one party contracts for the service for a second party; which had previously been contracted for by the second party in his own name. Also, when a business concern (i.e., individual partner-ship, syndicate or corporation) with unchanged personnel contracts for its existing service under a new name; or when one or more members of a business concern continue the business of the old concern and contract for the service of the old concern under a new name. Exceptions following. g. 900 Blocking Option - is provided at the customer's request and gives the customer the ability to restrict calls from his/her exchange access line to 900 service access codes. This service is provided for use with individual residence and business lines and multiline business and Centrex lines where facilities permit. The initial blocking of a customer's line or lines will be provided at no charge to the customer. Subsequent blocking (after an intervening elimination of the Blocking Option at the customer's request) will be subject to the non-recurring charge set forth in B.3.d. following. There is no charge for unblocking except where they are specifically identified in this or other Sections of the Product Guide. Feature Processing Service Changes, as defined in the Guide for Detariffed Services - Competitive, apply to Centrex CustoPak Service for the 900 Blocking/Unblocking option. This option blocks access to all 900 services reached by either dialing NXX+XXXX or using a Verizon Delaware LLC operator. (N) (M) Material on this page formerly appeared on Page 2. (N)

Toll Service Restoral When a customer whose access to Toll Service has been restricted due to nonpayment of toll charges, as described in Section 1, Paragraph 12, of this Product Guide, a non-recurring Toll Service Restoral Charge set forth in Paragraph B.3.e. following will apply to each line when the customer requests that toll service be restored. The Telephone Company may require payment of the Toll Service Restoral Charge in advance of the actual restoral of toll service. This charge is not subject to the Option Payment Plan for non-recurring charges described in Section 1A of this Product Guide. i. Local Exchange Service Restoral When a customer's local exchange service has been suspended because of non-payment of charges as described in Section 1, Paragraph 12, of this Product Guide, a non-recurring Service Restoral Charge set forth in paragraph B.3.f. following will apply to each line restored. In the event that Local Exchange Service is terminated following suspension, the Dial Tone Line Connection Charge as described in A.1. preceding is applicable in lieu of the Service Restoral Charge to reestablish service. Premises Work Charges may also be applicable. The Telephone Company may require payment of the Service Restoral Charge in advance of the actual restoral of local exchange service. This charge is not subject to the Option Payment Plan for nonrecurring charges described in Section 1A of this Product Guide. j. Service Change Charge This charge applies when a customer (Residence, Business or their agent) moves the location of their service on the customer's side of the Telephone Company's Rate Demarcation Point (RDP), to a location within the same building or single tenant campus (i.e., multi-tenant apartment building, dormitories, nursing homes, etc.). The Service Change Charge applies when the move requires a change in the customer's main telephone service or location address and applies to customers with Listed, Non-Listed or Non-Published Telephone Number Service. The charge will be applied on a per-order basis.
