The Company contributes to the Colorado High Cost Support Mechanism as required under 4 Code of Colorado Regulations A surcharge for the High Cost Support Mechanism will appear as a line item on all monthly bills. Monthly Surcharge is 2.9% (I) The Colorado Universal Service Charge may be changed on a quarterly basis. Advice No Issued June 1, 2011 Effective July 1, 2011

No. 14 is established to provide standardized schedules of tariff rules, regulations, rates, services and concurrence for CenturyTel of Eagle, Inc. and CenturyTel of Colorado, Inc. (T) II. RATES, CHARGES, RULES AND REGULATIONS Rates, charges, rules and regulations stated herein apply uniformly to all exchanges unless otherwise specified. Basic Exchange Access Service and other offerings which are not universal to all exchanges are noted as such and they shall apply only to the exchange where applicable. III. 16 Original Definitions Sheet No. 1 Access Line DEFINITIONS All of the Central Office equipment and outside plant facilities between the Central Office and the subscriber's premises, including a Network Interface Device (NID), that are needed to provide access to the telephone network. This includes dial tone and access to enhanced services. Actual Cost The cost of materials, labor and necessary overhead actually incurred by the Telephone Company to complete a particular project or task. Additional Listing Any listing of a name or information in the directory or Company's information records in connection with a customer's telephone number beyond that to which is normally provided. Air Line Mileage The shortest distance between the points involved. Applicant A person or other legal entity who (1) applies for service with the Company or (2) reapplies for service at a new or existing location after permanent disconnection. 17 Original Definitions Sheet No. 2 Automatic Dialing and Announcement Service DEFINITIONS A device that can be programmed to dial telephone numbers, randomly or in a predetermined sequence, and play a recorded message when a call is answered. Base Rate Area The developed position or positions within an exchange service area as stated in this Tariff. Service in this area is generally furnished at uniform rates without charges that vary with distance from the Central Office. Basic Local Exchange Service The furnishing of telecommunications services which provides for local dial tone, access line and local usage necessary to place or receive a call within any exchange area. Basic Termination Charge A non-recurring charge which applies if service or equipment is not retained for a specified period. A basic termination charge reduces each month that service is retained in an amount proportionate to the total number of months in the specified period. 18 Original Definitions Sheet No. 3 Billing Date The latest of: 1. Date stated on billing, 2. Date billing was placed in the mail. Building DEFINITIONS A structure under one roof, or two or more structures under separate roofs but connected by enclosed passageways, not crossing a public thoroughfare other than an alley, in which telephone wires or cables may be safely run. Pipes and conduits are not considered enclosed passageways. Business Service Business service is Basic Exchange Access Service furnished to customers whose actual or obvious use of the service is for conducting a business, trade, or profession or whose use of the service is obviously not confined to domestic use. Calls Customer's telecommunications messages. Central Office The inside plant of the Company as an operating unit, including the switch or other facilities used to establish connections between customer lines or between customer lines and trunk or toll lines to other Central Offices within the same or at other exchanges. 19 Original Definitions Sheet No. 4 Central Office Connecting Facility DEFINITIONS A facility furnished to another common carrier by the Company between the terminal location of the other common carrier and a point of connection on the Company premises for intra- or interstate use. Channel A transmission path for telecommunications between two points. It may refer to as a one-way path or, when paths in the two directions are always associated, a two-way path. Generally, a channel is the smallest subdivision of a transmission system by means of which a single type of communications service is provided. A voice grade transmission channel has a normal bandwidth of 4000Hz. Circuit A channel that is used as a transmission path of electrical or light wave energy in the furnishing of telecommunications service. Class of Service The various categories of service generally available to the customer: business, residence, public and semi-public. 20 Original Definitions Sheet No. 5 Communications Systems DEFINITIONS A combination of equipment and facilities which fulfills the communication requirements of a customer and when not connected to the local exchange network is capable of communications between customer-provided terminal equipment. Company CenturyTel of Eagle, Inc. and CenturyTel of Colorado, Inc., the Companies engaged in the business of furnishing telecommunications service to the public under the jurisdiction of the Commission, and for which all or part of these services fall within the provision of Section (2) C.R.S. Pursuant to C.R.S (1), the Company is a local exchange carrier (LEC). Connecting Arrangement The equipment provided by the customer which is used to accomplish the direct electrical connection of customer-provided facilities with the Company-provided facilities. Connecting Company A corporation, association, partnership or individual owning or operating one or more exchanges and with whom telecommunications traffic is interchanged. Connection Charge (See Access Line Installation Charge.) Contiguous Exchanges Two exchanges which share a common boundary. Advice No Amended Issued July 18, 1998 Effective September 1, 1998

21 Original Definitions Sheet No. 6 Continuous Property DEFINITIONS Property owned or leased and occupied by a customer, which is not separated by public thoroughfare or by property occupied by others. Contract The service application agreement between a customer and the Company under which service and/or facilities are furnished in accordance with the provisions of applicable tariffs and under special conditions. Cost (See Actual Cost.) Custom Calling Features A term describing special features provided from digital central offices or where facilities permit. - Call Forwarding A function which allows incoming calls to be advanced to another telephone number in the same or different exchange. The number to which the calls are advanced by the customer may be changed as required. - Call Waiting A tone provided to the party using the telephone to indicate another call is waiting on the line. Successive transfers between calling parties can be accomplished through switchhook operations. - Speed Calling A feature allowing a telephone user to reach frequently called numbers by abbreviated dialing. - Three Way Calling A feature providing the capability to add a third party to an existing conversation Advice No Amended Issued July 18, 1998 Effective September 1, 1998

22 Original Definitions Sheet No. 7 Customer DEFINITIONS A person or legal entity who has applied for, been accepted, is currently receiving telecommunications service and is responsible for payment of the bill. Customer Premises Equipment (CPE) Telecommunication devices, equipment and associated wiring provided by the customer and located on the customer's side of the protector/network Interface Device (NID). Customer Premises Inside Wiring (CPIW) Wire for telecommunications purposes which is provided by the customer and is located on the customer's premises. Customer Premises Inside Wire begins on the customer's side of the Network Interface Device (NID). Customer Trouble Report Any oral or written report from a customer or user of telecommunications services relating to a physical defect or to difficulty or dissatisfaction with the operation of the Company's facilities. 23 Original Definitions Sheet No. 8 Date of Presentation DEFINITIONS The date upon which a bill or notice is mailed, postage prepaid, in a sealed envelope properly addressed to the customer, or if not mailed, the date upon which that bill or notice is presented to the customer by a representative of the Company. Demarcation Point The point of interconnection between Telephone Company communications facilities and customer-provided terminal equipment, protective apparatus or wiring at a customer's premises. The network interface or demarcation point shall be located on the customer's side of the Telephone Company's protector, or the equivalent thereof, where a protector is not employed, as provided for under the Telephone Company's non-discriminatory standard operating practices. Deposit A cash payment made by the customer to establish or reestablish credit with the Company. Deregulated Service and/or Equipment Service and/or equipment no longer included in this tariff and not required to be included by order of the Colorado Public Utilities Commission, by statute or Federal preemption of state regulation. Centrex Line A station line providing access to the network, consisting of all facilities, including the Centrex line number, intercommunication, and outside plant facilities from the Centrex switching equipment to the network interface device on the customer's premise. Associated software features are deregulated and are not included in the tariffed rates for Centrex Lines but are required. Centrex COL/Trunk A trunk that provides pooled access to the network; (i.e., the equivalency of physical PABX or key system type trunking). 24 Original Definitions Sheet No. 9 Direct Electrical Connection DEFINITIONS Denotes a physical connection of the electrical conductors in the communications path. Direct Inward Dialing A service provided to PBX customers which permits both locally dialed and toll calls to be directed to PBX stations without the assistance of an operator. Directory Listings Essential information in the telephone directory or information records of the Company whereby telephone users may ascertain the telephone number of a listed customer. Disconnection Discontinuance of service made at the request of the customer or at the option of the Telephone Company for nonpayment of service or other tariff violations. Drop Wire Wires between an open wire lead, aerial or underground cable terminal and the point of entrance to the building in which the customer's telecommunications service is located. Emergency Reporting Service (911) An arrangement provided to one or more political subdivisions for the purpose of receiving reports of emergencies from the general public within the designated serving area. 25 Original Definitions Sheet No. 10 Exchange DEFINITIONS The entire telecommunications plant and facilities used in providing telecommunications service located in a defined area. Exchange Access Line (See Access Line.) Exchange Area A geographical area established by the Commission which consists of one or more central offices together with associated facilities which are used in providing basic local exchange service. Exchange Boundary The limiting lines as specified on exchange maps or otherwise which defines the bounds of an exchange. Exchange Service Telecommunications service furnished between customer stations located within the same local service area. (See Basic Exchange Access Service.) Extended Area Service (EAS) An exchange service in which the local calling area, i.e., the customer may call without the payment of a toll charge, includes one or more other exchange areas in addition to the normal serving exchange, as specified in the basic local exchange tariff. 26 Original Definitions Sheet No. 11 Facilities DEFINITIONS Supplemental equipment, apparatus, wiring, poles, cables and other materials and mechanisms necessary to, or furnished in connection with telecommunications service. Facility Reservation The reservation of Access Lines or telephone numbers for customers for a limited period of time. Flat Rate Service Telecommunications service furnished at a fixed monthly recurring rate without separate billing for the number, time of day, distance or duration of calls. Foreign Exchange Directory Listing An alphabetical listing in the directory of an exchange other than the exchange in which a subscriber is furnished local service. Jurisdictional Service Any telecommunications service subject to the authority of the Public Utilities Commission of Colorado and the statutes of the State of Colorado. LATA A local Access and Transport Area which has been designated in Colorado. A LATA may encompass more than one contiguous local exchange service area in Colorado which serves a common social, economic or other purposes, even when that area transcends municipal or other governmental boundaries. Line Extension The outside plant required in addition to existing facilities to render telecommunications service in areas without service. Link-Up America A national consumer education and outreach program designed to promote universal service to low income households and to get those who do not have service on to the telecommunications network. Local Exchange The area within the boundaries of exchange maps filed with and approved by the Commission. Local Exchange Access Service Telephone service furnished between customer's stations located within the same local service area. Local Exchange Carrier (See Company) Local Message A communication between a calling station and any other exchange station within the local service area of the calling station. 30 Original Definitions Sheet No. 15 Local Private Line DEFINITIONS A non-switched single or multi-point line located wholly within an exchange furnished for the customer's own use for communications or signaling between points on that line. Local Service Area The area within which telephone service is furnished under a specific schedule of rates. This area may include one or more exchanges without the application of toll charges. Low-Income Telephone Assistance Program A program that provides for a discount of 25% of the recurring monthly Basic Local Exchange Service rate, or not less than the FCC Subscriber Line Charge, for low-income customers who meet the eligibility requirements established by the Colorado Department of Social Services. Measured Rate Service A basic local exchange service that depends on measurement of actual usage to compute the local usage charges which apply for outgoing calls completed on a local basis. Message A completed customer telephone call. Message Rate Service A basic local exchange service that charges for each outgoing completed local call in excess of a specified allowance of those calls during a billing period. Minimum Contract Period The minimum length of time for which a customer is obliged to pay for service, facilities and equipment, whether or not retained by the customer for such minimum length of time. Network Control Signalling Denotes the transmission of signals into the telephone network which performs functions such as supervision (control, status and charge signals), address signaling (dialing), calling and called number identification, audible tone signals (call progress signals indicating re-order or busy conditions, alerting, coin denominations, coin collect and coin return tones) to control the operation of switching machines in the telephone system. Network Interface Device (NID) The points of common termination of Company-provided and customer-provided facilities. Non-Contiguous Exchanges Exchanges that do not share a common boundary. Non-Published Telephone Number Service An arrangement whereby a customer's listing is not available from the telephone directory or from directory assistance service. Non-Recurring Charge (NRC) (See Installation Charge.) Off-Premises Lines and Extensions Off-premises lines are additional appearances of a one-party access line at a location other than the premises where the primary access line is located. Off-premises extensions are additional appearances of a private branch exchange or key station line, or an additional appearance of a one-party access line or station at a location other than the premises where the primary station line or access line is located. One-Party Service A grade of basic local exchange service which provides that only one customer shall be served by the access line connecting the customer's service location with the serving central office. Permanent Disconnect A service is permanently disconnected when the customer's service has been totally discontinued. Premises In multiple occupancy buildings a customer's premises are restricted to the portion of building owned or leased by the customer. In single occupancy buildings or where more than one building is occupied by one firm or individual, the customer's premises includes the buildings occupied by the customer which are within 200 feet of the primary station and not separated by a main thoroughfare or property occupied by others. Private Line Service A point-to-point or point-to-multi-point service dedicated to the exclusive use of an end user for the transmission of any telecommunications service. Rate Center The geographic location in an exchange which is established at the point from which distance is measured for the computation of message toll rates and interexchange mileage rates. Registered Terminal Equipment Denotes terminal equipment, multiline terminating systems and protective circuitry which comply with and have been approved within the registration provisions of Part 68 of the FCC Rules and Regulations. Residence Service A class of exchange service furnished to an individual at a residence or place of dwelling where the actual or obvious use of the service is for domestic purposes. Tariff DEFINITIONS The entire body of rates, tolls, rentals, charges, classifications, maps, rules and regulations adopted and filed with the Commission by the Company. Telecommunications Service The electronic or optical transmission of information between separate points by prearranged means. Telecommunications Service Priority (TSP) Denotes the regulatory, administrative, and operational system developed by the Federal Government to ensure priority provisioning and/or restoration of National Security Emergency Preparedness (NSEP) telecommunications. The Federal Communications Commission (FCC) defines NSEP telecommunications services as those services which are used to maintain a state of readiness or to respond to and manage any event or crisis, which causes or could cause harm to the population, damage to or loss of property, or degrades or threatens the NSEP posture of the United States. Temporary Disconnect The suspension of telephone service at the request of the customer or on the initiative of the Company without permanent disconnect of service. 37 Original Definitions Sheet No. 22 Temporary Service DEFINITIONS Local service definitely known to be required for a short period, such as service provided for contractors for use during construction of a building, sales campaigns, athletic contests, conventions, fairs, circuses, and other enterprises of like limited duration. Termination Charge The charge applicable when a agreement for service is terminated by the customer before the expiration of the minimum agreement period. Toll Message A completed call between two exchange stations located in different local service areas for which charges are made on a per unit basis. Toll Rate The rate prescribed for toll messages which may be based upon the duration of the message, time of day, number of calls and distance between exchanges. (May be intrastate or interstate.) Toll Restriction Toll restriction presents access to the toll network and to 900 type service. Toll Service The furnishing of telecommunications service between stations of different customers in different exchanges or, where available, extended service areas. This jurisdictional service is provided by either intralata or interlata carriers and is also referred to as message telecommunications service (MTS), message toll or interexchange telecommunications service. Trade Name The name or style under which a concern conducts its business and by which it is generally known to the public. Trunk An exchange access line between a customer-provided Private Branch Exchange and a Company central office. Type of Service Refers to business and residence local exchange access service, pay telephone service or any other service provided by the Telephone Company. Wire Center The building which houses the local switching equipment (central offices) from which exchange and private line services are furnished and where cable facilities are terminated which furnish telephone service within a designated wire center serving area. Exchange service is available through facilities owned and maintained according to the standards of the Telephone Company, and in multioffice exchanges, is operated from the central office designated by the Telephone Company. 2. Each individual access line will be terminated on an appropriate interface device. 3. Except as stated in the provisions for Payphone Services, telephone service is provided for the use of the customers, their family, employees or business associates, or persons residing in their household. 4. The resale of any service provided by the Telephone Company is not permitted except as provided elsewhere in this tariff or as specifically authorized. 5. Any special structural work required for supporting telephone equipment or telephone wiring on the customer's premises shall be provided at the expense of the customer. Application of Rates RULE AND REGULATION NO. 2 DESCRIPTION OF SERVICE The applicability of business and residence rates is governed by the actual or obvious use made of the service. The use to be made of the service will be ascertained from the applicant at the time of the application for service. 1. Business rates apply at the following locations: a. In offices, stores, factories, and all other places of a strictly business nature; b. In boarding and rooming houses, colleges, clubs, libraries, lodges, hospitals, public, private and parochial schools, offices, lobbies and halls of hotels, apartment buildings, churches, and other similar institutions. c. At any location when the listing of "office" is provided, or when any title indicating a trade or profession is listed (except as may be modified under the directory listing rules and regulations and conditions governing directory listing service). d. At residence locations with an off premises business extension or when the customer has no regular business telephone service and the use of the service by the customer, members of the household, or guests, is more of a business than residence nature as might be indicated by advertising through newspapers, handbills, billboards, circulars, business cards, or otherwise. If it is found that a customer is using residence service for business purposes, the Telephone Company will thereafter require the sub-scriber to take business service, except in cases where the customer thereafter agrees to use the service for social or domestic purposes only.
