General Base Rate Areas for each exchange are identified on maps filed as a supplement to this tariff. 3. The rates for service and equipment not specifically shown in this section are presented in other sections of this tariff. 4. The NC Telecommunications Service Tax (Combined General Rate) is applicable to all local service, long (C) distance service provided within the state, and all long distance service crossing the state boundary. Individual line residence and business service is comprised of one rate element: Exchange Access Line - The serving central office line equipment and all the Company plant facilities up to and including the Network Interface. These facilities are Company-provided and maintained and provide access to and from the telecommunications network for message toll service and for local calling appropriate to the tariffed offering selected by the customer. Lifeline Service provides for a Federal credit amount of $9.25, pursuant to the FCC Order released on February 6, 2012, as well as a state credit. Funding for Lifeline Service is obtained from a universal service support mechanism to which all telecommunications carriers that provide interstate telecommunications services contribute on an equitable and nondiscriminatory basis. These credits are applied to the local service portion of the monthly telephone bill for qualified residential customers. Lifeline Service is limited to only one Service per qualified customer or household; within this section, household is defined as any individual or group of individuals who are living together at the same address as one economic unit, with an economic unit defined as, all adult individuals contributing to and sharing in the income and expenses of a household. The access line may be a flat rate, message or measured rate line and may be bundled with other features. Local usage charges (excluding operator surcharges) are those applicable for calling within the local or expanded local areas. C C 2. Applications and Regulations a. The specific guidelines for implementation of this credit are as follows: (1) The following toll blocking options will be offered to Lifeline customers at no charge. (a) Customized Code Restriction Option #4 (b) Third Number and Collect Call Block (2) A Lifeline customer s local service will not be disconnected for non-payment of regulated toll charges. Local service may be denied for non-payment of local service charges in accordance with Section 4. Access to toll service may be denied for non-payment of regulated tolls. In such cases, Customized Code Restriction Option #4 and Third Number and Collect Call Block, as described in F.2.a.(1), will be applied to Lifeline service at no charge to the customer. A Lifeline customer request for reconnection or re-establishment of local service will not be denied if the service was previously suspended or disconnected for non-payment of toll charges. (3) Partial payments made by customers will be applied first to local service charges, then to toll charges. (4) A service deposit is not applicable to a Lifeline customer that elects toll blocking as specified in F.2.a.(1). D T T 3. Eligibility a. In order to qualify for the Lifeline Service, a customer must provide certification or authorize agency verification of their participation in at least one of the following programs: Supplemental Security Income (SSI), Food Stamps, Medicaid, Federal Public Housing Assistance or Section 8, Low-Income Home Energy Assistance Program (LIHEAP), Temporary Assistance to Needy Families (TANF), or National School Lunch s free lunch program (NSL). Additionally, an applicant whose income, as defined in 47 CFR Section (f), is at or below one hundred and thirty-five percent (135%) of the Federal Poverty Guidelines, may also apply for eligibility certification. A standard application form will be available only from the agency administering the qualifying program and not from the Company. All applications are subject to verification pursuant to the FCC Order released on February 6, Certification a. The Company will accumulate all completed application forms and apply the credit on the customer s monthly bill. An explanation of the credit will appear on each telephone bill. b. When a customer is determined to be ineligible as a result of an audit, the Company will send the customer a written notice of ineligibility. If the customer cannot provide eligibility documentation, the Lifeline credit will be discontinued. c. Resellers providing Lifeline service from this tariff are responsible for determining proof of eligibility prior to requesting the service. 5. Rates and Charges a. The Secondary Service charge in Section 4 is not applicable when existing residential service is converted intact to Lifeline Service. b. General The Company furnishes Directory Assistance Service for the purpose of aiding subscribers in obtaining telephone numbers. When a party in North Carolina requests assistance in obtaining telephone numbers of subscribers who are located within the same local calling area as the calling party, the charges set forth below apply. At customer request, the address associated with a customer's telephone number may be furnished in conjunction with providing the telephone number requested. 2. Rates a. A charge of $1.00 is applicable for each direct dialed inquiry for directory assistance except as noted (I) below: each number requested constitutes an inquiry except that the first two numbers requested on any one call constitute only one inquiry. b. In order to make allowance for a reasonable need for local calling area Directory Assistance including numbers not in the directory, directory inaccessibility and other similar conditions, no charge applies for the first five direct dialed inquiries per month per main telephone or PBX trunk or for the first direct dialed inquiry per month per Centrex main station. The allowance is cumulative for all group-billed services furnished to the same subscriber within an exchange, and applies only to requests for telephone numbers of subscribers located within the same calling area as the calling party. The allowance applies only to calls placed to the designated service code or number for local Directory Assistance. No allowance is applicable for calls for telephone numbers of subscribers located outside of the local calling area of the calling party. c. Charges for directory assistance service are not applicable to inquiries received from public and semipublic telephones, nor to inquiries received from services provided for subscribers or primary users who are blind or physically handicapped to the extent they are unable to use the telephone directory. General Verification and Emergency Interrupt Service is furnished where and to the extent that facilities permit. The customer shall identify and save the Company harmless against all claims that may arise from either party to the interrupted call or any person. a. Verification (1) The Company furnishes Verification Service for the purpose of aiding subscribers with legitimate call completion problems. Upon request, the operator will verify and provide the line status condition of a local subscriber line. (2) A subscriber originated request for verification of a local number is a chargeable verification request if a Company operator determines that the line is in use. No charge applies if the line is out of order. b. Emergency Interrupt Service (1) The Company furnishes Emergency Interrupt Service when a subscriber who has originated a verification request to a line which has been found in a busy talking state informs the operator that an urgent or emergency situation exists and requests that the operator have the busy line cleared. (2) A subscriber originated request for Emergency Interrupt to a local number is a chargeable Emergency Interrupt request. 2. Application of Rates and Charges a. No charge will apply if the requesting customer identifies that the call is to or from an official public emergency agency. An official public emergency agency is defined as a government agency which is operated by the Federal, state or local government and has the capability and legal authority to provide prompt and direct aid to the public in emergency situations. Such agencies include the local police, state police, fire department, etc. b. Charges may not be billed on a collect basis or on a third number basis to the number being verified or interrupted. c. If the number verified is not in use, or as a result of interrupt the line is cleared, and, at the calling party's request, the operator completes the call, the charge for a Coin Telephone Local Call will apply in addition to the applicable verification and emergency interrupt charges. Operator Assisted Local Calls 1. Operator Assisted Charges a. All types of local exchange service have local calling areas as specified in Para. E. page 3 of this Tariff which are the areas that can be called on a flat rate basis (no charge for individual calls), or on a local coin call rate basis. b. Local dial call: The call must be dialed and completed without the assistance of a Company operator and must be billed to the originating telephone when a charge is applicable. c. The following service charges for operator assisted local calls apply in addition to the local dial rate** (T) applicable. (1) Station to Station customer dialed credit card local call Nonrecurring USOC Charge a) Each $1.05 NA (I) (2) Station-to-Station operator assisted sent-paid, collect, third number, and non-customer-dialed credit card calls a) Each $1.36 NA (3) Person-to-Person operator assisted local call (a) $3.74 NA (I) d. The following Operator Assisted Local Calls are exempted from the service charge: (1) Calls to designated Company numbers for official telephone business (2) Emergency calls to recognizable authorized civil agencies. (3) Those cases where a Company operator provides assistance to: (a) Re-establish a call which has been interrupted after the calling number has been reached. (b) Reach the calling telephone number where facility problems prevent customer dial completion. (c) Place a non-coin, sent-paid call for a calling party who identifies himself as being handicapped and unable to dial the call because of his handicap. ** The local dial rate applicable for operator-assisted local non-sent paid calls originating from a PSP access line (N) and processed by a Company operator is $0.35. Local Directory Assistance Call Completion (DACC) is an optional service provided to users of Local Directory Assistance (DA) Service. When dialing (411), Local DA customers may choose to have the telephone number they are requesting dialed by the DA Operator. b. The service is available to Business and Residence customers except as limited in 4. following. c. Individual message detail is not included as part of this service. d. The service is available only where billing and terminal capability exists. e. Customers may request blocking of DACC calls originating from their telephone lines by contacting the local Company Business Office. There is no additional charge to the customer for blocking. 2. General Regulations a. The service is not subject to concessions. 3. Use of the Service a. The service is furnished subject to all applicable regulations in Section 28 of this tariff. 4. Limitations of Service a. The service is not available for the following classes of service call categories: (1) Non-Concord Telephone Carrier customers (2) Any Special Line Class Codes (3) 976 DA number requests (4) Alternately Billed Calls; e.g., Collect, Calling Card, or Billed to Third Number (5) Any PBX type customer who requires real-time notification of charges (6) Calls from tandems where the end user cannot be identified (7) Calls from the Company and COCOT Coin Stations 5. Application of Charges and Exemptions a. The charges specified in 6. following will be applicable to all subscribers. b. Chargeable Calls (1) For charging purposes, a DACC completed call is as defined in Section 29 of this tariff. 6. Rates and Charges a. PREPAID LOCAL SERVICE (PLS) can be offered to customers only after the company has discussed all other options for obtaining regular local service, or establishing credit under Commission Rule R12-2(a). PLS is a class of service designed to provide basic local dial tone service, including a telephone number and a Directory Listing 1, to residential customers who are unable or unwilling to establish credit under Commission Rule R12-2(a). b. PLS can be offered to customers who were disconnected for non-payment for local service within the previous three years when they pay or make arrangements to pay such past due balances. c. A PLS customer may also convert to basic local service at any point in time by establishing credit under provisions of Commission Rule R12-2(a), and paying or making arrangements to pay any past due balances for basic local service which were incurred within the previous three years. d. PLS Customers will have Toll and Operator Restrictions to prevent any additional calling charges. e. PLS Customers may place calls within their Local Calling Area, including Unlimited Metro Calls. PLS Customers may place calls to toll-free 800 numbers, will have access to 911 Service, and will have access to dual party relay service. f. PLS does not permit a Customer to originate calls to direct dial 1+ or 0+ toll services or to caller-paid information services ( 900 or 976 ). g. The usual and customary installation charges will not be applicable. h. PLS Customers may utilize prepaid long distance calling cards for their long distance calling needs. i. PLS Customers have the option of adding non usage-based features to their service. Optional features will be charged at the regular rates in Section 17 of the General Exchange Tariff and those rates will apply, in addition to the rate for PLS. There will be no secondary service order charge for adding optional features to their prepaid service at the time of connection. However, to add optional features at a later date there will be a secondary service order charge as found in Section 4 of the General Exchange Tariff. Those optional features are: Call Block Call Manager Power Pack Call Forward Call Manager Deluxe Power Pack Deluxe Call Forward Series Call Return Starter Pack Caller ID Call Waiting with Cancel Three Way Calling Caller ID Deluxe Call Waiting Deluxe Talking Call Waiting (N) Call Return with Name (N) 2. To establish PLS service: a. Customers will be required to sign a Customer Service Agreement. b. Customers will be required to pay one months charges in advance (basic service + optional features), which are non-refundable. c. All payment transactions must be in person at a CTC business office or other location as CTC may designate, via cash, credit/debit card, or money order. 3. The Company may disconnect PLS after a disconnect notice: a. for failure to comply with the terms of a deferred payment plan for any past due services. b. for violation of the Company s rules pertaining to the use of PLS in a manner which interferes with the service of others or the operation of nonstandard equipment, if a reasonable attempt has been made to notify the customer and the customer is provided with a reasonable opportunity to remedy the situation. c. if the customer accrues new billable charges for toll or other services on their telephone bill. d. where service is connected without authority by a person who has not applied for the service or who has reconnected service without authority following termination of service. e. or non-payment of monthly charges by the specified due date. NOTE 1: See Section 6A and 6C of the General Exchange Tariff for information on Directory Listings. A customer subscribing to PLS may convert to basic local telecommunications service provided the customer: a. has had service for at least six continuous months with no late payments or service interruptions (toll restrictions may apply 1 ); b. has paid all outstanding local service debt incurred within the previous three years or made arrangements to pay such past due balances; and, c. requests subscription to basic local telecommunications service from the Company. 5. Restoration of Service a. In the event service is disconnected for nonpayment of charges, such service will be restored upon payment of: (1) All charges due or, at the discretion of the Company, a substantial portion thereof and (2) The Restoration of Service Charge as found in Section of this General Exchange Tariff. 6. Rates and Charges a. Monthly Recurring Charges Monthly Prepaid Local Service 2 $39.95 b. Non-Recurring Charges Initial Installation Charge $ A Secondary Service Order Charge will apply for Conversion to basic local Service. This charge can be found in Section 4 of the General Exchange Tariff Note 1: A PLS customer may convert to basic local telecommunications service without the need for toll restriction after having service for twelve (12) continuous months or longer with no late payments or service interruptions due to non-payment or establishes credit under provisions of Commission Rule R12-2(a). Note 2: Monthly Prepaid Local Service includes unlimited Local and Metro calling, and a Directory Listing (Section 6A and 6C of the General Exchange Tariff). Pay-Per-Use Blocking (PPBA), Directory Assistance Call Completion Blocking (DABL), Directory Assistance Blocking, and Customized Code Restrictions (CCRS) will be used to prevent additional charges. (C) (C) Note 3: Service Order, Premises Visit, and Central Office charges in Section 4 of the General Exchange Tariff do not apply for initial installation of service. CTC Connections service provides a residence service exchange access line and Custom Calling Services and Class Features specified following. b. The rate for CTC Connections specified herein entitles a residence subscriber to unlimited calling to all access lines within the serving exchange and the associated local calling area as defined in Section E on page 3, and, in addition, entitles the subscriber to unlimited calling to all access lines within the Metro plan calling area as defined in Section 30.1 of the General Exchange Tariff. c. The rate specified herein also entitles a residence CTC Connections subscriber to unlimited use of the following subscription services/features on their CTC Connections line as specified in Section 17 of the General Exchange Tariff. Call Waiting w/cancel Caller ID Deluxe Call Forwarding with Series Completing (Variable) Three-Way Calling Call Selector Call Waiting Deluxe Call Return Call Block Repeat Dialing Call Acceptance All rules, regulations and limitations specified in the section listed apply to the services/features as part of this service. d. Service charges specified in Section 4 of the General Exchange Tariff do not apply to the installation of new CTC Connections service or to the conversion of an existing line to/from CTC Connections. (C) e. Existing customers of CTC Connections service may not take advantage of any special promotion for CTC Connections or any of the CTC Connections services/features unless specifically allowed by the terms of the special promotion. f. Upon customer request, temporary suspension of this service will be provided as specified in Section 2 of the General Exchange Tariff. CTC Connections-Lite service provides a residence service exchange access line and Custom Calling Services and Class Features specified following. b. The rate for CTC Connections-Lite specified herein entitles a residence subscriber to unlimited calling to all access lines within the serving exchange and the associated local calling area as defined in Section E on page 3, and, in addition, entitles the subscriber to unlimited calling to all access lines within the Metro plan calling area as defined in Section 30.1 of the General Exchange Tariff. c. The rate specified herein also entitles a residence CTC Connections-Lite subscriber to unlimited use of the following subscription services/features on their CTC Connections-Lite line as specified in Section 17 of the General Exchange Tariff. Call Waiting w/cancel Caller ID Deluxe Three-Way Calling Call Return Call Block Talking Call Waiting and/or Call Return with Name can be added for an additional fee of $2.00 each. All rules, regulations and limitations specified in the section listed apply to the services/features as part of this service. d.
