2 Dancer Conflicts Dancers in the Company are required to attend all rehearsals and events. If you foresee conflicts that may interfere with attendance, please list them below. Company can only take into consideration conflicts received at the time of audition. Conflicts may affect the dances you perform at certain events. If none are selected we will not take scheduling conflicts into consideration for your role(s). If you are unable to make rehearsals and conflicts were not noted, you may not be allowed to perform. This is your only chance to let us know! Times for additional rehearsals may change weekly. Events may be scheduled that are not currently listed. 4 Form Dancer #: (To be assigned at time of audition) Company Performer Agreement Performer Name: Company fees are non-refundable. Additional expenses may include specific leotards, tights, and dance shoes to wear as part of Company costumes, in addition to make-up and hair supplies and costumes if required. Other than the June Production, the Company will work to utilize items that dancers already own or that can be readily borrowed for a nominal fee. Additional costume fee is required if participating in the June Production. Tickets to events for family and friends are not included in membership, unless the participating facility provides comp tickets or the event is free. Parents & Dancers must check regularly for updates and important information. If you do not receive a Company by 3/5/16, please contact us immediately. Rehearsal schedules may change weekly. Dancers must abide by Theater/Rehearsal Etiquette. All food must be eaten and disposed of outside in the picnic area when rehearsing at IAD. Please be considerate of all venues. Dancers attend all scheduled rehearsals unless excused for illness or okayed on conflict sheet. Diligence is expected regarding rehearsal times each week. Dancers will notify IAD prior to rehearsals regarding lateness or illness. Dancers must arrive on time in appropriate dance attire, warmed up and ready to dance with a positive attitude and stay for the duration of all rehearsals. Dancers may lose the opportunity to perform or continue as a Company Member if unable to cope with the demands of rehearsals or if unable to remember or perform choreography to standards. Damaged or missing costumes will need to be replaced or repaired at the expense of the dancer or responsible adult. Parents and dancers (especially adult and teen dancers) are required to clean-up after all rehearsals and performances, including props, costumes, sets and belongings. Talking out of turn, inability to pay attention or otherwise acting out in a disruptive manner during rehearsals, backstage or at performances will not be tolerated. Dancers that are unable to focus or act appropriately during rehearsals, backstage or at the performances will be withdrawn from the Company. Dancers will refrain from talking (or will speak in a quiet voice when necessary) during rehearsal and in backstage areas. Rehearsals are closed to parents and guests. Please wait outside during rehearsals. If you are dropping off, please wait with your dancer until rehearsal begins. IAD is not responsible for dancers before or after rehearsals or events. Please pick up dancers promptly after all rehearsals and events. I have read this release and fully understand its content, meaning and impact and agree to its terms. Property left on the premises may be discarded or donated. We hold IAD and SCDG harmless for injury and negligence. We understand that Company rehearsals and performances may be physically strenuous and participate with full acknowledgment that there is risk of personal injury, property loss or even death. Photo-Image Release: Dancers in the Company may be videotaped and/or photographed at any time throughout the rehearsal and performance process without advance notice. NOTE: Advance warning will always be given if photos will be taken in dressing rooms. Images or video footage may be used for commercial purposes, advertisement and/or any other purposes IAD or SCDG deems suitable without further consent, notification or compensation. Dancers names will not be used without permission - with the exception of programs for events or performances. Any such action will be taken in the best interest of myself/my child and will be reported to me as soon as possible.
