HAVE FUN, MEET FRIENDS, BUILD RELATIONSHIPS IN A PLACE OF FAITH, AND DANCE, DANCE, DANCE. 2. Students must have all required forms completed and signed by a parent or guardian or they will not be permitted into the dance. This includes code of Conduct form, Annual Consent & Release Form, and this one. 3. Students must act in a responsible and respectful manner, respecting the church property, showing respect for one another and one self, and using appropriate verbal and body language AT All TIMES. 4. Students will not disrespect or challenge the chaperones. 5. No drug or alcohol use will be tolerated; anyone found to be under the influence or in the possession of such items may be subject to arrest. A Police Officer and/or Security Office will be on duty at the dance. 6. Boys and Girls must wear appropriate clothing. No halter/tube/spaghetti strap shirt, No really short skirts or shorts, or inappropriate clothing advertising permitted. No student is permitted to change clothing upon entering the dance. 7. No student will be permitted to leave the building before the dance is over. Parents who need to pick up their child early must enter the dance to sign the student out. 8. Parents must provide reliable on time transportation, and must provide a phone number where they can be reached during the dance. Phone number must be provided on this form above or the student will not be permitted into the dance. 9. In the event of an emergency, St. Elizabeth Ann Seton Church will contact the parents and authorized medical personnel to provide emergency services to the student. 10. If a student does not follow the rules, parents will be notified to come and pick up their son/daughter, in addition to any guests sponsored. No refunds will be issued.

2 11. Students will respect each person by keeping their hands/objects to themselves and use only proper displays of friendship at the dance, while on church property. The Youth Ministry Dance (CLUB SETON) is held at St. Elizabeth Ann Seton Church located at 345 Bear-Christiana Road Bear, Delaware CLUB SETON opens its doors at 6:30pm; the dance is from 6:30 to 9:30pm in the Parish Hall. The cost is $10 CASH per person. Admission Stipulations, Required Forms, and Drop off/pick up parking lot procedures Admission/Required Forms-The Dances are for School Students Only in grades 2-5, attending a Wilmington Diocese Catholic School, or are a member of one of the Diocese of Wilmington Catholic Churches. As a student or member of a parish within the Diocese of Wilmington, you are permitted to bring guests for which your parents are willing to give a character reference for. Permission slip, character reference forms, code of conduct agreement, and/or signed consent/release form (see links below) are required for all persons attending the dances. No Form, No admission to the dance. All Forms and $10cash will be collected at the door. (Please note that having these forms does not guarantee entry into the dance) The entrance to the dance is located behind the church at the Seton Formation Center. Parents are not permitted to drop and go; you must escort your kids to the entrance doors or wait in your car until you get confirmation that they have received admittance to the dance. All required form must be completely filled out prior to entering the dance. All Youth Members belonging to parishes within the Diocese of Wilmington, who have submitted all the required forms and who submits an annual permission forms will receive a Club Seton Picture Membership Card. This membership card will expedite admittance to all future dances. The membership cards and permission forms will be valid for the current calendar year ending in December. Please use caution when driving on the church property. For the protection of our children, parents, volunteers, and parking lot security. Your speed limit is not to exceed 15mph at all times. Drop off begins at 6:30 pm, the doors will not be open prior to that time.- Do Not Drop and GO Following the direction of the parking lot security, you will be directed to the parking lot behind the church and into parking spaces. You MUST escort your child(ren) to the dance doors, please remain in the parking space until you have received your child s confirmation that they are admitted into the dance. When the dance is over, you and your child/ren should have a designated parking row, meeting place. This will cut down on the dismissal confusion. The Entrance Doors will close at 7:30pm no admittance will be permitted after this time. The DANCE IS OVER AT 9:30pm, arrive prior to 9:20pm,. Parents should arrive for pickup prior to 9:30pm, following the direction of the parking lot security, park in your designated row. The Dance will dismiss at 9:30pm, Exiting from the Formation Center doors facing the parking lot. Parents can wait for their child(ren) in the marked pedestrian area of the parking lot. During the car loading time no cars will be permitted to enter or exit the back parking lot until all cars are loaded and cleared. You will be directed to exit by the parking lot security. Once directed by the parking lot attendant, cars should alternate as they exit the parking lot. You have our commitment that we will do our very best to expedite the process of exiting.
