1 Type 1 Facilities May Version 1.0 This document sets out NBN Co s proposals in respect of certain aspects of the National Broadband Network. The contents of this document represent NBN Co s current position on the subject matter of this document. The contents of this document should not be relied upon by our stakeholders (or any other persons) as representing NBN Co s final position on the subject matter of this document, except where stated otherwise. NBN Co s position on the subject matter of this document may also be impacted by legislative and regulatory developments in respect of the National Broadband Network. 2 - Information Who is required to read this site induction information? All personnel requiring access to NBN Co Sites which are Type 1 Facilities, are required to confirm they have read and understood the following information as a pre-requisite to gaining entry. If you are not sure if you are required to complete this course, contact the Authorised Persons representing your organisation and if necessary they will direct your enquiries to NBN Co. This NBN Co Site Induction Information is a prerequisite for entry into Type 1 Facilities only. If you require entry into a Type 2 Facility you must also undertake the Site Induction Course applicable to Type 2 Facilities. If you require any further information related to your responsibilities associated to completing NBN Co Site Induction for Type 2 Facilities, contact the Authorised Persons representing your organisation. 3 - Information Welcome to the NBN Co Network and Facilities Site Induction Information for Type 1 Facilities! This Site Induction Briefing will provide you with an overview of the requirements to work safely and securely on National Broadband Network (NBN) Co Sites which are Type 1 Facilities. Unless otherwise specified all future references to an NBN Co Site or Site is a reference to a Type 1 Facility. Upon completion of this course you should be able to: List the requirements and process to enter and exit an NBN Co Site. Identify the difference between NBN Co Type 1 and Type 2 Facilities. Outline your responsibilities related to requesting and maintaining access. Outline the process to follow for Lost, Stolen, Missing, Faulty or Damaged Electronic Access Cards (EACS) Identify certain key rules for working on an NBN Co Site. Identify the general HS&E requirements of working on an NBN Co site. Identify the consequences of not complying with your responsibilities related to obtaining access and working on the site. 4 Module One: Pre-entry Requirements Working On NBN Co Sites and Pre-Requisites. This module will outline what is the difference an NBN Co Type 1 and Type 2 Facilities and identify the pre-requisites related to requesting and gaining access to NBN Co Sites. Upon completion of this module participants will be able to: List the requirements and process to enter and exit an NBN Co Site. Identify the difference between NBN Co Type 1 and Type 2 Facilities. 5 Module One: Pre-entry Requirements Facilities that your organisation may access in connection with the NBN may be owned by NBN Co or owned by a third party. This Site Induction Course only deals with NBN Co owned or operated Sites called Type 1 Facilities. You will need to undertake a different Site Induction Course for other NBN Co Type 2 Facilities. NBN Co Sites include: NBN Co Site Type 1 Facility Type 2 Facility Description A building that is occupied, owned or operated by NBN Co and in respect of which: a Point of Interconnect is established; and/or an Aggregation Node Site is located. The NBN Co Type 1 Facilities are; Port Melbourne, Nunawading, Asquith, Eastern Creek, Aspley, Acacia Ridge, Greenfields, Lonsdale, Wangara and Bentley Depots. A building that is owned or operated by an entity other than NBN Co (i.e. a third party also referred to as an Underlying Facility Provider in this course) and in in respect of which: a Point of Interconnect is established; and/or an Aggregation Node Site is located. 6 Module One: Pre-entry Requirements Before Entering an NBN Site that is a Type 1 Facility. After you have successfully read this Site Induction Briefing you are authorised to use the Induction Briefing reference number By using the Induction Briefing reference number you agree that you have read and understood this Site Induction Information. This Induction Briefing reference number is required in your subsequent Physical Access Request, including your application for an Electronic Access Card (EAC) or keys. The Physical Access Request Form must capture the locations to which you require access and must be lodged by the Authorised Persons representing your organisation. Ensure you hold a valid/approved Work Request for the specific work you will be performing. Ensure that you are competent to perform the specific work you will be performing and hold any required certifications and licences before entering a NBN site. Ensure that you seek authorisations to undertake any notifiable works on site. (see page 17 for what notifiable works are) Organise your own personal protective equipment (PPE) as well as any tools and equipment you will need. Be aware you may be subject to quality and safety audits and inspections by NBN Co at any time. Be aware you may be required to complete site-specific Induction briefing on-site at first attendance to some NBN Sites before physical access to a NBN Site will be granted. Be aware you may also be provided with reasonable directions by NBN Co or any site owner relating to HS&E matters, that you must comply with. Be aware that post activity quality audits and inspections may be carried out by NBN Co. Version 1.0 Page 6

7 Module One: Pre-entry Requirements Access & Work Requests Request Physical Access to a site then submit the relevant Work Request to perform the work on that site on a specific date. Physical Access Request means a request to gain access to an NBN Site, which involves submitting a physical access order form in order to obtain an Electronic Access Card: A Physical Access Request will provide physical access for nominated individuals to any nominated NBN Site in a particular State or Territory Where you require physical access across multiple States and/or Territories, this should be requested in the physical access order form. Following approval of your Physical Access Request, you will be issued with an Electronic Access Card for the NBN Site(s) you have requested access to. Once approved, a Physical Access Request will generally remain valid for 12 months. Type 1 Facility sites require access cards that display your photograph. Your organisation s Access Administrator should ensure a recent photograph of you is submitted as a jpg. file with your physical access request form. You must ensure your access card is visible at all times when working in or visiting NBN Co Sites. A Work Request refers to your submission of a request to gain access on a specific date to a location where you will be performing the work. A Work Requests may take the form of: Where your organisation is a Customer of NBN Co and the access is for any Facilities Access Service acquired by your organisation this is generally via a Service Request. Where you are an employee of NBN Co or a contractor of NBN Co a Change Request or an Incident Report depending on the work type that you wish to conduct. Follow all process steps relevant to progress your request. The process will include an Authorisation by a second party and an approval activity. Version 1.0 Page 7

Before entering a site, your organisation must have all of (but not limited to) the following site entry requirements in place: All Authorised Persons have completed the NBN Co Site Induction Courses and/or Site specific Briefing inductions. Your organisation's Authorised Persons have the correct Electronic Access Cards and/or Keys, as required for the relevant site(s). An approved Work Request (e.g. Service Request Trouble Ticket within the NBN Co Service Portal) Agree that all other on-site requirements and responsibilities are met. Version 1.0 Page 8

This module will outline what is the responsibilities related to maintaining access to NBN Co Type 1 Facilities. Upon completion of this module participants will be able to: Outline your responsibilities related to maintaining access Outline the process to follow for Lost, Stolen, Missing, Faulty or Damaged Electronic Access Cards (EACS) Version 1.0 Page 9

10 Module Two: Access Responsibilities Maintaining Access EACs renewals are managed by submitting a Physical Access Request form. NBN Co recommends that renewals are lodged between 1-2 months prior to expiry and must be lodged by the Authorised Persons representing your organisation. Whilst in possession of Electronic Access Cards and/or Keys, you must ensure that do all of the following: Adopt effective security measures to prevent loss, damage and unauthorised use or access to the EACs and/or Keys that have been issued to you. Ensure there is NO unauthorised copying of EACs and/or Keys. Provide assistance as requested by NBN Co or by the Authorised Persons representing your organisation, in ensuring adequate security and safety in NBN Co Sites to which you have access. Immediately return any EACs and/or Keys to NBN Co or to the Authorised Persons representing your organisation, when you no longer require (or are no longer authorised to) access the relevant NBN Co Sites. EACs cards cannot be transferred from one person to another. Keys cannot be transferred from one person to another without NBN Co approval. To request the transfer of Keys, the Physical Access Request Form must be lodged by the Authorised Persons representing your organisation. Version 1.0 Page 10

11 Module Two: Access Responsibilities Lost, Stolen, or Missing Electronic Access Cards As soon as you become aware that an EACs and/or Keys have been lost, stolen or are missing you or the Authorised Persons representing your organisation must: Notify the Physical Security Operations Centre PhySOC immediately on (03) Log the incident immediately through the isafe application. Lost Cards: Required information When contacting NBN Co to notify that an EACs and/or Keys have been lost, stolen or are missing, you or the Authorised Persons representing your organisation must provide the following information (as relevant): The access card number. The key code and serial number. The circumstances surrounding the loss, theft or misplacement of the EACs. In addition to reporting the lost/stolen/missing EACs and/or Keys, you must also request a replacement EACs and/or Keys via a Physical Access Request Form lodged by the Authorised Persons representing your organisation. 12 Module Two: Access Responsibilities Damaged or Faulty Electronic Access Cards When you discover that an EACs for an NBN Co Site is faulty or damaged, you or the Authorised Persons representing your organisation must: Notify the Physical Security Operations Centre PhySOC immediately on (03) Log the incident immediately through the isafe application. Follow the instructions detailed within the Operations Manual. Return the faulty or damaged EACs to the following address: Security Service Desk NBN Co Level Elizabeth St Melbourne VIC 3000 In addition to reporting the faulty or damaged EACs, you must also request a replacement EACs via a Physical Access Request Form lodged by the Authorised Persons representing your organisation. As soon as you become aware that an EACs and/or Keys have been lost, stolen, missing, faulty or damaged notify the PhySOC and ensure an incident is logged in isafe, this may be via the Authorised Persons representing your organisation Replacement EACs and/or Keys are requested via a Physical Access Request Form lodged by the Authorised Persons representing your organisation. This module will outline the general HS&E requirements and housekeeping rules and responsibilities whist on site. Upon completion of this module participants will be able to: Identify certain key rules for working on an NBN Co Site. Identify the general HS&E requirements of working on an NBN Co site. Identify the consequences of not complying with your responsibilities related to obtaining access and working on the site. Describe the notification requirements for hazardous work activities. 15 Module Three: Site Responsibilities and HS&E On every NBN Co Site, it is your responsibility to: Display your EACs at all times. Ensure that all risks associated with work undertaken are identified, assessed and controlled. Report all HS&E or Security hazards, property-related damage, faults and incidents to the NBN site contact and your company contact. Adhere to all site specific HS&E Procedures, reasonable directions, orientation briefings (if required) and signage. Work only within your approved areas. Conduct yourself in a civil and inoffensive manner at all times. You must NOT carry out any activities that may: Endanger the health and safety of other persons onsite or Interfere with or cause damage to any network, communications, infrastructure or other property. Notify onsite staff immediately if the action of another person threatens your safety or the safety of other personnel on site. Comply with all other legal and contractual requirements applicable to you or your employer (including, in the case of persons doing work on behalf of NBN Co Customers, the WBA and its Operations Manual). Any breach of the rules may result in you being requested to leave the NBN Co site and may revoke your future access rights Version 1.0 Page 15

16 Module Three: Site Responsibilities and HS&E While on site it is important to: Maintain familiarity with emergency procedures and participate in any evacuations. Ensure clear access to and from emergency exits, emergency equipment and switchboards. Ensure any electrical equipment used is in serviceable condition and has current electrical test labels. Adopt effective security measures to prevent loss, damage and unauthorised use or access. Register or sign in and out using any applicable on-site attendance record tools. Remove all tools, equipment, surplus materials and waste generated each day. No surplus equipment, goods or substances are to be stored at an NBN site. Comply with all relevant requirements specified under Work, Health and Safety Legislation that are applicable to the activities being undertaken. That you may be escorted by Site Security or NBN Co Staff as part of your work. That your organisation may be subject to HS&E audits and inspections. You will be required to present documentation associated with your work. Note: NBN Co Sites are Alcohol and Drug free zones and Non Smoking facilities. Version 1.0 Page 16

17 Module Three: Site Responsibilities and HS&E Notifiable Works Some works are considered notifiable works due to potential health, safety, environment and/or security risks. Prior authorisation must be sought from NBN Co and you will be required to show relevant documentation demonstrating you have appropriate competence and safe systems of work to undertake the task. You must carry Safety Data Sheets (SDS) for any chemicals/ hazardous substances that you take onsite and bring all the required personal protective equipment (PPE), tools and equipment. Notifiable works include: Access or entry into Confined Spaces (Fibre Pits in Type 1 Facilities have been classified as confined spaces) Working at Heights Hot Works Trenching Drilling or cutting works that produce dust or smoke; these can hinder site operations or trigger fire suppression systems. Drilling, coring, cutting, demolition or other works that may disturb potential asbestos containing materials. Use of any powered or mobile plant equipment that may expose other people to physical hazards and/or require specific competency to operate (for example, elevated work platforms, cranes, hoists). Working on High Voltage Power and/or within Power Modules. Working on Air Conditioning units. Equipment Power-up, specifically: Connection of DC Cabling Works that may Restrict Access (for example, Blocking off site access to others for extended periods) Installation and maintenance of telecommunications plant and equipment Isolations (electrical, thermal, pneumatics, hydraulic, stored energy) Other identified high-risk activities not described above. 18 Module Three: Site Responsibilities and HS&E Site Maintenance and Housekeeping rules are required to keep the NBN Sites clean and tidy A clean and tidy site is not only important for health and safety reasons, but is critical to the efficient and reliable operations of the network and operational equipment. Dust and other contaminants generated within the network environment can enter sensitive equipment and negatively impact the performance of the network. NBN Co Sites MUST be kept free of unnecessary obstructions and all rubbish. All excess equipment, packaging and fibre filings must be cleaned up promptly from module and warehouse areas and disposed of correctly. Waste receptacles are for general daily waste and must not be used for waste from construction or project activities. Drink and food is STRICTLY prohibited within the Warehouse or any areas with Network Equipment. Food preparation and consumption must only occur in designated lunch rooms; if available Food waste must be disposed in the appropriately identified waste disposal receptacles on site Version 1.0 Page 18

19 Module Three: Site Responsibilities and HS&E When leaving the site Remove all your tools, equipment, surplus materials and all waste generated each day. Reinstate all equipment as appropriate. Ensure all racks or exits are securely closed and locked. Return Keys. Switch off lights (if possible) when leaving an unoccupied area. Report any HS&E security or facility hazards, incidents or faults to site staff. Sign out in the attendance logbook. Version 1.0 Page 19

20 Module Three: Site Responsibilities and HS&E Site Responsibilities and HS&E Summary! Attend any briefing on safe work practices, environmental workplace and HS&E as required by NBN Co. Sign in and out using any applicable on-site attendance register. Identify, assess and control all risks associated with the task you intend to undertake and be competent to perform the work. Carry relevant documentation to demonstrate your safe systems of work (for example; Safe Work Method Statements SWMS; competency to perform the work Permits if required and carry Safety Data Sheets (SDS) for any chemicals/ hazardous substances that you take onsite) Bring all required personal protective equipment (PPE), tools and equipment to complete the work(s). Do not use or operate any facility owned equipment on site without prior authorisation. Report all OH&S/HSE hazards and incidents to the NBN Co Site contact and the Authorised Persons in your organisation. Log the hazard in the isafe application, if you do not have access this will need to be completed by the Authorised Persons in your organisation Report any property damage, faults or incidents to the NBN Co site contact and the Authorised Persons in your organisation. Adhere to all site-specific HSE procedures and signage, including any HSE and security framework. Remove all tools, equipment, surplus materials and waste generated each day. NO surplus equipment, goods or substances are to be stored at an NBN site. PUBLIC NBN Co 2013 Version 1.0 Page 20
