2 Group #4 IND REL 731 October 2, 2007 I. Executive Summary The XYZ Corporation strives to employ and retain a well-qualified and motivated office staff. Studies show that a healthy workforce can increase productivity, reduce absenteeism and turnover, along with reduced health care and insurance costs to both the employer and the employee. A company wellness program should provide both an efficient and cost-effective way to promote a worksite culture that encourages workers to lead healthier lives. The Human Resources Department will conduct a wellness program survey, by department, to a random selection of all the office employees. This needs assessment survey will provide the Executive Committee with a base line on employee interest and motivation to participate in a company sponsored wellness program. Should a wellness program be put into action, the initial survey will provide the Human Resources staff with valuable data in establishing any future longitudinal surveys or studies, along with cost effective information needed prior to program implementation. 1

3 Research Proposal Wellness Program Survey II. Problem Statement The primary research objectives identified by the Human Resource Department of XYZ Company include the employee s overall need of a wellness program for the organization and the types of programs that should be implemented. Specifically, the study will: 1. Determine the percentage of employees who are currently participating in health benefit programs outside of the organization. 2. Determine the percentage of employees who would be interested in a program being provided to them. 3. Determine the types of wellness programs that would be the most benefit and would be of interest to the employees. Conducting the survey will assist in determining the current interest level and need of providing a wellness program for our employees. The data will assist with developing a program that will have a high participation rate and successful outcome. The results of a well-created wellness program will increase the participating employees health and provide a higher quality of life. The benefits for the employer would be increased productivity and morale of the employees, decreased absenteeism and reduced medical costs. The information found in the survey will benchmark the health and interest of the employees. This will allow the Human Resource team to continually monitor the affects of the wellness program and overall interest moving forward. The results will help to determine what changes and improvements need to be made to the program and maximize the interest and participation. 2

4 Group #4 IND REL 731 October 2, 2007 III. Research Design Sample Questionnaire: See appendix A for a sample survey. The surveys will be inter-office mailed to a random sample of all active employees. The surveys will be numbered for tracking purposes. There will be a two-week time frame for the employees to complete the survey and return it to the Human Resources department. Reminders to complete the survey will be done in several forms place flyers in break rooms mid-week one and a company-wide reminder beginning of week two. Finally, if less than half of the chosen employees completed the survey by mid-second week, individuals in top management will ask department supervisors to remind individuals in his/her department that if they were chosen to participate in the survey it is strongly recommended that they participate. Data Analysis: Upon receiving the completed surveys, the Health and Wellness Coordinator will compile and analyze the data. The surveys have been pre-coded, with a minimal number of questions that will need to be post-coded. Data will be entered into a Microsoft Excel spreadsheet so the data can be calculated and analyzed. All statistically significant percentages and correlations will be noted, so the Health and Wellness Coordinator can provide results and recommendations that will be presented to the executive committee in a report format with the data graphically illustrated to visually display the results that were found. 3

The Health and Wellness Coordinator will evaluate your survey and no individual responses will be provided to anyone in the organization. Your answers will be kept in strict confidentiality. A.
