1 Program Name Eligibility Program Summary More Information Wellness Credit Program Wellness Credit Program, spouses and who are enrolled in a Total Rewards medical plan (see Wellness Credit Program Guidelines on My HR for additional criteria) The My Health wellness credit program is designed to focus on what's important - you and your health. We want to support you in your efforts to be healthy and reward you with credits toward the cost of your Total Rewards medical coverage when you participate in certain activities and programs. Each year, we will outline specific activities that qualify for a wellness credit. If you participate in these activities and enroll in Total Rewards medical coverage the following year, you'll receive a credit toward that coverage. And if your spouse/domestic partner does the same, he or she can earn a wellness credit. Credit Program. Weight Management We make it easy and inexpensive to keep your weight under control through our partnership with Weight Watchers. We pay a 50 percent subsidy for associates off a special discounted rate for the following types of subscriptions: At Work meetings bring the Weight Watchers experience right to your workplace where a trained Leader facilitates weekly meetings, and you can benefit from the proven advantage of group support from coworkers. Weight Watchers. Weight Watchers Spouse or domestic partner may take advantage of a special negotiated rate but are not eligible for the subsidy Local meeting vouchers are a prepaid savings plan that offers the flexibility of attending Weight Watchers meetings in your community when and where it suits you best. Available in 13-week or 18-week vouchers, or through a Monthly Pass, which provides unlimited meetings each month in your local community and free etools, Weight Watchers Internet weight-loss companion. Online: You can follow Weight Watchers, step-by-step online, with interactive tools and resources like a weight tracker, progress charts, restaurant guides, and hundreds of recipes and meal ideas. then Weight Management. Call At Home kits give you information and resources by mail to follow our plan stepby-step at home. The kit includes weight-loss tools and bonus products that are conveniently delivered to your home, along with access to a toll-free helpline with

2 26 weeks of progress reporting. Lighter Start Weight Challenge The Lighter Start weight challenge This eight week challenge will encourage participants to track their weight online and establish and track progress on related health goals. Participants will be able to form teams and engage with their teammates in an online discussion board, creating a support system to help them achieve their weight loss goals. via and on WorkNet beginning in February. Nutrition Onsite cafeteria & vending with access to onsite cafeterias and/or vending Healthy options are available at onsite cafeterias and vending machines to help make selecting healthy foods easier. WellPoint subsidizes healthy food choices at onsite cafeterias through a healthy dining discount program. You will automatically receive the discount at the register. then Nutrition. Visit onsite cafeteria Onsite catering with access to onsite catering We have provided healthy guidelines for you to use when making food choices for meetings. We encourage consumption of fruits, vegetables, whole grains, beans, and calcium-rich foods while limiting the intake of fat, sodium and sugars. A discount incentive is offered to departments choosing healthy choices. then Nutrition. Physical Activity and Fitness Onsite Wellness Centers (National Institute for Fitness & Sport) who work at a location with an onsite wellness center WellPoint's onsite wellness centers are fully equipped and are open extended hours, 7 days per week to fit any schedule and any fitness level. They are staffed by NIFS (National Institute for Fitness & Sport) and also offer personal training regimes, programs for weight loss, smoking cessation, lunch and learns, and free blood pressure checks. Examples of programs include: Maintain not Gain: encourages realistic weight maintenance over the 12-week holiday season. CLUB PED TM : eight-week walking and running program to help motivate you to stick with your exercise and physical activity routines. Butt Out! TM: six-week tobacco cessation program, with weekly 30 to 45- NIFS. then Physical Activity. Visit onsite wellness

Health YOUniversity (National Institute for Fitness & Sport) who work in a location without a staffed onsite wellness center If you work in a location that does not have an onsite wellness center, we have partnered with NIFS (National Institute for Fitness & Sport) to bring you Health YOUniversity. Through Health YOUniversity, you can sign up for virtual wellness programs and/or work with a Health YOUniversity Coach by phone to develop a personalized fitness and/or nutrition plan. NIFS. then Physical Activity. Health Club Reimbursement All full-time associates and parttime associates regularly scheduled to work at least 20 hours per week, who work in a location more than five miles from a site with an onsite wellness center WellPoint encourages you to maintain a healthy lifestyle through regular exercise. If you work at a location that is more than five miles from a WellPoint fitness center, WellPoint will reimburse you up to $35 each month for individual membership dues to a company-recognized health club. then Physical Activity. National Walk at Lunch Day, contractors, temporary staff On the last Wednesday in April each year, all WellPoint locations participate in the Blue Cross and Blue Shield Association s National Walk at Lunch Day, a walking program designed to complement, not compete, with busy lifestyles. This walk is led by local senior leaders and shows how easy it is to incorporate physical activity into every day. via and on WorkNet beginning mid April. WellPoint Fitness Challenge The WellPoint Fitness Challenge begins the last Monday of April and is a fun and friendly competition that encourages you to get active. By logging your exercise sessions of at least 30 minutes, you can help your state win the challenge. You can also set up fitness teams with your peers and challenge other teams to a competition within a competition. Four states win one in each via and on WorkNet beginning mid April.

5 Better Health Digital Coaching Whether you and your family members are looking for help with exercise, eating, weight, stress, or getting better sleep, this set of interactive, self-paced online programs will provide personalized guidance. These online programs are available to you and your family members even if you are not covered under one of our medical plans. You'll get motivational tips and meaningful tools to help you make a rewarding and lasting change, and you'll gain the knowledge and support you need to reshape both your mind and body, and get more out of life every day. Better Health (Health Assessment & Digital Coaching)" in the Tools & Information box. Know Your Numbers Your health matters to us. Offering you a free health screening is one way we show our commitment to helping you take care of your health. We want to make it easy for you to be aware of your health and "know your numbers," because this is the first step toward better health. Know Your Numbers is a chance to have screenings done for blood pressure, total cholesterol/hdl, body mass index (BMI) and glucose (which gives information on your risk for diabetes). In the fall, we will offer onsite screenings at many of our locations and we will let you know how you can participate if you are unable to attend an onsite screening. on WorkNet beginning in the fall. Flu Shots Every year, we offer onsite flu shot clinics for our associates and provide coupons for community clinics for those who can not attend an onsite clinic. on WorkNet beginning in the fall. Health Assessment The health assessment is essentially a health calculator. Just answer some questions in the secure site about your current health, lifestyle and health history, and you'll receive helpful information and suggestions on how you can improve your health and achieve your goals. The health assessment is available to you and your family members, even if you are not enrolled in one of our Total Rewards medical plans. Well-Being and Work Life Balance Employee Assistance Program, spouses, domestic partners and dependents, regardless of medical benefit plan election The program offers six free confidential counseling sessions (per person, per condition) to help you or any household member with personal issues. Counselors are trained to help with such issues as marital or family distress, grief counseling, substance abuse and mental illness. This program can also assist with issues such as legal assistance, financial consultation, day care and elder care searches. EAP services are available 24 hours a day for emergency or urgent situations. Call Anthem EAP. Nursing Mothers Support Program New mothers have unique needs in the workplace. To help meet these needs, many of our locations are equipped with New Mother Rooms. For more information on the availability of these rooms, check WorkNet under Office Locations to find your site and review Amenities. Our Nursing Mothers Support program also provides help with breastfeeding support, nursing mothers' kits and a website with information and resources. 7 Call Work Life Educational Kits Special kits to help you manage a variety of life events are available to you at no cost. The kits contain sample products and practical info to help you more effectively manage specific life events. Anthem EAP Monthly wellness teleseminars Local Wellness Programs and Support State Wellness Teams On the third Thursday of each month, we offer interactive well-being teleseminars from nationally renowned presenters, physicians and authors. These are recorded and archived for convenient playback. In all our states, we have wellness teams made up of associate volunteers who help bring wellness to you at a local level. These teams help promote and implement our corporate programs, as well as deliver local programs and events such as health fairs and local wellness campaigns. If you are interested in becoming a wellness team member, contact your local team lead to learn how you can make an impact. See details in Online News each month.
