1 HealthWizard 5 is a seamlessly integrated suite of six software programs designed for organizations that promote wellness and fitness. Offering maximum flexibility, the six program modules can be purchased separately or as a complete suite. You can install one or two of the programs to begin with and add more as you expand your wellness business. The HealthWizard 5 suite includes six programs: Health History Fitness Profile Wellness Profile A short questionnaire used to determine a client s health status and their readiness for exercise. This program is included free with the purchase of any of the other programs. A collection of more than 40 tests that let you analyze and track physical health & fitness measurements like body weight, body fat, blood pressure, blood cholesterol, flexibility, muscular strength, and cardiovascular fitness. A questionnaire that analyzes and tracks lifestyle behaviors in the areas of exercise, nutrition, alcohol drinking, smoking, safety, and stress. SF-36 Health Survey Exercise & Diet Planner MicroFit Manager A questionnaire that analyzes and tracks clients perception of their health status. The SF-36 Health Survey is the most widely used patient-based health status survey in the world. A powerful program that performs complete dietary analysis and helps you create individualized exercise and diet plans. A collection of productivityenhancing features to help you run a more efficient and profitable wellness business.

2 Quick health screen Customizable The Health History program is a short set of questions based upon the popular Physical Activity Readiness Questionnaire (PAR-Q) developed by the Canadian Society for Exercise Physiology. It is recommended as a minimal screening tool for anyone starting an exercise program. This program can identify the small number of people for whom exercise might be inappropriate or those who should seek medical advice concerning the most suitable type of exercise activity. Free program Determine if doctor s approval is needed prior to assessment. You can customize the Health History program by adding an unlimited number of questions to the questionnaire. Using this feature, you can collect, track and report the client health history information that is important to your wellness program. The Health History program is included free of charge with your purchase of any one of the other HealthWizard 5 programs. With the Health History, you can Add unlimited number of user-defined questions. Print questionnaires and answer sheets for off-site group assessment. Create client reports that can be saved to a file and ed to the client or physician. Share a database on network or combine databases using Merge function. Identify existing medical conditions that could be made worse with exercise.

3 Measures physical health and fitness The Fitness Profile program can record, analyze, and track physical health and fitness measurements for males and females ages 6 to 90. The program includes over 40 tests in the areas of body shape and composition, cardiovascular fitness, muscular strength, and flexibility and you can add an unlimited number of your own tests to the program. The health and fitness tests included in the Fitness Profile program are based on the guidelines of the American College of Sports Medicine. Comparison scores come from the American Heart Association, the Canadian Department of Public Health, the President s Council on Physical Fitness and Sports, and data from over 100,000 MicroFit assessments. You can use the Fitness Profile program as a stand-alone software program and manually enter results from your own assessment equipment, or you can connect the Fitness Profile program to MicroFit s FAS-2 Interactive System, for computer-controlled measurement of body weight, body fat, blood pressure, heart rate, biceps strength, back flexibility, and cardiovascular fitness. Follows ACSM guidelines Dynamic, visual feed-back in real time Powerful graphics and animation capture your client s attention. Colorful reports can be viewed on screen, in print or PDF format. On-screen instructions guide you through the entire assessment.

4 The Fitness Profile program features over 40 health and fitness tests covering the areas of Biometrics Body height and weight Body mass index Waist-to-hip ratio Circumference measurements Blood pressure Heart rate Cholesterol (total, HDL and ratio) Body Composition Skinfold Adult (three-, four-, and seven-site protocols) Youth (two-site protocol) Circumference (large adult) Enter percent body fat score determined from another device Cardiovascular Fitness Bike protocols Adult (Astrand, U.S. Air Force, user-defined) Youth (modified Astrand) Treadmill protocols (Bruce, Balke, user-defined) Step test protocol Enter cardiovascular fitness score determined from another device Strength and Flexibility Biceps strength Hand-grip strength (both hands combined) Push-ups, pull-ups, and sit-ups Youth field tests of muscular strength and endurance Trunk flexibility (sit-and-reach protocol) The Fitness Profile Lets you add an unlimited number of user-defined tests. Provides test protocols and comparison scores for males and females ages 6 to 90. Interfaces to the FAS-2 Interactive System for computer-controlled health and fitness measurements. Can create multiple assessment formats that define the type of test, test order, and report style. Creates client reports, history graphs, and group reports. Reports can be saved to a file and ed to the client or physician. Allows you to edit the report interpretation text (includes special insert field codes). Lets you merge the results into other databases or share a database on a network.

5 Lifestyle behavior assessment The Wellness Profile program examines lifestyle behaviors that can affect an individual s health and longevity. The questionnaire focuses on the areas of exercise, nutrition, alcohol drinking, safety, tobacco use, and stress. The Wellness Profile analyzes your client s current health behaviors and provides specific recommendations to reduce the risk of future disease or disability. The Wellness Profile consists of 46 questions and can be completed in about 5 minutes. It is more motivational than the typical health risk appraisal because everyone can achieve a perfect score and not be penalized for uncontrollable factors like family history of disease. The Wellness Profile program also differs from other health risk appraisals because the report recommendations are specific to the client s responses, not just a list of standard good-health practices. The Wellness Profile is the result of a joint development project between MicroFit and the Stanford University School of Medicine. Technical information for the program was derived from a number of professional organizations, including the U.S. Centers for Disease Control, American Heart Association, American Cancer Association, and the American College of Sports Medicine. With the Wellness Profile, you can Add unlimited number of user-defined questions. Edit user-defined report text and general health information comments. Print questionnaires and answer sheets for off-site group assessment. Create client reports, history graphs, and group reports. Share database on a network or combine databases using Merge function. Find out how often your client demonstrates good eating habits.

6 Measure health status 5-minute questionnaire The SF-36 Health Survey provides a comprehensive and efficient way to measure health from the client s point of view. It is an excellent complement to the Fitness Profile program because it tracks non-physical changes that result from a health improvement plan. For example, there may be periods of time when a client will show little or no improvement in their blood pressure or body fat measurements. However, they may report significant improvements in areas measured by the SF-36 Health Survey such as reduction in pain, improved social life, and greater productivity at work. Physical & psychological Measure your client s perception of his or her health. The most widely used patient-based health status survey in the world, the SF-36 Health Survey has proven useful in screening individual clients, differentiating the health benefits produced by different treatments, and monitoring groups of people. The SF-36 Health Survey questionnaire can be completed in about 5 minutes. It can be self-administered by persons 14 years of age and older or be administered by a trained interviewer in person or by phone. Has the client s physical condition affected their work? With the SF-36 Health Survey, you can Determine the general physical and mental health of males and females age 14 and above. Print questionnaires and answer sheets for off-site group assessment. Create client reports, history graphs, and group reports. Reports can be saved to a file and ed to the client or physician. Share database on a network or combine databases using Merge function. Does the client s physical health limit their activity level? SF-36 Health Survey (Medical Outcomes Trust)

7 Individualized planning Multimedia instructions The Exercise and Diet Planner module is loaded with powerful features that enable you to analyze your client s diet and create individualized exercise and diet plans for them. The Exercise Planner includes more than 200 exercises for improving strength, flexibility and cardiovascular fitness. Most of these exercises have multimedia instruction videos that show the proper lifting and stretching techniques. To select an exercise for a specific muscle group, you simply click on the appropriate body part on the onscreen anatomical figure. You then choose one or more exercises from the exercise list and define the weight to use as well as the number of sets and repetitions. The program allows you to create a specific exercise plan for one individual or a more general plan that can be used with groups of clients. With the Diet Planner, you can create food plans or analyze the caloric and nutrient content of a client s diet. The program includes data on over 6000 foods and information on 30 nutrients. It also has an advanced recipe database that can be sorted and viewed by cuisine, meal type, or key ingredient. You can customize the Diet Planner program by adding your own foods, recipes and diet plans. The program lets you create customized diet plans for a specific client or more general plans that can be used with groups of clients. Diet analysis Create individualized exercise and diet plans. Select an existing exercise plan template or create your own. Analyze the nutritional content of individual foods, meals and daily intake. View the video gallery for proper lifting and stretching techniques.

8 Facilitate marketing Efficient management The MicroFit Manager program is a collection of features and reports designed to help you run a more efficient and profitable wellness business. Mail Merge With this feature, you can create letters and mailing labels for some or all of the clients in your database. Clients can be selected by group, gender, age, contact date, counselor, and date of last health assessment. The mail merge feature provides a quick and easy way to market your services and products to a select group of clients in the HealthWizard 5 database. Calendar A convenient and efficient way to keep track of business meetings and tasks, the calendar feature lets you log appointment times for health assessments, counseling sessions, health improvement classes, and other important meetings and events. The calendar also includes a task list feature that helps you manage your to do list. Client List Report This feature lets you print a list of clients from your database along with important information such as ID number, phone number, address, counselor, contact date, and date of last test. You can use the report to efficiently manage routine client contact tasks. For example, you can print a list of the phone numbers for those clients scheduled to be contacted next month. Or you can print the address and last health assessment date of all clients assigned to a particular counselor. Database reports Calendar helps you keep track of your assessment appointments. Fast and convenient way to create letters and mailing labels. Print a list of clients based on criteria like counselor or last test date.

9 Software Architecture The HealthWizard 5 program utilizes a Microsoft Access database that can hold records on more than 20,000 clients who collectively have over 100,000 health assessments. Multiple computer stations on the same network can read and write client information to the same central database; computers not on the network can share information with the central database through a merge process.
