3 Program Description Public Health Nurse Ready (PHN Ready) is an online certificate program for RNs working in public health, or RNs desiring to know about how public health works. LPNs and other RN's may find the certificate of interest, as well. All courses are self-paced and online, and reflect the Tier core public health competencies set forth by the Quad Council of Public Health Nursing Organizations. PHNReady Public Health Nurse Ready The certificate consists of 8 required CORE online courses (which should take roughly 11.5 hours to complete) and 9 ELECTIVE courses (from which you may choose an additional 4 hours of education). Many of the courses provide continuing education credits. These credits (or contact hours) are awarded by course. The PHN Ready certificate does not provide credits, nor does the New York State Department of Health Learning Management System. Empire State Public Health Training Center (ESPHTC) is a partnership between two accredited schools of public health: the University at Albany School of Public Health (UA-SPH) and the University at Buffalo School of Public Health and Health Professions (UB-SPHHP). The mission of ESPHTC is to improve the public health infrastructure in New York State by strengthening the current and future public health workforce, particularly within medically underserved areas such as rural communities and American Indian Reservations. 7 Program Requirements Requirements: PHN Ready is targeted toward RNs working in public health, and to RNs desiring to know about how public health works. LPNs and other RN's may also enroll. There is no residency requirement; students may enroll from any geographic location within the US states or territories. Enrolling: Candidates should enroll through the New York State Department of Health Learning Management System website at Please see complete REGISTRATION DIRECTIONS located here in the Student Guidebook on pages The New York State Department of Health Learning Management System (LMS) is a registration system used to group many courses together. The courses are from various educational institutions and are not housed on the LMS. You will need to go into the courses systems and create a username and password. Helpful Hint: Use the same username and password for each system. You may use the space below to keep track of your username(s) and password(s). 8 Tracking & Application Tracking your progress: Participants may want to track their progress to ensure they meet the requirements for core and elective courses. Page 9 includes a tracking sheet that may be used for this purpose. A few PHN Ready courses are self-certify, where upon completion of a short quiz, the student will be automatically recorded as having completed the course in the Learning Management System. Detailed self-certification instructions are included on Page 18. PHN Ready Evaluation & Request for Certificate Once you have completed all core and elective requirements, you must complete a brief PHN Ready Evaluation & Request for Certificate, which can be found at This evaluation will take minutes to complete and is designed to improve the PHN Ready certificate program and to assist us in ensuring that PHN Ready stays relevant and remains a useful tool for nurses. All responses and information will remain confidential. After submitting the PHN Ready Evaluation & Request for Certificate, please expect to receive your certificate within 2-4 weeks. Questions? 10 Registration Directions Learning Management System Step 1: Register on LMS To register for the Public Health Nurse Ready certificate program, please go to the New York State Department of Health s Learning Management System: Click on Click Here on the left upper hand side. (below Login) Step 2: Create a Profile You will be asked to create a profile. Please complete all required fields and include your agency name in the first line of your work address, in addition to the Agency drop down menu. (You DO NOT need to take any assessments. Those are optional and are not a part of this certificate.) Once you have completed your profile, please click on Submit your Registration. If you are already registered on the LMS, you DO NOT need to create a new profile. 10

12 Registration Directions Core Courses Step 3: Click on the title of the Program to begin registering for all of the required CORE courses. Step 4: Please read the information on the page and scroll down to the bottom of the page and click Begin Program Enrollment. 12

Step 6: After clicking Enroll, you must select your credit type. NOTE The LMS does not provide credits. The courses and their educational institutions provide the credits. 13

Whenever you return to this, or any other ESPHTC course the system will remember you and ask you to login using the Username and Password you selected during the PHTC registration. Once you login you will be launched directly into the course you requested. NOTE Upon completion of your courses, NYLearnsPH.com LMS Administrators will be made aware of your status and your course record will be updated within one week of completion. If you have enrolled successfully, you will see a message indicating that you have successfully registered for the course. To view the courses you have registered for, click on My Courses, then click on Current Courses or Current Programs. NOTE When you select Launch Course, you will be taken to the website of the institution that created the course. The courses that make up this certificate are largely housed on the Public Health Training Center Online page, not the LMS. 20 Orientation to Public Health (NYNJ-201) Core Description: Orientation to Public Health is a web-based course that provides learners with a basic understanding of the mission and functions of public health. Designed to orient newly hired support and technical staff to the field, the course consists of two parts: Part One introduces the mission and six obligations of public health. Part Two explains the ten essential services. The course uses the metaphor of a new worker orientation (computer-based) to present information. By reading s, completing assigned tasks, and participating in simulated desktop conferences, the learner discovers that public health is very much a part of everyday life, and learns how many different agencies work to carry out its mission. Upon finishing the course, you may print out a certificate of completion. 21 Exploring Cross-Cultural Communication: Communicate to Make a Difference (NYNJ-042) Core Description: This web-based course invites learners to spend time thinking about and developing their own responses to a variety of ideas and situations about culture, communication and public health. Learners will explore the meaning of culture, methods of communication, and strategies for communicating more effectively by taking part in virtual group conferences, reading and responding to simulated s, and utilizing resource documents. 22 Field Epidemiology (UACPHP-206) Core Description: Field Epidemiology provides an overview of field epidemiology and the steps of an outbreak investigation. The course describes the context of field epidemiology and its relationship to epidemiology in general, and reviews some basic techniques typically used in data analysis in the field. This course is intended for public health nurses, sanitarians, and other public health professionals who may play a role during an outbreak investigation. 32 Evaluating a Public Health Program (NYNJ-277) Elective Description: "Evaluating a Public Health Program" is the last in a series of trainings dealing with the development and evaluation of public health programs. This training serves as a comprehensive tutorial on the Evaluation of a Public Health Program. The process of Program Evaluation continues the use of pertussis reduction in Lakeshore County as an example program and utilizes the logic model developed in the "Introduction to Logic Models" training. The primary focus of the course is to explore the six steps and the four standard groups in the Center for Disease Control's Framework for Program Evaluation. 33 From Evidence to Practice: Elective A Systematic Approach to Address Disparities in Birth Outcomes (EBPH-075) Description: "From Evidence to Practice" is a self-paced, web-based course, that teaches the evidence-based public health decisionmaking process by plunging learners into a realistic situation. In the course, learners are asked to understand and employ the evidence-based framework to research and develop an intervention strategy that addresses disparities in birth outcomes in a local community. Learners make a series of choices about finding data to accurately describe the problem, evaluate interventions based on evidence and applicability to the community, and defend their choices. The course outcome securing funding for interventions chosen depends on sound evidence-based decision-making. 35 Elective Messenger Chronicles: Introduction & The Four C s (NYNJ-182a) Description: The Messenger Chronicles: Effective communication strategies for difficult conversations presents a new framework for communication in difficult situations. This framework shifts focus away from managing difficult people towards an understanding of the process of difficult conversations and accepting responsibility for one s own performance. Given realistic situations and real-world conversations, learners will experience communication strategies and practical techniques in context. This course is organized into a number of modules that simulate realistic conversations. Many of the conversations are split into three sections: Read, Think and Analyze. Learners will read a conversation and then will be asked to think about certain aspects of the conversation by answering questions or engaging in activities. Further information can be gained from an interactive analysis of the conversation. This training serves as an introduction to logic models with information about why logic models are important, an explanation of the model components and real-world examples of how logic models are built.
