Visit co.delaware.i.us > Your Govermet > Health & Huma Services > Huma Resources > Workplace Health Cliic ad click o Book NOW! to access the olie schedulig tool. Istructios for First-time Users Click o Create a Accout i the tool ad complete the required iformatio. Please ote: The logi ca be shared by the etire family; however, each idividual family member must create a uique password. Oce all iformatio has bee etered, click Create a Accout. A ew scree will appear. Follow the directios to proceed. Select Service A ew scree with a Select Service drop-dow box will appear. Click o the box to select the service (e.g., ew patiet cosult, illess visit, mior ijury, flu shot) that best describes the reaso for your visit. Select Appoitmet Time A caledar will pop up showig available appoitmets (i white boxes). Click o the date that works best for you. Please ote: To see other optios, click o the << Previous or Next >> buttos to select a ew date o the caledar. A ew Select Appoitmet Time box will appear. Choose a time by clickig i the white space ext to the time you prefer. A scree will ope to cofirm your appoitmet iformatio. Please add the specific Reaso for Your Visit i the desigated box ad ay ecessary details for the provider i the Special Istructios box. Click Fialize Appoitmet. Three parters the City of Mucie, Delaware Couty ad Mucie Commuity Schools have cotracted with IU Health to provide this dedicated healthcare ceter to serve the parters employees, qualified retirees ad their family members. Healthcare ceter services are available to ayoe who participates i a health pla offered by oe of the three parters. How much does it cost to use the healthcare ceter? There is o out-of-pocket cost to you for office visits, laboratory services or medicatios provided by the ceter. Where is the healthcare ceter located? Workplace Health Parters is coveietly located south of IU Health Ball Memorial Hospital i Mucie. The address is 120 N. Tillotso Ave. Who staffs the healthcare ceter? Health ad welless services at the ceter are provided by qualified professioals employed by IU Health. Workplace Health Parters offers a wide rage of health services. 6 Frequetly Asked Questios What if I eed medicatios? Does the healthcare ceter offer pharmacy services? Approximately 100 commoly used geeric medicatios are available from Workplace Health Parters at o out-ofpocket cost to you. These i-formulary medicatios will be provided wheever possible. Prescriptios will be writte by the healthcare ceter practitioer for medicatios that are ot o the ceter s formulary. You will receive your medicatios from the practitioer i the healthcare ceter wheever possible. If you eed a medicatio that is ot available o the healthcare ceter s formulary, you will receive a writte prescriptio to take to your local retail pharmacy. Refills of maiteace medicatios will be available. With the healthcare ceter practitioer s approval, you may receive 90-day supplies of medicatios for chroic illesses through the ceter s mail-order service. Oly i-formulary prescriptios writte by the healthcare ceter practitioer are eligible for this service. Commo over-the-couter (OTC) medicatios, such as allergy remedies, vitamis ad pai relievers, will be provided for seve days (o refills). For cotiued use of OTC medicatios, you will be give a prescriptio for your records, which may allow you to access your persoal isurace accout fuds for these medicatios. Please ote: Workplace Health Parters will oly fill prescriptios writte by the ceter s providers. Tell me more about the health coachig. What is it ad how ca it help me? Persoal health coachig ca help you make better lifestyle choices that ca lead to a more healthy life. The healthcare ceter s certified health coach will support you o your jourey ad provide accoutability to help achieve your health goals. Coachig sessios are scheduled at your coveiece. If you are a employee or qualified retiree of oe of the three sposorig healthcare ceter parters (City of Mucie, Delaware Couty, Mucie Commuity Schools) ad participate i the parter s health pla, you are eligible to use Workplace Health Parters. Your family members erolled i the health pla may also use healthcare ceter services. Ca I brig my childre to Workplace Health Parters for care? Yes. If your childre are icluded i your health pla, they are eligible to visit the ceter for services. While we ecourage you to maitai a relatioship with a pediatricia for ogoig care of youg childre, the healthcare ceter s medical staff will be glad to treat your child if immediate care is required ad your pediatricia is ot available. Will the healthcare ceter share my persoal medical iformatio with my employer? Absolutely ot. Workplace Health Parters is a physicia practice of IU Health. Your privacy is protected, ad the services provided are strictly cofidetial by law uder the Health Isurace Portability ad Accoutability Act (HIPAA). Will Workplace Health Parters work with my doctor, if ecessary? Yes. If you sig a coset/release form, the healthcare ceter staff will share appropriate iformatio regardig your visit with your doctor(s). What if I eed to see someoe whe the healthcare ceter is closed? If you have a medical emergecy, dial 911 or go to the earest emergecy room
