1 PROJECT CONCEPT PROPOSAL. Requesting agency:. Project title: 3. Cluster/sector/cross-cutting issue:. Geographic areas of implementation targeted with emergency funding: 5. WASH Intervention for the prevention of faecal-oral disease outbreak in targeted villages and schools in postcyclone Tomas affected areas in Fiji. WASH Cluster Up to affected communities and 5 boarding schools including most high risk communities and 5 boarding schools in remote parts of Napuka, Rabi Island, Taveuni Island, Karoko and Koro. a. Total individuals: Up to 5, beneficiaries in communities and boarding schools including 3,9 beneficiaries in most vulnerable communities and 5 highest risk boarding schools. 6. Nil Background: Cyclone Tomas hit Fiji from th March and over two days left a trail of destruction through some of the most remote and vulnerable villages in Northern and Eastern Divisions of Fiji. The government and people of Fiji have demonstrated resilience in combating the disaster, but serious problems still remain. A number of cases of typhoid and diarrhoea have been reported in various cyclone affected communities and boarding schools giving evidence of an increase in water related disease outbreaks. Almost immediately after the cyclone, the Ministry of Health and WHO were on high alert for an outbreak of typhoid, diarrhoea and other faecal-oral transmission diseases in the affected areas. The concerns regarding such outbreaks are most acute in certain high risk areas or Hot Spots, which include Napuka, Rabi Island, Karoko, Taveuni Island and Koro. WHO has set up a typhoid monitoring system in conjunction with the Ministry of Health to monitor the potential outbreak of disease in cyclone affected areas. Page of 5
2 With many basic water and sanitation facilities either destroyed or damaged by the cyclone, the risk of contracting water and sanitation related diseases has became exceptionally high in many hard hit and very remote communities and boarding schools, the Hot Spots. It is a priority to get to these hard-to-reach villages and boarding schools to apply appropriate WASH interventions and provide adequate and holistic services to meet minimum standards of access to safe drinking water and adequate sanitation for immediate to longer-term prevention of faecal-oral disease outbreaks. Members of the WASH Cluster in the emergency meeting on st April agreed that the priority for WASH emergency interventions should be to prevent faecal-oral transmission diseases such as typhoid and diarrhoea by conducting specific targeted interventions in the hot spots identified. Key members recognized the need for a holistic WASH intervention to ensure the potential outbreak of faecal-oral transmission diseases can be prevented in these hot spots. It was agreed that UNICEF will work with the MoH, Red Cross, and other interested NGOs, to take the lead in preparing holistic WASH interventions in the targeted hot spot communities and boarding schools. Goal: To prevent outbreaks of water and sanitation related diseases in targeted high risk communities and boarding schools affected by cyclone Tomas. Objectives: A. To improve the existing water, sanitation and hygiene conditions of the most vulnerable communities affected by cyclone Tomas; B. To promote sustainable and appropriate Village Level Operation and Maintenance (VLOM) mechanisms for water and sanitation systems, and; C. To promote culturally acceptable and appropriate hygiene practices to prevent fecaloral transmission diseases among the most vulnerable beneficiaries such as children and women in the targeted communities and schools. Strategies:. Participatory approach to Total Community Participation on WASH Interventions in Emergency: Conduct stakeholders meetings with communities, local authorities, key government ministries (MoH, NDMO, Water Authority of Fiji, etc.), Red Cross, local NGOs/CBOs, in national, divisional, sub-divisional, and local community level. Detailed assessments of targeted locations shall be conducted by all key stakeholders, including the local communities and school boards, to determine the most appropriate WASH interventions for prevention of further outbreaks of faecal-oral transmission diseases. Interventions for prevention of outbreaks are expected to include improvement of sustainable basic WASH services and appropriate and effective hygiene promotion methodologies;. Professional approach on improvement of basic water and environmental sanitation infrastructure and services: Professionals with proper engineering knowledge on rural water and sanitation infrastructure and services are required to lead the participating communities to improve basic WASH facilities and services. Potential implementing partners with appropriate engineering capacity are Water Authority of Fiji (Rural Sector), Fiji Army Corp of Engineers, Fiji Society of Registered Engineers and participating NGOs in the WASH Cluster. Full engagement of local communities with the professional engineers is mandatory for the success of the WASH Hardware interventions. Depending on the levels of required assistance and the outcome of the stakeholder meetings at each community and boarding school, appropriate WASH Hardware interventions shall be drafted and presented by the professional engineers to all stakeholders for review and comments before implementation; 3. Integrated Hygiene Promotion and Village/School Level Operation and Maintenance (VLOM/SLOM) by Village/School WASH Committee: Develop appropriate, sustainable, Page of 5
3 and effective WASH committees at village and school level in each targeted site to ensure hygiene standards of individuals and communities are improved and basic water and environmental sanitation is operating and maintained in a proper manner. Encourage continual improvement in WASH in each and every community and school targeted on prevention of faecal-oral transmission diseases outbreak in future. Potential implementing partners are Fiji Red Cross, International and local NGOs who participate in the WASH Cluster and have experience in Fiji Hygiene promotion methodologies who are accepted and supervised by the MoH;. Periodic Reviews, Monitoring and Evaluation: Conducted independently by a selected organization working in conjunction with the WASH Cluster for a transparent, timely, participatory and comprehensive review of the WASH Emergency Intervention on post- Tomas disease outbreak prevention. Activities: I. Conduct detailed assessments of the hot spot sites to determine the most appropriate WASH interventions for each site and identify key implementing partners of appropriate capacities, and, where assessment indicates a need; II. Supply and distribute family and personal hygiene kits as required to selected vulnerable families in communities and school children in boarding schools; III. Repair and/or construct rainwater harvesting systems for selected households to obtain a safe water supply for drinking and cooking; IV. Repair and improve existing water infrastructure or install appropriate alternative water supplies in selected boarding schools for drinking, cooking and domestic washing and cleaning; V. Repair, improve or construct appropriate drainage systems in selected communities and boarding schools to prevent flooding and wastewater contamination; VI. Repair, improve or construct culturally appropriate and functional water-seal toilets with septic tanks in selected communities and boarding schools; VII. Repair, improve or construct appropriate solid waste facilities in selected communities and boarding schools; VIII. Setup and provide sustainable development to Village (and School) Level Operation and Maintenance (VLOM) WASH committees in selected communities and boarding schools to ensure all water and sanitation facilities are well operated and maintained; IX. Provide suitable and sustainable development to WASH committees on appropriate hygiene promotion to targeted communities and boarding schools; X. Conduct baseline survey, periodic reviews, regular monitoring and evaluation of WASH interventions in targeted areas; Expected Outcomes and Indicators: I., beneficiaries in 3 communities and 5 boarding schools have reduced potential of contracting faecal-oral transmission diseases due to improved water and sanitation conditions and practice of appropriate hygiene behaviors; # beneficiaries in communities and boarding schools with reduced potential for contracting faecal-oral transmission diseases due to improved water and sanitation conditions and practice of appropriate hygiene behaviors; II. 3, beneficiaries in the highest risk communities and 5 school children in the 5 highest risk boarding schools avoid an outbreak of faecal-oral transmission diseases by major improvement in water and sanitation conditions and practicing appropriate hygiene behaviors; # (%) beneficiaries in the highest risk communities and # (%) school children in the 5 highest risk boarding schools avoid an outbreak of faecal-oral transmission disease due to a major improvement in water and sanitation conditions and practicing appropriate hygiene behaviors; Page 3 of 5
