3 INTRODUCTION The Australian Private Hospitals Association (APHA) seeks to stimulate, encourage and recognise the pursuit and achievement of excellence among its members. During the Inaugural Private Hospitals Week in April 2010, the APHA announced a new Awards program to recognise and reward the valuable contribution private hospital staff play every day in making lives better. Recognising that hospital staff play a variety of roles that are vital in providing high quality care and support for patients, family members and carers, working independently and in teams, these Awards will recognise those who have made an outstanding contribution to patient treatment and care, as well as the management of hospital facilities. The APHA Valuing Private Hospital Staff Awards will recognise achievement in three categories: 1. Excellence and Innovation in a clinical role - individual 2. Excellence and Innovation in a non-clinical role - individual 3. Excellence and Innovation in a clinical or non-clinical role team Recognition under the APHA Staff Award scheme places the recipients at the forefront of Australian healthcare and in addition, the Awards also provide personal recognition for outstanding achievement. I am pleased to acknowledge HESTA Super Fund s sponsorship of these awards. A prize of $10,000 for each individual category and $10,000 for the outstanding team award winner provides yet another reason for your hospital to nominate your exceptional staff. 5 APPLICATION PROCESS & INSTRUCTIONS CATEGORIES There are 3 categories for the APHA Valuing Private Hospital Staff Awards. 1. Excellence and Innovation in a clinical role - individual 2. Excellence and Innovation in a non-clinical role - individual 3. Excellence and Innovation in a clinical or non-clinical role team PRIZES There is a total prize pool of $30,000. $10,000 will go to each individual award winner and $10,000 to the team winner. The CEO of the hospital that wins the team award will be responsible for determining how to allocate the $10,000 across the team. ENTRY & DEADLINE Individuals and teams may not nominate themselves for an Award. Submissions must be made by someone who has first-hand knowledge of the individual or team s effort. Submissions must be signed off by the hospital s CEO or equivalent on the provided Cover Sheet. Submissions may not be ed. Submissions remain the property of APHA and information & photos provided may be used for promotional purposes. Deadline: Submissions must be mailed to APHA & postmarked by 31 January Mail to: APHA Valuing Private Hospital Staff Awards Judging Panel Australian Private Hospitals Association PO BOX 7426, Canberra BC ACT 2610 ELIGIBILITY Staff from all APHA Full Members are eligible to enter an application submission. APHA members must have membership dues paid in full for the financial year. Individuals and team members must be employees of an APHA member hospital at the time of entry. Hospitals that are not members of APHA are not eligible to enter the Awards. The Award submissions will cover the period 1 January 2010 to 31 December Only one submission per category is allowed per hospital. Sponsored by 5

6 INSTRUCTIONS Entry submissions must follow the prescribed format / outline specified for the category of entry. Submission must include the completed Cover Sheet & applicable Check List provided at the end of this prospectus. The Cover Sheet and Check List are not included in the page limit for entries. The application must be clear and concise. The appropriateness and completeness of the application is a critical part of a successful entry. The application MUST address the Assessment Criteria applicable to the Award category. The application may include information not directly referred to within the criteria, ONLY if it would add to the value of the application. Wherever possible, statements of achievements MUST be supported with quantitative or qualitative data. Use ONLY A4 size paper throughout. Each page MUST be clearly numbered. Use only size 11 Arial font in full justification. Only applications in the required format will be considered. Submissions must adhere to the category page limit: o Individual Clinical Excellence Award 4 pages maximum o Individual Non Clinical Excellence Award 4 pages maximum o Team Excellence Award (clinical or non-clinical) 4 pages maximum There is no allowance for appendices. All applications MUST be accompanied by at least 10 colour photographs, diagrams or images (as both high resolution electronic images on memory stick or CD and as hard copy images). These MUST be relevant to the application, and may be used should the application prove successful as a Finalist for promotional purposes. Applications remain the property of APHA and the information provided may be used for promotional purposes. APHA reserves the right to request further information from applicants. 7 JUDGING Judging will be by the direct consideration of each application, plus (if necessary), discussions with the nominator. An Award will only be granted where warranted by the standard of entry. That is, should all applications in a category fail to meet the required standard; no Award will be presented in that category. The judges decision will be final and no correspondence will be entered into. The judging panel consists of the following persons: 1. Chief Executive Officer, APHA; 2. Trustee Director, HESTA, 3. CEO of Health Workforce Australia 4. APHA Workforce Taskforce Chair 5. Director, Marketing & Communications, APHA Should any of the above persons, or the organisations they represent, be nominated for an Award, those persons will be required to step aside from the judging panel and be replaced by a nominee of the National Board of the APHA. The judging panel commits to confidentiality and each member of the panel is required to declare and avoid any possible conflict of interest, in writing. AWARD ANNOUNCEMENT The finalists and winner will be notified by phone and post, between 23 March 2011 and 30 March 2011, by a representative of APHA and their details will be released to the media during Private Hospital Week (4-8 April 2011). TERMS AND CONDITIONS The prizes are not transferable or exchangeable. APHA reserves the right to use any entries, photographs, media and / or articles of the finalists and winners of this award for promotional purposes. APHA will contact the intended finalists / winners to obtain their approval to use any entries, photographs, media and / or articles for future promotional, marketing and publicity purposes in any media with or without any further reference, payment or other compensation to the finalists / winner. Nominators and finalists must agree to be contacted by a representative of APHA to verify any information submitted and the eligibility of the nominee. 8 CATEGORY: INDIVIDUAL EXCELLENCE - QUALITY, EXCELLENCE & INNOVATION IN A CLINICAL ROLE AIM: The aim of the Award is to recognise those private hospital staff in a clinical role who have excelled in the provision of clinical treatment and demonstrated excellence in patient care. TIME FRAME: The application must ONLY relate to the period between 1 January 2010 and 31 December FORMAT / OUTLINE: In addition to the general instructions for applications, the written application for this Award must follow the prescribed format: I. Executive Summary Summarise what was done & how II. Identify the Specific Problem / Challenge What improvements were needed to achieve better clinical outcomes & patient care? III. Process & Approach Detail the process(es), innovation and leadership the nominee offered to improve service, quality of care, patient outcomes or leadership Describe the approach How did the staff member go about implementing the changes to clinical treatment & patient care. IV. Results & Outcome What is the measurable impact on patient outcomes, patient community and the hospital? How were results monitored / measured? Has this initiative or the nominee s leadership been recognised and applied across the hospital? Has the nominee s leadership or exceptional service enhanced team morale, strengthened staff connectivity, supported continuing pride and professional development? Sponsored by 8

9 CATEGORY: INDIVIDUAL EXCELLENCE - QUALITY, EXCELLENCE & INNOVATION IN A NON-CLINICAL ROLE AIM: The aim of the Award is to recognise those non-clinical staff who have excelled in the provision of administrative, hospitality or maintenance services that improve efficiency, patient and family experiences, staff development or other initiative. TIMEFRAME: The application must ONLY relate to the period between 1 January 2010 and 31 December 2010 and may involve either a new administrative, hospitality or maintenance service that has improved patient experience, hospital procedures or staff development. FORMAT / OUTLINE: In addition to the general instructions for applications, the written application for this Award must follow the prescribed format: I. Executive Summary Summarise what was done & how II. Identify the Specific Problem / Challenge What improvements were needed to deliver better services and support? III. Process & Approach Describe the approach How the nominee went about implementing any changes to operations, administration, service delivery. Describe any perceived innovations. Are there any aspects of the service that you would consider unique? Describe the demonstrated leadership, innovation or excellence that improved personal, team or hospital services and operations IV. Results & Outcome Use evidence to describe the measurable impact on hospital operations, patient outcomes, patient community and staff How were results monitored / measured? How has the nominee s leadership or exceptional service enhanced team morale, strengthened staff connectivity, supported continuous improvements and/or is connected to better patient care and experience?
