Bldg., no later than May 30th, 2013 at 4:00 p.m. Beginning in 2013, Peoria Public Schools will recognize someone from every facet of our organization that contributes something remarkable everyday to our students, our schools and our District. For certified staff members, this recognition will continue to be in the form of the Donald M. Phares Award. The District winner of each category will be our representative for the Those Who Excel competition, sponsored by the Illinois State Board of Education. Help us recognize REMARKABLE at Peoria Public Schools!

2 NOMINATION GUIDELINES 2013 PEORIA PUBLIC SCHOOLS RECOGNITION PROGRAM (INCLUDING DONALD M. PHARES AWARD) Program Description The Peoria Public Schools Recognition Program annually recognizes and honors people who have made outstanding contributions to Peoria Public Schools and its students. A selection committee comprised of District staff members and community leaders is convened in the spring to evaluate and score the nominations. After a winner is chosen for each category, the nominator and the nominee s administrator will be notified about the nominee s application results. A course of action to award the winner will be planned with the nominee s school and fellow staff members. Awards may be announced as a surprise to the nominee. The District award winner for each category will then represent the District in the Illinois State Board of Education s Those Who Excel/Teacher of the Year program. Nominations can be made in seven categories: Student Support Personnel (Certificated) School nurse, psychologist, social worker, guidance counselor, media/librarian/audio visual, or speech/language therapist who spends more than 50 percent of time in the indicated field. Educational Service Personnel (Non-Certificated) Includes aides, cafeteria workers, custodians, maintenance workers, secretaries, bus drivers, crossing guards and others. Community Volunteer individual member of an Adopt-A-School organization, Reading Buddy, community volunteer or supporter and others. Classroom Teacher (Certificated) Pre-school, primary, middle or high school teacher who spends more than 50 percent of the time instructing students. School Administrator Principals, Assistant Principals, Coordinators, Directors, Central Office Administrators or others whose responsibilities are more than 50 percent administrative. Team Includes, for example, teams of teachers or administrators, civic organizations, parent organizations, school boards, booster clubs, etc. Early Career Educator teachers in their 2 nd 5 th years of teaching may be nominated for the Outstanding Early Career Educator award. The criteria for this award is the same as for any teacher, but publicly recognizes the accomplishments of a teacher in the early years of their careers.

3 REQUIRED NOMINATION FORMAT Each nomination submitted must follow the format described below. Nominations that do not meet these requirements will be returned. Nominations submitted to the Peoria Public Schools Public Relations Department should be hand delivered by Friday, April 20 th, 2012 at 4:00 p.m. (delivery via Pony Mail is strongly discouraged) 1. Nominations may be made by any person or organization. The nominees must be directly involved with Peoria Public Schools and can be nominated only for their positions or roles in the education community during the school year. 2. A form must be completed for each nominee. In order for nominations in more than one category to be made, the form and cover sheet may be duplicated. 3. The first two pages of all completed nomination packets must consist of a) the cover page (use the cover of this packet), and b) the nomination form. The subsequent packet sections should follow in numerical order. 4. Each section of the nomination packet has a maximum length requirement that will be enforced. 5. Except for letters of recommendation, which may be single-spaced on letterhead or personal stationary, each nomination should be double-spaced with one-inch margins on plain, white 8 ½ by 11-inch paper, one side only. Type size should be at least 12 point. 6. Center the name of each section (e.g. Background) at the top of its first page. Put the nominee s name in the upper right corner of all nomination packet pages. 7. All materials should be stapled together in the upper left corner. 8. Nominations must be hand delivered by Thursday, May 30th, 2013 at 4:00 p.m. Addressed to: Peoria Public Schools - Administration Building c/o Public Relations Department 3202 N.
