2 FACILITIES Last Door offers 100 residential primary-treatment and transitional-living beds for males ages 13 and over. Treatment facilities are located in New Westminster, BC, while affiliate offices offer Continuing Care Services across Canada and the USA. 3 APPROACHES APPROACHES APPROACHES Social Model Last Door Social Model Program offers clients and their families an evidence-informed opportunity to experience recovery in an established and vibrant recovery community. Community Recovery at Last Door is about gaining connectedness with a healthy community. Clients access peers, addiction professionals, medical resources and activities that ensure all their needs are met. Portability Recovery is an experiential-learning process that teaches clients lifelong skills. Last Door provides a foundation for ongoing recovery by drawing on the collective experience and energy of alumni, clients, families, staff and the community. Newly learned recovery skills are transferable post treatment to the client s home community, workplace or school, optimizing long term recovery BREAKING THE CHAINS OF ADDICTION 3

5 ADULT PROGRAM Last Door Adult Program is a long-term residential addiction treatment centre for men aged 19 and over. Minimum Program length at Last Door is 90 days. Last Door Adult Program provides experience based learning that results in a positive change of values, beliefs and behaviours. Last Door Adult Program daily group therapy is divided into generational groups; 19 to 30 year olds and over 30s. Life-stage streaming promotes better peer interaction. Client Profile Alumni include petrochemical industry employees, government employees, teachers, firefighters, military personnel, physicians, lawyers, nurses, students and retired persons. Referrals of men with addictions and co-occurring disorders come to Last Door from across North America. ADULT PROGRAM Last Door provides two weekly alumni groups. This is Wednesday evening s Group. It is common for Alumni with over 20 years in recovery to attend the groups BREAKING THE CHAINS OF ADDICTION 5

7 YOUTH PROGRAM Last Door Youth Program is a long term, residential treatment program for male youth ages 13 to 18. Clients average length of residential treatment is 120 days. Post treatment Youth may reside in semi-independent transitional living accommodations for up to one year. Last Door Youth Program fosters experience based learning leading to increased self-esteem and better decision making. Last Door Youth Program collaborates with medical and allied healthcare professionals and with community resources. Last Door Youth Program encourages clients to explore challenging situations and learn to make responsible choices. Clients play an active role in New Westminster s youthful recovery community. Youth find solutions to living situations using peer support while having fun in recovery. 9 FAMILY PROGRAM Last Door Family Program suppor ts healing... Last Door Family Program began in 2005 and is designed to support regaining trust, responsibility and structured accountability. Last Door s unique Family Program optimizes treatment outcomes and carries on post treatment. Parents, Co-Parenting and Women s groups meet weekly in the evenings and are open to all clients and their families. 10 BIOPSYCHOSOCIAL SPECIALIZED SERVICES Lifestyle Group This twice weekly, twelve week group cycle supports the development of healthy and informed lifestyle behaviours. Topics include physical fitness, nutrition, basic health knowledge and balance. Topics cycle in response to participants needs. Gambling, Media and Inter net Addiction Group This twelve week didactic group focuses on recovery from process addictions by increasing awareness and developing relapse prevention skills. Participants develop a peer support network and are introduced to community resources to assist them in making lasting changes. Recreation and Social Activities SPECIALIZED SERVICES Positive social and recreational activities are integral parts of recovery at Last Door. Activities include softball, dodge ball, fishing excursions, camping trips, along with weekly mixed martial arts training. There is access to the local fitness facility featuring an Olympic size swimming pool and state of the art Fitness Equipment. Last Door encourages parent and child activities. Allied Health Professionals Optional weekly Yoga, Acupuncture, Guided Meditation and Naturopathic Services are incorporated into clients schedules at no extra cost. Diversity Last Door supports spiritual, gender and cultural values in a safe and accepting environment. 11 BIOPSYCHOSOCIAL SPECIALIZED SERVICES Support for Co-occurring Disorders Last Door Addictions Treatment Team is supported by a Psychologist, Psychiatrist and CSAM, ASAM and OEMAC Certified Addiction Physicians who collaborate to enhance mental health and addiction recovery services. The client s medical history and medical assessment along with diagnostic instruments (DAST, HoNOS, GAIN and SOGS) inform the treatment plan. Last Door s electronic screening process flags PTSD, depression, anxiety, cognitive impairments, brain injuries and post-acute withdrawal symptoms for further attention. Assessments and reviews at 30, 60 and 90 days along with weekly case management notes help identify additional areas that may require further support. Last Door s Social Model approach has addressed complex client needs for 30 years. Clients with Co-occurring Disorders may benefit from longer treatment durations - often five to six months. 12 FINANCIAL INFORMATION AND FEES Last Door believes in LONG TERM TREATMENT for LONG TERM RECOVERY. Average stay is 90 to 150 days. Scheduling and Billing Fees include a full spectrum of services and activities - screening, admissions, group therapy, individual counselling, case management, report sharing, medication management, social and recreational activities, post treatment planning, return to work planning, housing, meals, family program and continuing care planning. FINANCIAL INFORMATION AND FEES Fees are structured on a Per Diem basis and payable monthly in advance. The rate is $250 per day for treatment services. Long term and short term transitional living rates are dependent on individual post treatment plans. Registered Charity Last Door Recovery Society is a registered Canadian Charity and welcomes donations. Charitable tax receipts are issued.
