1 Student Life Camp 2013 at Myrtle Beach June 17 th - 21 st Cost: $340 This year our student ministry has the opportunity to spend a week at Myrtle Beach, SC for summer camp.the speaker for the week is David Platt and the worship leader is Kristian Stanfill. The spots for camp this year will go quickly. We will be staying at the Sandcastle Oceanfront Resort. Sign-ups for Student Life Camp will begin March 1st of this year. A deposit of $50 is required to hold your spot for camp. Scholarship application are available upon request. Both deposits and scholarship applications are due April 1st. There will be a parent meeting on Sunday April 14 th at 4:30pm in the youth area. The parent meeting is the best chance to take care of form that need to be turned in. There will be a notary at the parent meeting.

Telephone: Name of sponsor s coordinator: Matt Bryant!! Telephone: Description of activity: Students and adults will be traveling by bus to Myrtle Beach, SC. During our stay students will spending time on the beach and in the ocean. There will be may or may not be lifeguards on duty on the beach. Students will also be swimming in the hotel pool, there will not be a lifeguard on duty during these times. Students must inform adults of there location and check-in at appointed times. Student will be staying four to a room in the hotel. We will be eating fast food for dinner. Students are responsible for paying for their own meals. Breakfast will be provided in a continental style by the hotel. Students will be attending worship services in the morning and evening. during free time there is also the possibility that students will be involved in various mission projects that could include, nursing homes, basic construction or yard work, or playing with children. If disciplinary actions are necessary they will include shortened curfew, contacting of parent, or removal from camp, in which case the students legal guardian will be responsible for picking-up the student from camp. Yes No If yes, name of insurer: Policy or group number: Participation Agreement I acknowledge that participation in the activity described above involves risk to the Participant (and to Participant s parents or guardians, if Participant is a minor), and may result in various types of injury including, but not limited to, the following: sickness, bodily injury, death, emotional injury, personal injury, property damage and financial damage. In consideration for the opportunity to participate in the activity described above (the Activity ), the Participant (or parent/ guardian if Participant is a minor) acknowledges and accepts the risks of injury associated with participation in and transportation to and from the Activity. The Participant (or parent/guardian) accepts personal financial responsibility for any injury or other loss sustained during the Activity or during transportation to and from the activity, as well as for any medical treatment rendered to the Participant that is authorized by the Sponsor or its agents, employees, volunteers, or any other representatives (collectively referred to hereinafter as the Activity Sponsor ). Further, the Participant (or parent/guardian) releases and promises to indemnify, defend, and hold harmless the Activity Sponsor for any injury arising directly or indirectly out of the described Activity or transportation to and from the Activity, whether such injury arises out of the negligence of the Activity Sponsor, the Participant, or otherwise. If a dispute over this agreement or any claim for damages arises, the Participant (or parent/guardian) agrees to resolve the matter through a mutually acceptable alternative dispute resolution process. If the Participant (or parent/guardian)and the Activity Sponsor cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel for resolution pursuant to the rules of the American Arbitration Association. If your fingertips are touching skin, your shorts or skirt need to be longer.)
