Dialog Axiata PLC wants to create database to record Employee information and their department information (i) Create Employee and Department tables with suitable data types. o Employee should have fields to record Employee_id(auto generated), First_name, Second_name, Tele_No, Address, Gender( Male or Female),Civil_States, Salary, DOB, Image. o Department should have department code and department name fields, Number_of_Employees. (ii) Create the relationship between two tables with referential integrity constrains. (iii) Enter sample data to the database (five recorders for each table). Consider the followings when you enter data. There should be at least two employees whose first names start with D All employees salary shod be more than DOB should in between 5/16/1984 and 5/26/1993 (iv) Create forms for both employee and department tables. Format the controls in forms. (v) Attach a company logo for each form (vi) Set the suitable titles for each form (01mark) (vii) Create a report to display employees information with their Employee_id, First_name, Second_name, Tele_No, Address, Gender,Civil_States, Salary, DOB. Group the data by Employee_id. Save the report as Employees Information. (viii) Create the queries to obtain following output using query design (a) Create a query to display employees whose first name start with D, with their Employee_id, First_name, Second_name, Tele_No, and Address. Save the query as Employees Information. (b) Create a query to display employees whose has Employee_id, 2 or 4 with their Employee_id, First_name, Second_name, Tele_No, and Address. Assign the site name as your Index no and change the local site folder to the desktop folder.
