3 KAUAI ENVIRONMENTAL, INC. EXECUTIVE SUMMARY Kauai Environmental was contracted to conduct a Hazardous Materials Survey of selected areas of the Kauai Veterans Memorial Hospital (KVMH). KVMH is a medical facility on the west side of the Island of Kauai located at 4643 Waimea Canyon Dr in the town of Waimea. A portion of the facility is scheduled to be upgraded in planned renovation projects. The objective of this survey is to identify the presence of lead-based and lead containing paint and asbestos containing building materials in areas identified in the scope of work provided by the client. The following list summarizes the major findings of this Hazardous Materials Survey: 1) Asbestos was identified in the floor tiles in patient rooms of the Long Term Care Facility and the physical therapy area, and in drywall and concrete / plaster walls in that building. The mastic on the floor tiles in this area did not contain asbestos. 2) The floor tiles in the hallway of the main hospital building were found to contain asbestos in the tiles themselves and in the mastic used in the installation of the tiles and in the mastic adhering the cove baseboard to the walls. 3) The results of the paint sampling for lead indicated that all of the paint samples taken in the facility contained concentrations of lead below the EPA and HUD level of 0.5 % representing a lead based paint. 4) The disturbance or removal of any asbestos containing building materials should be conducted by contractors certified and accredited in asbestos abatement by the State of Hawaii. 5) Any contractors performing work on the areas inspected during this project and facility maintenance personnel should be made aware of the findings of this report so that they will better be able to ensure worker safety and the protection of occupants and the environment while conducting work in these areas. 4 HAZARDOUS MATERIALS SURVEY KAUAI VETERANS MEMORIAL HOSPITAL LONG TERM CARE FACILITY 1.0 INTRODUCTION Kauai Environmental, Inc.(KEI) was subcontracted by the Hawaii Health Systems Corporation to conduct a hazardous materials survey of a portion of the Kauai Veterans Memorial Hospital located at 4643 Waimea Canyon Rd. in the town of Waimea on the west side of Kauai. The area included in this survey included the Long Term Care Facility (LTC), the Physical Therapy (PT) wing, and the hallway of the main hospital building. This report details the methods used in the evaluation of the hospital areas, the types of materials present in the building, the locations of samples collected and the results of the analysis of the samples. This survey was conducted by David Gerow, a Certified Industrial Hygienist, a Certified Safety Professional, and an accredited asbestos building inspector in the State of Hawaii with over 33 years of experience in the evaluation of workplace hazards. 1.1 Project Objectives The objective of this survey was to identify the presence of hazardous building materials that will be encountered during planned renovation work in the hospital and that may present hazards to construction workers, patients, the general public, or the environment, or which are required by law to be handled in a specific manner. Once identified, certain materials may require removal or other treatment prior to or during demolition to prevent the release of the hazardous constituents. 1.2 Scope of Work The areas of the KVMH that were evaluated during this hazardous materials survey were identified in the scope of work provided by the client. The areas specified in the scope of work are shown in Figure 1. The following areas and materials were surveyed in this project. 1) Asbestos Survey of Long Term Care Facility and Physical Therapy wing - materials that were sampled for asbestos analysis included but were not limited to ceiling, walls, flooring, sealants, etc. Areas above the ceiling and the roof were not included. 5 2) Lead in Paint Survey of Long Term Care Facility and Physical Therapy wing - materials to be sampled for lead included painted surfaces on the exterior and interior of this facility. 3) Asbestos Survey of the Flooring in the Hallways of the Main Hospital Building - this task included a survey of the flooring materials in the hallways of the hospital, including floor tiles, mastic, and baseboards. 2.0 SURVEY METHODS The sampling and analysis procedures used in this investigation were designed to address the stated objectives, to comply with state and federal regulations, and to ensure that the planned construction activities are performed in a safe and effective manner. The field inspections were performed on December 21, 2015 by David Gerow, an AHERA accredited asbestos inspector (Cert No. HIASB-0689). This survey involved a thorough inspection of the areas identified, sampling of suspect materials, analysis of the samples for asbestos and lead, review of the laboratory analysis reports, the preparation of this report, and comparison of the reported results with regulatory limits for each type of material. 2.1 Visual Inspections During the inspections, information on each of the identified areas that is relevant to the construction process and the possible presence of asbestos and lead based paint was gathered. This information includes, square footage, types of building systems and construction materials, and their condition. All of the areas included in this survey were subjected to a thorough visual inspection. The purpose of these inspections was to locate asbestos-containing materials and lead based paint and to discern the construction and maintenance history of the building and materials. Inspection and sampling procedures for these materials are identified in the following sections, 2.2 Asbestos Sampling and Analysis Methods An inspection to identify the presence of asbestos-containing materials was conducted on December 21, This survey was performed in accordance with guidelines published by the Environmental Protection Agency (EPA) in the Asbestos Hazard Emergency Response Act (AHERA) for the evaluation of schools and public buildings for asbestos containing building materials, Hawaii Administrative Rules, and the National Emissions Standards for Hazardous Air Pollutants (NESHAPS) for the evaluation of buildings that will be undergoing renovation or Hazardous Materials Survey Kauai Environmental Kauai Veterans Memorial Hospital 2 January 2016

6 demolition. The survey was performed by first observing the construction of the building, identifying all types of building materials throughout the building, and inspecting all accessible areas of the building. Materials that were suspected of containing asbestos were identified and sampled for analysis. No samples were collected of materials that were above the ceiling as defined in the scope of work. Since asbestos has not been used in building materials in the United States for over 30 years, building materials that were installed recently are not likely to contain asbestos. In accordance with regulations in place in the State of Hawaii, at least three samples of each type of suspect material were collected to allow for a positive determination of the presence or absence of asbestos. All building material samples collected during this inspection were submitted to Environmental Hazards Services Laboratory in Richmond, Virginia for analysis. This laboratory is accredited to perform this analysis by the American Industrial Hygiene association and is involved in the National Voluntary Laboratory Accreditation Program (NVLAP). Samples were analyzed in accordance with the EPA Method Interim Method for the Determination of Asbestos in Bulk Insulation Samples, 40 CFR 763, Appendix E, updated This analysis uses polarized light microscopy to identify and quantify the asbestos content of the building materials sampled. The results of the analysis of the asbestos samples collected during this survey are summarized in Section Paint Sampling and Analysis Methods The inspection of the building for the presence of lead-based paint was performed on December 21, 2015, according to guidelines published by the U.S. Department of Housing and Urban Development (Guidelines for the Evaluation and Control of Lea-Based Paint Hazards in Housing, June 1996) and the US Environmental Protection Agency. Representative areas of the identified buildings were assessed in order to determine whether any hazardous conditions would be encountered by the workers during the proposed construction project. When sampling of paint, the painted surface was scraped with a sharp chisel from the top layer of paint to the substrate to ensure that the entire paint layer is included in the sample. Reported lead concentrations represent the average concentration of lead in all of the paint layers. Representative samples were collected from the interior and exterior painted surfaces of the identified areas and doors. The paint samples were submitted to Environmental Hazards Laboratory in Richmond, Virginia for laboratory analysis of lead content using EPA Method SW This analysis involves an acid digestion of the sample with subsequent analysis of the extract by atomic absorption spectroscopy. 7 perform the analysis of paint chips for lead. The results of the lead analysis of the paint samples collected during this survey are summarized in Section HAZARDOUS MATERIALS SURVEY RESULTS This section details the results of the inspection activities described previously. Laboratory analysis reports are included in Appendix B to this report. 3.1 Building Inspection The Kauai Veterans Memorial Hospital was originally constructed in It is a single story building with a concrete foundation and concrete walls and roof. The interior walls are covered with a hard plaster or other concrete-like material. The flooring is completely covered with vinyl floor tiles in traffic areas or ceramic tiles in wet areas such as bathrooms and showers. The ceilings in the hallways and some offices are finished with dropped ceiling tiles in a metal grid, while most of the patient rooms have no ceiling tiles with the concrete roof exposed. The hospital is generally in good condition and in a well maintained condition despite the age of the building. It is apparent that some of the hospital areas have been renovated in the recent past. The flooring in the hallways of the Long Term Care facility were replaced a few years ago, a couple of rooms at the end of the LTC area are newly renovated and a wing adjacent to this area was also completely remodeled. These areas were not sampled as they are not likely to contain asbestos due to the age of the materials. The paint layer on most of the exposed surfaces appears to be intact with only small areas of damage. No significant areas of peeling, chipped, or deteriorating paint were noted. The exterior paint layers are generally in good and intact condition. 3.2 Asbestos Inspection Results Each of the areas included in this survey were visually inspected for the presence of suspect materials that have the potential for containing asbestos. All suspect materials identified during this inspection were then sampled and analyzed. A total of 47 samples were collected, representing materials from the Long Term Care Facility, the physical therapy wing, and the hallway and offices between the main hospital building and the LTC area, and the hallways of the main hospital. The locations from which asbestos samples were collected are presented in Figures 2, and 3. Results from the analysis of these samples for bulk asbestos content are presented in Table 1. Complete laboratory analytical reports including chain of custody documentation are included in Appendix B. All 9"x9" floor tiles in the building should be assumed to contain asbestos. The drywall in the offices in the hallway between the main hospital and the Long Term Care area was reported to contain a trace of asbestos by the laboratory during the initial analysis of the samples. When re-analyzed using a point counting procedure that is more accurate for low concentration samples, the result was reported as 0.25 % asbestos. This indicates that the material, likely the joint compound on the drywall panels contains asbestos, but in concentrations well below the regulatory limit of 1 % asbestos by weight. 10 3.3 Lead Paint Inspection Results Representative paint samples were collected from the painted surfaces, both interior and exterior, of the areas included in this survey. In all, a total of 14 paint samples were collected during this lead-based paint survey. The locations of the samples are not listed in the figures since representative samples of each type of paint was collected from random locations, i.e. interior wall paint or exterior paint. These surfaces appear to have the same painting history and materials. Results from the analysis of these samples for lead content are presented in Table 2. Complete laboratory analytical reports including chain of custody documentation are included in Appendix B. The U.S. Department of Housing and Urban Development defines a lead-based paint as a paint coating that contains greater than 0.5 % lead by weight. Paint with concentrations in excess of this concentration, particularly if there is significant damage or peeling of the paint, may represent a potential hazard to workers performing renovation or demolition activities in the building. The interior and exterior paint coatings on the buildings were generally intact with very few areas of damaged or flaking paint. 11 There may be additional requirements to ensure that workers are protected from fugitive lead concentrations during the demolition work even when working with paint that contains lead below the EPA limit. These may include air monitoring, training, dust suppression, and medical monitoring. The contractor performing the demolition work must be notified of the presence of the lead and the need to comply with OSHA regulations. During the renovation project, it is possible that the debris generated may be required to be sampled for hazardous waste characteristics, specifically Lead by the Toxicity Characteristic Leaching Procedure. This requirement would be dictated by the landfill accepting the debris for disposal. Due to the reported concentration of the lead in the paint, it is unlikely that the debris would be considered a hazardous waste and that the material should be acceptable for disposal at any construction landfill facility. 4.0 CONCLUSIONS A survey was conducted of selected areas of the Kauai Veterans Memorial Hospital located in Waimea, Hawaii for the presence of hazardous building materials, including lead-based paint and asbestos containing building materials. The field inspections and survey was conducted on December 21, This inspection was performed to identify the presence of potentially hazardous materials that may require removal or abatement prior to the planned renovation of the buildings. Asbestos Containing Building Materials A total of 47 building material samples were collected from the subject buildings and analyzed for the presence of asbestos. Several of the materials sampled contained asbestos in excess of the regulatory limit of 1.0 % that defines an asbestos containing building material. These materials include: 1) Approximately 3,000 square feet of a gray 9" x 9" floor tile in the patient rooms and some offices of the Long Term Care Facility and the physical therapy wing. All 9"x9" square vinyl floor tiles should be assumed to contain asbestos. 2) Approximately 5,000 square feet of floor tiles in the hallway of the main hospital building, 3) 1000 linear feet of black mastic on the black cove baseboard in the hallway of the main hospital building, 4) The concrete wall material in the LTC and PT areas. 12 5) The drywall around the offices in the hallway between the main hospital and the LTC area contained less than 1 % asbestos and can be treated as a non-asbestos containing building material or may be handled as if it contained asbestos above the regulatory limit. The contractor performing the renovation should be made aware of this survey and be instructed to be on the look out for additional materials that may be present in the building and to stop work if any suspect materials that were not identified during this survey are located and to inform the owner before proceeding with the work. Lead-Based Paint A total of 14 representative samples of paint were collected from the areas included in this survey. These samples were representative of the interior and exterior painted surfaces of selected areas. All of the paint samples taken in this survey contained lead below the regulatory limit defining a lead based paint of 0.5 % lead by weight. OSHA regulations for worker health and safety may also apply to materials that contain lead below the EPA limit of a lead based paint. In some cases, any lead present in the paint may be enough to trigger some safety requirements. The contractor should assume that the paint on surfaces within the identified areas contain similar concentrations of lead as found in the samples taken for the purposes of worker health and safety and for disposal purposes. The contractor should be notified of these results and required to adhere to worker protection and environmental protection regulations in the management of these items.
