About the Competition: In honor of National Breast Cancer Awareness Month (October), the Kupferle Breast Center at Texas Health Harris Methodist Hospital Fort Worth (Sponsor) is holding a Bra Art competition On Wednesday, Nov. 9, The purpose of the event is to raise awareness about breast cancer and educate women about the importance of cancer screening and treatment options. Entries should be a source of inspiration and support and should represent the personal story of someone who has gone through or is currently going through treatment. Design Guidelines: Have fun and be creative. Any sized bra may be used as the basis for your design. Entries can be hand-painted, dyed, pieced with fabric and/or decorated with trim, feathers or sequins. Designs must be created and named in good taste. For examples of previous entries, visit Art design may not contain any profanity. Art design will be judged on originality, creativity, inspirational and story behind the art. Sponsor and/or judges reserve the right to select additional winners and award additional prizes. One Bra Art entry per person. One Bra Art entry may also be submitted by a group. Winners will be chosen by a panel of judges selected by competition Sponsor. All decisions of the judges are final. Original Bra Art needs to be submitted by the bra designer or designated representative(s). All Bra Art entries must be received by noon on Friday, Nov. 4, Only the first 50 preregistered Bra Art entries will be accepted. Preregister by submitting Waiver/Disclaimer Form and story with 100 words or less, describing how you have been touched by breast cancer. NOTE: Preregistration must take place in order for Bra Art to be accepted. Page 1 of 5

2 Winners will be announced on Wednesday, Nov. 9, 2016 during the Bra Art Event. Winners may be required to provide proof of identification to claim their prize. No purchase required. No submission will be entered into the competition without preregistration. To download a Waiver/Disclaimer Form, go to 1. ELIGIBILITY: Competition is open to U.S. residents. Residents who are twenty one (21) years of age or younger at the time of entry must have Waiver/Disclaimer Form signed by a parent or other adult legal guardian. All Waiver/Disclaimer Forms, stories and Bra Art entries must be received by Nov. 4, 2016 at noon. 2. TO ENTER: To enter competition, preregister by completing the Waiver/Disclaimer Form and your story by Friday, Nov. 4, 2016 at noon. Bra Art may be submitted by post or delivery service to: 1300 W. If a group submission is entered, the group will be responsible for dividing any potential prizes. Bra Art entries will be returned to entrants upon request and must be picked up by 5:00 p.m. (CST) on Nov. 18, 2016, or entries will become the property of Texas Health Fort Worth. 3. CONDITIONS OF ENTRY. By participating, entrants agree (a) to these official rules and to the decisions of the judges and Sponsor and its agents, whose decisions shall be final in all respects; (b) to release, indemnify and hold harmless Texas Health Fort Worth, their affiliates and subsidiaries and their respective directors, officers and employees, agents and assigns from and against any and all claims, liabilities, losses, damages or injuries of Page 2 of 5

3 any kind resulting in whole or in part, from their participation in the competition and the acceptance and use of a prize; and (c) that Sponsor and its agents are not responsible for late, lost or misdirected entries or mail. Late, damaged, incomplete, or illegible entries are void. Sponsor reserves the right, in its sole discretion to cancel the competition at any time. (d) Bra Art entries may be posted on Sponsor s website and (e) Sponsor may use likeness of entries on website, ads, social media, posters or other marketing materials of their choosing. Entrants must also write their full name, phone number, city and on the back of the actual piece or affix a sticker containing this info onto the piece. Entrant must also submit a story of how they have been touched by breast cancer. 4. JUDGING. Bra Art entries will be judged by Sponsor or judges assigned to this competition by Texas Health Fort Worth. All entries will be judged on creativity, originality, inspiration and story behind the art. There will be one winner and one runner-up. 5. WINNER NOTIFICATION. If winners are not in attendance at the Bra Art event on Nov. 9, they will be notified by Nov. 18 via a traceable form of delivery and by telephone. Winners will be required to complete, sign and return within a specified time period an Affidavit of Eligibility, a Liability Release, a Publicity Release (where lawful) and a consent to its use by Sponsor in all media for commercial and other purposes without additional compensation. Failure to return the required documents within 14 days, or if a notification is returned as undeliverable, will void the selection and an alternate will be selected. 6. PRIZE. Prizes will be awarded in four categories. Sponsor and/or judges reserve the right to select additional winners and award additional prizes. No cash substitutions, transfers or assignments of the prize is allowed, except by sponsor for reason of unavailability, in which case a prize of equal or greater value will be awarded. All taxes are the sole responsibility of the winners. Only one prize per entry will be awarded. Winners agree that by accepting a prize each winner waives all liability against Texas Health Fort Worth, its distributors, agencies, employees, sponsors, and third party vendors for any injuries, losses or damages of any kind resulting from winner's acceptance or use of a prize. 7.
