Communication skills, interview techniques, health history, and physical exam skills are introduced. Opportunities to apply health assessment skills, documentation, and critical thinking for nursing care planning are provided. Prerequisite: BIO 2281; Admission to Upper-Division Nursing Co/Prerequisite: NUR 2013; NUR 2003; BIO 2291 Course Learning Outcomes: Upon satisfactory completion of this course, the student will be able to: Course Outcome 1. Demonstrate a method of health assessment that portrays a caring, professional attitude. 2. Conduct health assessment in a systematic sequence and applying General System Model physiological, psychological, sociocultural, developmental, and spiritual aspects. 3. Communicate health assessment data accurately, systematically, and with appropriate medical terminology. 4. Relate principles of therapeutic communication appropriate to the individual s needs. 5. Apply the nursing process to determine an accurate holistic assessment. Students are encouraged to spend at least one additional hour in lab practicing skills each week. Lab hours will be posted. May schedule with lab coordinator for additional practice. Methods of Evaluation: 1. Quizzes 10% There are NO make-up quizzes. The date and time of each quiz will be given at the discretion of the professor. 2. Course Tests - 15% each 45% 3. 3 LAST DAY TO WITHDRAW WITHOUT ACADEMIC PENALTY IS July 10, 2015 LAST DAY FOR ADD/DROP June 26, 2015 Kaplan Resources (nursing.kaplan.com): Focused Review Students are required to take the Health Assessment Focused Review Test one week prior to the Kaplan Wellness & Physical Assessment Integrated Test. You will have two opportunities to take the Focused Review test. You must have 48 hours between the first and second attempts. If you do not have 48 hours between retakes, you will receive a zero for the grade. The first and second attempt scores will be averaged for the Focused Review Test grade. In addition, all questions from the first take must be remediated for a minimum of 45 seconds prior to the second retake to be considered within your grade. If a student does well on the first attempt, they may use that score as their grade. Kaplan Integrated Test Students must achieve an identified proficiency on the Kaplan Wellness & Physical Assessment. Passing the Kaplan test partially meets NUR 4710 Nursing Professional Competency requirements. The point allocation is as follows: 5% Scores Greater than 60 percentile 3 % + 1 Extra Credit % th percentile 3 % th percentile 2 % percentile 1 % plus NUR 4710 remediation < 30 percentile 0 % plus NUR 4710 remediation Remediation Remediates all questions minimum of 45 seconds per question. To be completed by at 11:59pm No remediation or < less than all questions. 2 % 0 % /At risk for failing this course. Students who do not successfully pass the Kaplan Integrated Wellness & Physical Assessment Test Kaplan Test must successfully complete remediation during the following semester in order to pass NUR (Please reference the SON student handbook and NUR 4710 syllabus.) Extra Credit point/percent will ONLY be applied if the student has a minimum course test average of 75% or greater AND has an overall course average of 75% or greater. Additional Responsibilities and Policies for this Course: 1. Check and access ecollege daily. 2. Attendance is required in class and skills lab. All excused absences from class must be reported prior to the absence. Tests will only be made up for excused absences and must be made up within one week. The format of the makeup exam will be at the discretion of the professor. Those students who are late for a test will also be required to take a make-up test. Tardiness is considered to unprofessional behavior. Leaving class before it is over is in the same category as tardiness. 3

4 3. If a student is tardy or absent he/she is responsible for arranging in advance with a classmate to obtain class notes and any handouts. Two points per day will be deducted from the overall grade for lab absences. Two tardies equal one absence. 4. It is expected that students will review, as needed, content from previous courses that have application to the current nursing topics (e.g. anatomy/physiology, pathophysiology, pharmacology, human growth and development, etc.). 5. Ethical Behavior: Nursing is a profession with a Code of Ethical conduct. Therefore, ethical behavior is expected of nursing students. You are responsible for your own work and for presenting it with the sources referenced appropriately according to the APA Publication Manual. 6. Cell phones should be turned off or on the silent mode during class. Students may only leave the class to respond to a phone call in an emergency situation. 7. The following policies are to be followed according to the current edition of the PBA web: Computer Usage Policy Dress Code: Standards of Dress and Personal Appearance Academic Standards of Conduct: for the Community Value System and the Core Values of the PBA Community as well as descriptions of infractions of the standards and the discipline process. 8. Policies are to be followed according to the current edition of the PBA School of Nursing Handbook. Grading Scale: The following grading scale will be used for all School of Nursing courses. Students must achieve a minimum of a C in each nursing course. This course will require a minimum of a 75% average for all examinations (includes tests and quizzes) to receive a final grade of C in the course. Health History: You will take a complete health history on a classmate, family member, or friend and document as instructed. Midterm & Final Check-off: You will perform a physical assessment on your lab partner. You must complete the full assessment with no prompting within the time limit in order to receive full points.
