1 Performance Verification Packet Heavy Equipment Operator: Excavators This performance verification is designed as one method to evaluate job skills and safe work habits of a participant. The performance of the participant must be evaluated by an certified evaluator, at an authorized assessment site and be approved by an accredited assessment center.
2 Performance Verification Form How to fill out and file your information Participant 1) Print your last name, first name, and social security number. 2) Print your company name, current employer, and the state where your employer s main office is located. 3) In the space provided for Participant Signature, sign your name and enter the date you signed the form. Performance Evaluator Administrator 1) In the space provided for Site Code, enter the postal zip code of the location where the performance verification is being conducted. 2) In the column provided for "Date," enter the date the participant completed each of the tasks. This date is important because there may be times a participant does not complete a performance verification in one day. 3) In the space provided for Performance Evaluator," sign your name. 4) In the space provided for Date," next to your signature, list the date the participant successfully completed all of the tasks. 1) In the space provided for Administrator, sign your name. Your signature indicates that the performance evaluator is certified to conduct this performance verification and that it was conducted within the guidelines of the. 2) In the space provided for Date, next to your signature, list the date that this performance verification form is being sent to the for entry into the National Registry. 3) In the space provided for Accredited Assessment Center, print the name of the accredited assessment center that is conducting this performance verification. 3 PERFORMANCE VERIFICATION CANDIDATE SUMMARY Objectives The candidate will demonstrate the ability to survey the job site to be excavated, identify grade stakes and their meaning, and perform pre-start inspection and maintenance procedures. Start, warm-up and shutdown an excavator, perform basic maneuvers and perform basic earthmoving operations with an excavator. Scope This Performance Verification provides a means to observe and evaluate competencies in the following areas: Pre-start inspection prior to excavation Maneuvering operations Excavation operations Job site and excavator safety practices Caution: The Performance Evaluator must be prepared to STOP this evaluation if any unsafe condition or practice occurs at any time during equipment operation. Materials Required Hydraulic excavator in proper operating condition Safe job site for maneuvering and excavation Proper Safety Equipment Barriers and barricades where appropriate Grade stakes Site plans for layout Specified checklists as required Time Required To be determined based on job site Tasks Pre-start inspection prior to excavation Obtain necessary equipment Follow specified checklists Document Findings
4 PERFORMANCE VERIFICATION CANDIDATE SUMMARY Tasks (cont d) Job-site Survey Identify potential job-site hazards Identify and determine meaning of grade stakes Follow equipment and site set-up procedures Verify safety of set-up Start-up and Warm-up of excavator Start engine Follow specified checklists Document Perform basic maneuvering skills Forward movement of excavator Reverse movement of excavator Accomplish a pivot turn Accomplish a spot turn Perform basic excavation skills Create a 20 foot by 20 foot excavation 3 feet deep within plus or minus 6 inches Excavator Shut-down Move excavator to designated shut-down area Shutdown as specified by manufacturer Secure excavator as designated by manufacturer Special Note: Your performance evaluator will STOP this evaluation if any unsafe condition or practice occurs.
5 PERFORMANCE EVALUATOR CHECKLIST Date Completed Task To Perform Pre-start Inspection Verify all safety items are identified; such as fire extinguisher, warning signs, guards, etc. Position barricades and warning signs Properly position excavator for pre-start inspection Identify location and check engine oil level Identify location and check hydraulic fluid level Operator cab area clear of loose items or equipment Basic operator instructions available to operator (capacity charts, operator s manual, etc.) Verify bucket condition Check for and identify any fluid leakage in tanks, lines pumps, cylinders, etc. Properly document findings on required checklist (as applicable) Job-site survey Identify potential site hazards (high voltage lines, underground utilities, etc.) Identify and verify grade stakes Properly position equipment for operation Verify safety of equipment set-up Start-up and warm-up of excavator Start engine Check engine gauges for proper readings Check all controls for proper function (bucket, dipper arm, swing and travel) Properly document findings on required checklist (as applicable) Perform basic maneuvering skills Position excavator as instructed for exercise Travel excavator forward at least 20 feet Travel excavator in reverse at least 20 feet Accomplish a pivot turn of at least 180 degrees Accomplish a spot turn of at least 180 degrees Travel the excavator forward while swinging the upper works to a position 90 degrees (clockwise) from the travel direction and continue at least 30 feet the bring the excavator to a full stop With the excavator at a full stop rotate the upper works back to the front (270 degrees) in a clockwise direction Perform basic excavation skills Position excavator as instructed for exercise Create a 20 foot by 20 foot excavation 3 feet deep plus or minus 6 inches as instructed by evaluator Excavator shut-down Move excavator to designated shut-down area Properly shut-down and secure excavator
