3 Thank you for your interest in volunteering at AMITA Health! We have many volunteer opportunities at our Hinsdale and La Grange campuses, as well as at our off-site locations. If you are compassionate, willing to help others and ready to take on a new adventure, we need you! In this packet you will find all the information needed to make an informed decision about volunteering. We hope that upon reviewing it, you will be excited about joining our team and will choose to attend our next Information / Orientation session. Also included in the packet is everything you will need to become a volunteer. This is designed to shorten the length of time between applying, and actually beginning your volunteer position. You can choose to do as much or as little before the Interview and Orientation sessions as you like. The more you complete, the sooner you can start after Orientation. Again, thank you. 4 AMITA Health Teen Volunteer Requirements Volunteering at AMITA Health is a commitment. Anyone giving service at our hospitals must meet stringent requirements in order to be considered for placement, even our volunteers! It s part of our ongoing commitment to excellence in patient safety, quality and compassion. To be considered for volunteering at AMITA Health, you must meet the following requirements: 1. Be at least 14 years of age and have completed 8 th grade 2. Be compassionate and willing to help others 3. Complete a 2-step TB test (through the hospital free of charge, or from another provider) 4. Receive an annual flu shot if working through the months of November through March 5. Complete initial safety / infection control sessions 6. Purchase the assigned uniform and wear it when volunteering 7. Wear your assigned ID badge 8. Agree to abide by hospital policies, regulations and standards of behavior If you are ready to take the next steps toward volunteering, please read on! 5 Getting Started Step 1 Check It Out! Interested in volunteering? The first thing to do is read this packet thoroughly and decide if volunteering at AMITA Health sounds like a good fit for you. If it does (and we hope it does!) please follow these seven steps to get started. Still not sure? You can choose to come and shadow a volunteer in an area you are interested in, before deciding to volunteer. Just call us. Step 2 Apply to Volunteer (if you applied online, you may skip to Step 3) At the end of this packet you will find an application form. Please fill it out completely and return it as directed on the form. (Missing information will slow down the process, so please check to make sure everything is filled out.) NOTE: Teen Volunteer Interviews and Orientation Sessions are held at 3 intervals throughout the year; in the late fall to begin work in January, in the spring to begin work in June, or in the summer to begin work in September. Spots fill very quickly in this area, so applying early is to your benefit. Step 3 References In order to volunteer at AMITA Health, we will need two references from people who can comment on your ability to volunteer. You may choose two types from the following list of acceptable references. Near the back of this packet you will find Teen Volunteer Reference Forms. Pick the appropriate ones for your two choices, print and give or send them to the people who will be your references. 1. SCHOOL REFERENCE May be a teacher, counselor, advisor, club sponsor, dean or principal from your school 2. MEDICAL REFERENCE Your physician 3. PERSONAL REFERENCE Must be a non-family member You (or the person completing your reference) may return the completed forms to the Volunteer office in the following ways: 1. Bring them to the Volunteer Office 2. FAX them to the Volunteer Office at (630) or (708) them to sandra.mueller@ahss.org 4. Please note: We will need both references returned before you can start to volunteer! 5 P age
6 Step 4: Sign Up to Attend Volunteer Interview and Orientation Sessions Teen Volunteer Interviews and Orientation Sessions are held at 3 intervals throughout the year; in the late fall to begin work in January, in the spring to begin work in June, or in the summer to begin work in September. Spots fill very quickly in this area, so applying early is to your benefit. Volunteers must attend both an interview session (a parent attends this with the teen) and an orientation session (teen attends alone) prior to beginning work as a volunteer. During these sessions we will complete the following: Welcome and Introduction to Volunteering at AMITA Health Safety / Infection Control presentation Pictures for ID Badges Volunteer job choice discussion Volunteer job choice request form completion Completion of required documents Volunteer check-in process training Wheelchair training Hospital tour To sign up for your Orientation Session, please contact Chris DeMaria, Coordinator, Volunteer Services at: christine.demaria@ahss.org or (630) Hinsdale office (708) La Grange office Step 5 TB (Tuberculosis) Test You will need to complete a 2-step TB (tuberculosis) test in order to volunteer at AMITA. This can be obtained through Employee Health at either Hinsdale or La Grange campus, or from your own physician. You may choose to complete this requirement before the Information / Orientation session, or wait until after (recommended if you are undecided as to whether to volunteer). You must complete this step before starting your volunteer position. Note: If you have previously tested positive on a TB test, please contact Employee Health or your physician for alternatives. 6 P age
7 To obtain your TB test from Employee Health: Note: Employee Health is open from 7:30am-3:30pm M-F. If you are unable to come during those hours, you may choose to get your TB test at your physician s office. Call and make an appointment: Hinsdale Campus La Grange Campus Please note: Getting a TB test is a 4-step process. It works like this: 1. Receive TB test at Employee Health 2. Return 2-3 days later for visual examination of the test site. This can be done by an Employee Health nurse or by our Volunteer Office 3. One-to-two weeks later, return to Employee Health for repeat of TB test 4. Return 2-3 days later for visual examination of the test site. This can be done by an Employee Health nurse or by our Volunteer Office NOTE: Failure to complete any of the steps will cause you to start all over at #1! You may have your test completed at either campus. Just tell the Employee Health nurse at which campus you will be volunteering. To obtain your TB test from your own doctor: Call your doctor s office for an appointment. Please note: TB tests are done in stages. It works like this: 1. Receive TB test at your doctor s office 2. Return 2-3 days later for visual examination of the test site. This can be done by your doctor or our Volunteer Department 3. One to two weeks later, return to doctor s office for repeat of TB test 4. Return 2-3 days later for visual examination of the test site. This can be done by your physician or at the Volunteer Department 5. Provide documentation of completed testing to Volunteer Department before beginning volunteer work. You will not be able to volunteer until we have your TB test results on file NOTE: Failure to complete any of the steps will cause you to start all over at #1! 7 P age
8 Step 6: Purchase Your Uniform You will need to purchase your uniform before beginning your volunteer work. This should be done after your job assignment has been designated, due to the fact that some volunteer positions do not require a uniform or require a job-specific uniform. Uniforms may be purchased at the Hinsdale or La Grange Gift Shop (at this time uniforms are site-specific so please purchase them at the Gift Shop where you will be volunteering). They may be purchased by cash or credit card, and the prices are as follows: HINSDALE: Teen o o LA GRANGE: Teen o Pinafore--$20 (girls) Alternative: Apron--$20 (girls) Polo--$20 (boys) Polo--$20 NOTE: Should the cost of your uniform stop you from volunteering, please speak to your Volunteer Coordinator; we can help! Also, if you are volunteering at both hospitals, the second uniform will be complimentary. Just inform the Gift Shop cashiers upon purchase and they will not charge you. Step 7: Flu Shot (winter months) For the protection of our patients, their families, our staff and our Volunteer Team, if you volunteer during Flu Season (November through March), you will be required to obtain a flu shot. Documentation of the flu shot must be on file in the Volunteer Department in order to volunteer from November 1 to March 31. If you are unable to receive a flu shot for physical or religious reasons, you will need to wear a mask at all times while on volunteering during these five months. For more details, please see your Volunteer Coordinator. Whew! That s a lot of Steps! Yes. It is. Volunteering at AMITA Health is truly a commitment; a gift that you give our patients, their families, our staff and yourself. If you have read this far and are ready to start, we are here to help! You can apply online at Note: this application currently says Hinsdale Hospital but will be applicable to either Hinsdale or La Grange campuses. 2. You can apply by printing an application from this packet. Print and complete the application and return it to us in one of the ways listed on the application.
