7 AIMS OF THE PROJECT WAS to provide easy-access services and counseling free of charge for the public to promote the customer s well-being, health and functional capacity to improve availability and accessibility of the services to develop proactive work methods, multidisciplinary management and evaluation methods for needs of social and health services to strengthen professional competence and proactive skills of employees and students to promote multidisciplinary cooperation with the City of Kuopio and other actors University of Eastern Finland Hospital District of Northern Savo Non-Governmental Organizations (NGO) Private companies, which provide welfare and health services Projects framework retells Chronic Care Model and Salutogenic approach

9 OUTCOMES 1. Well-being Kiosks was established in Kuopio and in Varkaus 2. Well-being Kiosks was taken in part of community centers in Kuopio 3. In Ylä-Savo s SOTE (social and health) Joint Municipal Authority was established Well-being Kiosk on wheels 4. Well-being Kiosk services was developed and produced in collaboration with various stakeholders and partners in Evaluation data was collected during operation

10 OUTCOMES 6. Employee s profession was confirmed with collective education 7. Operation was informed using several different channels, especially social media 8. Projects operations model and practices were included in every worker's and units basic work 9. All the best practices from project were available in other municipalities
