7 Problem Current client-level data between HIV care data systems and housing data systems lack an interconnectivity capability to share indicators that could be used to track the performance and evaluation of housing, care, and health outcomes
8 Project Aim To support the electronic integration of housing and HIV care data systems in the coordination of housing and HIV care services to improve health outcomes along the HIV Care Continuum, especially among racial and ethnic minority people living with HIV (PLWH) who are unstably housed, at risk for, or experiencing homelessness. This request for proposal (RFP) announcement solicits applications to support a minimum of 4 sites
9 Eligibility 1. Agencies receiving funding by both HRSA s RWHAP and HUD s HOPWA programs RWHAP Parts A, B, or C recipients or subrecipients AND HOPWA grantees or project sponsors
10 Eligibility 2. RWHAP solely funded recipients or subrecipients or HOPWA solely funded grantees or project sponsors An applicant that is solely funded by either RWHAP or HOPWA must partner with their respective other RWHAP or HOPWA agency in order to complete data integration and workflow coordination for the enhanced service delivery. Letters of commitment from partnering agencies must be dated and signed at the level of the highest official (CEO, Executive Director, or Board of Directors, if applicable), showing how the two agencies will collaborate together for the purposes of this initiative.
11 Eligibility Examples include, but are not limited to: The primary applicant is a RWHAP Part A or B or C recipient or subrecipient in a State or metropolitan statistical area (MSA) partnering with a HOPWA grantee or project sponsor within the same or overlapping service area. The primary applicant is a HOPWA grantee or project sponsor and the partnering agency is a RWHAP Part A, B, or C recipient or subrecipient within the same or overlapping service area.
12 Funding Funding comes from the HHS Secretary s Minority AIDS Initiative Funding (SMAIF) as authorized under the Consolidated and Further Continuing Appropriations Act, 2015 Administered through HRSA s HIV/AIDS Bureau (HAB) through the Special Projects of National Significance Program Collaboration with HUD s Office of HIV/AIDS Housing (OHH) RAND s CTAC will be responsible for funding performance sites through subcontracts
13 Funding Performance site applicants may apply for a ceiling amount of up to $525,000 over the entire project period for the design, implementation, and evaluation of the project Funding period 1 (~4 months) Dates May 1 August 31, (12 months) September 1, 2016 August 31, (12 months) September 1, 2017 August 31, 2018 Maximum funding amount $125,000 $200,000 $200,000
14 Performance site roles Performance sites, with assistance from the Coordination and Technical Assistance Center (CTAC), will design, implement, and evaluate the effectiveness of an integrated HIV care and housing data system and enhanced service delivery strategy, including service utilization, cost, impact on health-related outcomes, and dissemination of findings, best practices, and lessons learned.
15 Coordination and Technical Assistance Center (CTAC) roles The Coordination and Technical Assistance Center will: Provide technical assistance (TA) and capacity building to a performance sites to implement electronic integration of housing and HIV care data systems Conduct an evaluation of integrated data systems and how it affects the provision of clinical practice in terms of provider acceptance, utilization, and in the coordination of housing and HIV care services to improve health outcomes along the HIV care continuum

17 1) Data Systems Integration Design of an integrated data system that effectively supports the coordination of housing and HIV care services Multiple potential models
18 2) Enhanced Coordinated Service Delivery Sites must develop and implement plans for agency and provider utilization of the integrated data system and strategies for coordinating workflows around navigation of patient care, supportive services, and housing needs
19 3) Multisite and Site-Specific Evaluation The CTAC will work with performance sites to codevelop individual monitoring and evaluation plans Sites will be expected to collect and report relevant quantitative outcomes, process, and cost measures to the CTAC throughout the course of the project The CTAC s multisite evaluation will be developed based on the site-specific monitoring and evaluation plans The CTAC will assist the performance sites in gathering data for the multisite evaluation
20 4) Dissemination of Best Practice Models and Lessons Learned Each site will contribute to writing up project implementation experiences, including documentation of challenges, lessons learned, best practices, and innovative models Sites and CTAC will partner on dissemination to RWHAP and HOPWA communities, at state and national conferences, to policymakers, and in peerreviewed journals

22 Submission Requirements Page limit is 25 pages Includes abstract, project narrative, budget documents, staffing plan, and work plan Biosketches, letters of commitment, and other attachments are not included in the page limit Project narrative page limit is 15 pages No particular font or spacing is required, but it would help if reviewers could read the proposal. A recommendation would be 12 point Times New Roman

26 Selection Process Eligible performance sites submit proposals CTAC, HAB, and OHH review proposals. Based on this initial review, up to 8 candidate sites will be selected Selected sites notified of their candidacy Final selection of at least 4 performance sites after in-person site visit assessments conducted by the CTAC
27 Final Site Selection Criteria Geographical location, defined as organizations located in areas of high HIV incidence and prevalence, and with PLWH with unmet and unstable housing needs Demonstrated organizational readiness and information technology infrastructure capacity Knowledge of and experience with the electronic health information systems used to collect and generate RSR and HOPWA reports Feasibility of implementation plan, including identification of key facilitators and barriers, strategies for navigating challenges, and availability of key staff and resources Variation and diversity among sites on capacity, resources, and location

30 Ask CTAC Questions Q: If working with a partnering site, are data sharing agreements required as part of the proposal submission? A: Ultimately these agreements will need to be in place before funding, but are not required until the awardees are named. For the proposal, a letter of commitment from partners is required.
31 Ask CTAC Questions Q: What kind of data will be required to be collected from sites? Specifically, will client-level data be required, and what staff is required (e.g., Evaluator, Data Manager, Interviewer) as part of this requirement? A: Sites need to provide the CTAC with data at multiple levels, including, but not limited to, program, provider, client, and service. Data will also be collected for a cost analysis. Much of this data should be collected from the integrated data systems. The staff required for each project will vary, but it will likely include someone who can help provide the CTAC with data
32 Ask CTAC Questions Q: Please clarify the target population, i.e., PLWH who are unstably housed, at risk for, or experiencing homelessness. If the agency is receiving HOPWA funds, wouldn t these clients be considered housed? Would HIV-negative clients at risk for infection be an appropriate target for this RFP scope? A: It depends. HOPWA grantees fund different kinds of housing programs and some clients might still be considered unstably housed. RWHAP clients may be unstably housed and require HOPWA services. HIVnegative individuals are outside the scope.
33 Ask CTAC Questions Q: Do we already have to have our HMIS system set up to apply for this RFP for integration? A: We expect a wide range of data system capabilities and situations. The CTAC will provide technical assistance in working with sites in these various situations to help identify and implement what will work best for each site.
34 Ask CTAC Questions Q: If multiple sites propose projects that include CAREWare modifications, will these applications be evaluated in terms of their ability to coordinate with other sites? Or is there a possibility that potentially conflicting modifications may be funded? A: Modifications to existing software interfaces would not affect the data that the system is collecting. The base CAREWare data would be extractable from any developed system to fulfill granting requirements.
35 Ask CTAC Questions Q: The award ceiling of $525,000, is this a direct cost limit or a total cost limit? A: The award ceiling reflects total costs (both direct and indirect). Q: Are indirect costs allowed on this proposal? If so, do you have a policy or limitation on what percentage is allowed? A: Indirect costs are allowed if you have a federally negotiated indirect cost rate.
36 Ask CTAC Questions Q: Are there specific forms that must be used for the submission? A: No forms are required, although you are welcome to use the standard HRSA forms if you d prefer. Q: Does the Executive level II salary cap apply to this proposal? A: Executive Level II Salary caps do indeed apply.
