We are also responsible for regional planning and for contributing to a free and accessible cultural life in the county. 3 Stockholm County Council is a growing and popular region, whether for living, studying or working. This is positive, but also means more commuters using public transport and more residents requiring good health and medical care. In 2013, the population increased by 36,036 people, which is about 40 percent of all of Sweden s population growth. Stable finances and cost control For the seventh consecutive year, Stockholm County Council reports positive results. A balanced budget is one of our long-term objectives. The surplus for 2013 of SEK 934 million was SEK 1,050 million less than the preceding year, but SEK 410 million higher than budget. With a balanced budget and a focus on cost control, we can modernise and invest in expanding healthcare and public transport throughout Stockholm County both now and in the future. Leading clinical research Stockholm County Council participates in and finances clinical research in health, medical and dental care. Each year, the assembly decides on a special research and education budget. In 2014, the county council will jointly finance research and education in the amount of about SEK 1.5 billion. This research will help develop health and medical care and improve well-being. The county council works very closely with, for the most part, Karolinska Institutet, which is a world-leading medical school. We also have a partnership agreement with Kungliga Tekniska Högskolan (the Royal Institute of Technology). Cost allocation Culture and art in the county Culture contributes to strengthening the Stockholm region, making it more attractive to residents, visitors and investors. Art and culture are also important in the creation of more healing healthcare environments. Residents in the county can enjoy art in different environments, such as in our hospitals and in the Metro. The county council contributes to broad cultural activities and is the principal financier of the Stockholm Concert Hall and the Royal Stockholm Philharmonic Orchestra. 5 Visions becoming Care at the right time is one of the county council s objectives for health and medical care. 5

6 How the county council is run A county council is responsible for political governance at regional level, that is, within the county. The municipal councils are responsible for the local level while the national government is responsible for the national level. Visions becoming A county council is an organisation that is ultimately governed by the residents in the county. Every four years, the county s residents elect the political parties and politicians who are to be represented in the county council assembly. The county council assembly for Stockholm County, with its 149 members, is Sweden s second largest executive assembly. The County Council is an organisation that is ultimately controlled by the people living in the county. Political governance The politicians most important governing document is the budget, which is decided by the county council assembly each year. The budget describes objectives, investments and budgetary constraints and provides direction as to what county council activities are to achieve. The county council assembly also determines the county council tax rate. Political bodies comprise the county council assembly, the county council executive board, committees, subcommittees and working groups. The overarching political governance throughout the year comprises meetings of the political bodies at which the politicians make decisions on the matters put before them. Democracy is also exercised via daily contact and meetings with residents and organisations throughout the county. Operational management Every political board and committee has an administration that is responsible for the day-to-day work. Civil servants working in these administrations compile material for political decisions and implement the decisions that are made. The county council s operating activities encompass the county council s 6

7 administrations and companies, as well as external contractors. The county council has agreements with about 1,000 private care providers and transport operators. The management teams at the county council s different administrations and companies make decisions concerning their operations and how they are best organised to implement the decisions made by the politicians. Operative units in the county council compile material for the county council budget based on what they believe needs to be done within their areas of responsibility to achieve the county council s overall objectives. Once the county council budget is decided, the administrations and companies concretise objectives, investments and budgetary constraints in final individual budgets to clarify exactly what each unit is to achieve. This is how a decision is made A case may begin with a question to the County Council, with a decision, or with an assignment. It can, for instance, be a request for a comment from governmental investigation, a motion that has been introduced, or an assignment from a political entity. The official statement or opinion is forwarded on for political management, where political officials prepare a policy document prior to the decision being made. Administration Civil servants work with the task and write an official response. The County Council Director writes out an official response or statement of opinion. Political Organisation Management Boards decide or make recommendations to the County Council. The County Council Board decides or makes recommendations to the County Councillors. The County Councillors assembly decides. The County Council preparatory committees makes recommendations to the County Council Executive Management. The decision leads to a new task or project The task or project is implemented in the activity or the management 7

When the county grows rapidly, pressure on the region s water also grows. Today, Stockholm County Council handles many issues of major importance to the county s water resources, as both a service provider and a regional planning body. The healthcare services work for improved water quality by reducing the use and emissions of environmentally-impacting chemicals and pharmaceuticals. In 2013, we took a unified approach to important water issues in the region with a new environmental strategy for water that shows how the county council can achieve the most good. 8

9 Regional city centres To provide relief to the central parts of our region (Stockholm s inner city), the development of eight outer regional cores is being planned. By developing the outer regional cores, we can counteract the dilution of business and urban development. A continued dilution of urban development is not sustainable in the long term. This leads to growing needs for transportation and greater reliance on cars. If the development of new businesses and homes can be concentrated to the regional cores, it can also promote service exchange, meetings, urban qualities and city life. Pioneering environmental efforts One area where Stockholm County Council is playing a vital role is our environmental efforts. The vision is to be groundbreaking, stimulate development in environmental technology and inspire others. The environmental work is a crucial element of the county council s work on sustainable social development and well-being. We use biogas, ethanol and electricity from renewable sources in public transport and prevent climate-impacting emissions of nitrous oxide from healthcare services. Our successful work to phase out environmentally and health-hazardous chemicals inspires other county councils. The Code of Conduct, with social requirements on suppliers, that we were involved in developing is now standard among all county councils in Sweden. Our environmental efforts are guided by the Environmental Challenge 2016 programme. We establish challenging objectives and encourage others to adopt them. In 2013, this made us Sweden s greenest county council. The areas in which we have made the most progress in our environmental work are characterised by driving politicians and knowledgeable civil servants. We don t do anything halfway, but rather make a whole-hearted effort and work systematically. 11 Future Plan for Health and Medical Care becoming reality In Stockholm County, the number of children and the elderly are increasing faster than other age groups. This combined with the continuing increase in the residents life expectancy is placing great demand on healthcare services. To be able to offer modern healthcare that meets patient expectations and the healthcare needs of the future, the Future Plan for Health and Medical Care is now being implemented. This will result in more care in better premises and with new approaches. Largest healthcare investment ever In total, SEK 42 billion is being invested to develop health and medical care in Stockholm County. This is one of the county council s largest ever investments in healthcare. SEK 14.5 billion will be used to finance New Karolinska Solna. SEK 28 billion will be invested in new constructions and renovations at hospitals and a number of other improvements. By 2018, the implementation of the Future Plan will result in a 16-per-cent increase in the care offering. Hospitals being extended At the beginning of 2014, plans were presented for the renovations and extensions to be made over the next ten years. Plans that also include improvements to existing premises and investments in medical technology. Södertälje hospital is the first in a series of county council investments in healthcare properties that will be carried out under the Future Plan. Visions becoming The Future Plan for Health and Medical Care shall result in more care in better premises and with new approaches. 11

12 Visions becoming Totally 870 more hospital beds 2018 Newly-built Karolinska Solna In Solna, one of Europe s most modern university hospitals continues to evolve. The newly-built university hospital will be an important actor in future health and medical care. Here, the most highly specialised medical care will be provided and basic research, clinical research close to the patient and education will be conducted. New Karolinska Solna will welcome its first patients at the end of New approach increases patient participation According to the Future Plan, new ways of working shall be introduced into healthcare. The objective is to create more effective healthcare with retained quality where the patients are more involved in their own care and where the care is closer to the patient. It shall also become easier for the patient to obtain the right care, at the right place and the right time. General practitioners (GPs) are given a clearer role in supporting patients through the healthcare process, such as in consultations with different specialists. Residents will encounter healthcare services wherein various care providers cooperate in networks with the GP in a coordinating role and the patient in focus. More effective e-health services The merger of the healthcare information services Vårdguiden and 1177 facilitates the development of more effective e-health services. New e-health services will be developed for both patients and care providers. It will be easier for residents to find the care they need and to influence their care. There will be opportunities to read one s medical records, order tests to take at home, get help with self-treatment and make appointments. For care providers, the investments will better facilitate cooperation between them. Through safe and effective systems, all care providers that work with a patient will easily find and share relevant information. In order to improve emergency treatment, A&E departments will be streamlined and parts of emergency and specialist care will be transferred to local hospitals and local emergency clinics. Improved emergency care One goal for Future Health and Medical Care is for emergency care to improve. 13 is estimated that at least 15 per cent of the patients who receive care at A&E departments could be better cared for by other care providers. Research closer to care Care providers cooperating in networks and specialist care more closely integrated with other care will increase the opportunities to conduct research and education. Research will be linked more closely to daily care operations, which will contribute to research results being converted to early diagnostics and better treatment sooner, as well as help generate new research. 14 Stockholms stad Modernisation and expansion of public transport Every year, Stockholm County Council invests billions of Swedish kronor to improve existing transportation. The major investments that have been made and will be made in the next few years also involve several large expansion projects. 15 Traffic systems for a growing Stockholm region Stockholm County Council is conducting several major investments to expand and modernise the county s infrastructure. Public transport is important to strengthen the regional connections, tying together various parts of the region and creating a polycentric urban structure. Well-functioning public transport is also vital to the labour market in the region and to reduce segregation. The trend in the region is for growing numbers of residents to travel by public transport. Traffic investments in the region A large number of traffic projects are underway in the region. Up to 2021, more than SEK 100 billion is being invested in around 70 traffic projects in Stockholm County. Everything from construction projects like the Citybanan commuter rail, Slussen and an extensive expansion of public transport to more bicycle paths and walkways will ensure accessibility in the county in the future. The Stockholm Traffic Improvement Endeavour is the comprehensive term for these joint investments that the Swedish Transport Administration, Stockholm County Administrative Board, City of Stockholm, Stockholm County Council and the Stockholm County Association of Local Authorities are making for better accessibility in the Stockholm region. stations 19 kilometers new subway Visions becoming Extended Metro Administration The county council has decided, together with the national government and several municipalities, that 19 kilometres of new Metro track, nine new Metro stations and 78,000 new homes will be built. This is the largest investment in the Metro in modern times. In March 2014, the Extended Metro Administration was established with the task of implementing the Metro expansion. The Traffic Administration retains responsibility for traffic administration and other traffic investments. A total of SEK 25.7 billion is being invested in the expansion of Stockholm s Metro. The Metro expansion will commence no later than 2016 and all new lines are expected to be finished and in service by
