3 SKILLED NURSING CARE Elant s skilled nursing home programs provide a variety of medical, clinical, social and psychological services to address the needs of individuals when the level of care needed can no longer be provided at home. Elant understands that this transition can sometimes be difficult. Comfortable living arrangements and a skilled, caring, highly-trained staff help to make the change as gentle as possible. 4 IN-PATIENT REHABILITATION SERVICES Whether the challenge is recovery from illness, injury, or surgery, Elant offers a full spectrum of quality therapeutic services for adults 18 or older. Comprehensive rehabilitation programs are specifically designed to help in recovery so you can get back to an independent and active lifestyle as quickly as possible. The rehabilitation continuum follows the steps of each patient s therapeutic needs and progress from hospital discharge through customized therapies provided at our inpatient rehabilitation care centers. n Elant at Meadow Hill n Elant at Fishkill n Elant at Wappingers Falls ELANT MEDICAL PRACTICE The first of its kind in the Hudson Valley region, the Elant Medical Practice consists of specialized physician and nurse practitioner teams who are available on-site and dedicated to providing full-time health care services, five days per week at each of Elant s Orange and Dutchess County care centers. The sole focus of the Elant Medical Practice is to provide quality care exclusively to Elant residents and patients a unique health care model in long-term care and a distinct service of Elant, unavailable at other area skilled nursing facilities. n Elant at Meadow Hill n Elant at Fishkill n Elant at Wappingers Falls
5 SHORT-TERM RESPITE CARE A short-term respite stay at one of our campuses can give caregivers a break during a vacation or temporary relief to refresh and reenergize. As a caregiver, you can relax knowing that your loved one is in a safe, welcoming environment with personalized care and support tailored to their needs. We are happy to arrange a trial stay that will meet the needs of both the caregiver and the individual in need of care. PALLIATIVE CARE For those in need of diagnostic-related palliative or end-of-life care, Elant has a variety of services available. Our palliative care program offers relief from the pain, symptoms, and stress of serious illness, regardless of the prognosis. We concentrate on the needs of the whole person and respect the individual s preferences and decisions regarding the care services they receive. Our program helps to provide comfort and support for both the resident and their family members.
6 ASSISTED LIVING For seniors who wish to maintain their independence, but need help with activities of daily living, Elant s licensed assisted living communities provide a safe and comfortable home. Residents who choose to make their home at Elant enhance their independence when they receive needed assistance, allowing them to focus on their quality of life. Elant also offers an approved, waivered Dementia Unit for those with cognitive care needs. n Glen Arden RETIREMENT COMMUNITY LIVING Self-sufficient seniors 62 and older will find a perfect combination of independence and security with the assurance of available long-term care, if it is ever needed, at Glen Arden, Elant s continuing care retirement community. Glen Arden offers a comprehensive program of independent living, assisted living and skilled nursing all on one campus. Glen Arden gives you peace of mind by providing quality care as your needs change over time. This comes with the guarantee that all of your long-term health care is covered for life, with minimal out-of-pocket expenses. HOME HEALTH SERVICES The Elant at Fishkill Long-Term Home Health Care Program helps individuals remain as independent as possible in the home. A dedicated case manager works with each client to develop a personalized plan of care designed to address the client s medical, social, psychological and environmental needs. The program is available in Dutchess County only. Elant s Adult Foster Family Care Program matches elderly or disabled adult clients with families in Dutchess County who share their home and provide personal care for the client, 24 hours per day, seven days per week in the stimulating and nourishing environment of family life.
7 ADULT DAY SERVICES With today s busy working families, we often find that seniors who are able to live at home require quality care while family and friends are at work. Elant s adult day services provide quality daytime care for individuals who need care and assistance. Programs: Daybreak (Social Model) The Daybreak Program provides professional care to adults 18 and older in an entertaining, stimulating and supervised group environment. The program provides an opportunity for those who can live independently at home to socialize during the day, allowing families to enjoy peace of mind while they are away from home, knowing their loved ones are safe and well-cared for. Adult Day Health Care (Medical Model) For adults who need assistance, are disabled, or chronically ill, but prefer to live at home, Elant s Adult Day Health Care Program provides individualized care under the direction of a registered nurse and in collaboration with interdisciplinary care team. The program focuses on providing personal care and strengthening and improving physical and cognitive abilities while also providing socialization and companionship. n Elant at Meadow Hill ELANT FOUNDATION The Elant Foundation supports the mission of Elant by garnering financial resources to fund initiatives that enable our staff to enhance the quality of life for all those we serve.
