3 Aurora Health Care A... urora Health Care s integration as a not-for-profit system began in 1984 when two Milwaukee hospitals, St. Luke s Medical Center and Good Samaritan Medical Center, joined to become St. Luke s Samaritan Health Care. It was the first healthcare system in Milwaukee to be created through the affiliation of two formerly independent hospitals. St. Luke s Samaritan Health Care adopted the name Aurora Health Care in 1987 after a merger between Mount Sinai Medical Center and nearby Good Samaritan Medical Center. 5 Aurora Visiting Nurse Association Aurora Visiting Nurse Association For more than 100 years, the Aurora Visiting Nurse Association (AVNA) has been providing home care with compassion, professionalism, and clinical excellence. Originally founded in 1906 by Milwaukee businesswoman Sarah Boyd, the Aurora Visiting Nurse Association of Wisconsin became part of Aurora Health Care in It now serves people throughout eastern Wisconsin. VNA Services The Aurora VNA provides healthcare solutions that enable people to remain independent within their communities and is the most comprehensive source of home care services in eastern Wisconsin. Specialty Services The Aurora VNA strives to meet patients needs from hospital to home, offering specialized home care services. IV Therapy and Respiratory Therapy Examples of IV therapies include antibiotic therapy, chemotherapy, and nutritional supplement. Respiratory therapy provides breathing treatments, oxygen therapy, sleep apnea monitoring, and CPAP therapy. Pediatric Services The Aurora VNA has developed a special home care program just for children and their unique needs. Healthcare at Home The Aurora VNA provides a wide range of home health services, including: Skilled Nursing Registered nurses and licensed practical nurses are available around the clock, and care is provided under the direction of a doctor. Home Health Aides Certified home health aides provide assistance with bathing, grooming, dressing, and exercising, other personal care, and light housekeeping. Rehabilitation Therapy Therapists work with patients to improve the quality of everyday living and to achieve the highest possible level of independence. Social Services Aurora VNA social workers help families meet the challenges of caring for a loved one at home. Hospice Care The Aurora Visiting Nurse Association Hospice Program provides services in Milwaukee and throughout southeast Wisconsin, and focuses on a special way of caring for the person whose life expectancy is limited. 6 Aurora Visiting Nurse Association, continued Palliative Care The goal of palliative care is to provide the best quality of life for the critically or chronically ill by ensuring their comfort and dignity. Palliative care is not disease or age-specific and can benefit any patient with an advanced disease or life-limiting illness. Some examples include cancer, advanced heart, lung, and kidney disease, AIDS, Alzheimer s disease, and multiple sclerosis. Palliative care is focused on relief of pain and other symptoms while addressing the psychological, social, cultural, emotional, and spiritual needs of the ill person and his/ her family. Aurora VNA Connect Personal Emergency Response Services are connected to the existing phone system and have a built-in two-way communication feature that is hands free, so there is no need to make it to the phone. Aurora VNA Connect Personal Emergency Response Services offer peace of mind at the press of a button. In-Home Health Monitoring Aurora Visiting Nurse Association Home Health Monitoring Program provides patients with the best of both worlds: the comfort and independence of staying at home, and the security of daily monitoring and proper medical attention. The system consists of a compact monitor and scale, which is placed in the home. The data is sent via the phone line to the Aurora VNA office where a nurse reviews the information 365 days a year and responds appropriately. 04 VNA Connect Aurora VNA Connect helps seniors and those with special needs feel more independent and safe at home. Whether the need is for emergency services or gentle reassurance and peace of mind, care center representatives are professionally trained and ready to respond anytime day or night. Home Medical Equipment & Supplies Durable home medical equipment helps patients live safely, comfortably, and independently at home. The Aurora VNA houses a complete supply of home medical equipment and products for timely delivery and set-up in the patient s home. Experienced customer service representatives assist providers and patients in choosing the best home healthcare products for each situation and then make arrangements for delivery and set up by trained respiratory therapists or service technicians. Aurora Adult Day Centers Many older adults may need support in meeting daily health and social needs to maintain as independent a life as possible. Aurora Health Care offers a program specifically designed for both older adults and some younger adults who require special care during the daytime hours.

The Good Night s Sleep Clinic of the Aurora Visiting Nurse Association is specifically designed to help clients with their CPAP and BiPAP therapy. Who Receives Aurora VNA Services The Aurora VNA provides an in-home assessment to establish a plan of care for people who: Have been hospitalized; Have had surgery; Have serious or multiple health problems; Suffer from a chronic disease; Are disabled, living alone or with a caregiver; and Need assistance to remain at home regardless of age, from newborn to elderly. 8 Position Description T he... President, Visiting Nurse Association provides overall strategic direction for all operations within the Aurora Visiting Nurse Association (AVNA) and Aurora s durable medical equipment and hospice operations to ensure the achievement of the Aurora Health Care s objectives, mission, and long-term strategy. Reporting Relationship The President, Visiting Nurse Association will report to the Executive Vice President and Chief Nursing Officer. Principal Accountabilities 06 Oversees the development, implementation, and direction of Aurora s visiting nurses association, durable medical equipment, and hospice operations. Ensures strategies are integrated and consistent with Aurora s long-term strategies and initiatives. Oversees clinical practices and creates an environment to support interdisciplinary clinical care. Develops and executes innovative marketleading strategic plans which enrich the patient experience, contribute to program growth, and support Aurora s objectives. Ensures solid operational models and metrics are developed and utilized to obtain operational excellence while creating and maintaining best practices in healthcare delivery. Collaborates with growth and market development to design and execute growth plans for new and existing programs/services based on various assessments, including community needs and awareness. Ensures quality of services by monitoring and evaluating programs provided. Oversees patient safety and the overall patient experience. Performs human resources responsibilities for staff, which include interviewing and selection of new employees, promotions, staff development, performance evaluations, compensation changes, resolution of employee concerns, corrective actions, terminations, and overall employee morale. Develops and recommends operating and capital budgets and control expenditures within approved budget objectives.

9 Position Description, continued Experience and Qualifications Master s degree (or equivalent knowledge) in healthcare administration or other related field. Ten years of management experience in home care and hospice operations, which includes overseeing the development, implementation, and direction of the operations, clinical practices, and strategies. Advanced knowledge of home healthcare, hospice, and durable medical equipment programs and operations. Superior business development, operations management, and leadership skills. Demonstrated success in revenue growth and in development and market planning. Expertise in developing strategic and tactical plans to meet business objectives in a large integrated healthcare system. Demonstrated understanding of healthcare delivery and financing systems. Expertise in managing multiple, conflicting, complex, and simultaneous demands in geographically dispersed areas. In-depth experience and knowledge of the legislative and regulatory environment, and emerging trends in hospice and home care and the broader healthcare arena, including an understanding of healthcare delivery, reimbursement, financing methodologies, cost containment, and new revenue generation strategies. A leader with a strong, engaging public image. Able to establish effective relationships with fellow healthcare leaders, physicians, and community and business leaders in the region as well as local, state, and federal healthcare regulators. Personal and Professional Attributes The successful candidate will possess a wide range of needed personality traits, work habits, and social skills necessary to perform effectively within Aurora Health Care. This individual will possess both personal and professional integrity, strong communication skills, and a professional appearance and presentation. Specifically, the following knowledge, skills, and abilities will be required to be successful in this position: Honest and a highly ethical team player. An extremely organized, disciplined, hands-on, and process-oriented leader who is not afraid of digging into details when necessary. Initiative, self-confidence, good judgment, and the ability to make decisions in a timely fashion. Strategic vision and thinking. Ability to position the organization for the future, looking beyond the present situation to conceptualize key trends and identify changing market demands. Opportunity Assessment The President of the Aurora Visiting Nurse Association will establish a clear vision and strategic direction through a successful collaborative planning process with the management and Board. The President will oversee the development, implementation, and direction of Aurora s visiting nurses association, durable medical equipment, and hospice operations. A key challenge for this leader will be utilizing resources to drive awareness, education, and collaboration to improve the capture rate for referrals across the Aurora system. By identifying and investing in resources, Astools, and systems, they will improve internal operating systems and procedures as well as internal communications. The President will advocate for AVNA within its broad service area and at the local, state, and national level to raise the organization s profile and visibility. 07

The city of Milwaukee is located on the western shore of Lake Michigan, about 90 miles north of Chicago. With a population of 589,000, it is the largest city in Wisconsin and the 22nd-largest city in the nation. The metropolitan area, which includes Milwaukee, Ozaukee, Washington, Waukesha, and Racine Counties, is home to 1.7 million residents. About half of the population lives in the area s five largest cities: Milwaukee, Racine, Waukesha, Wauwatosa, and West Allis.
