You must complete an application for the Florida Medicaid Program by contacting the Department of Children & Families DCF services. Call Client Relations at to locate the nearest DCF office to you. A financial evaluation must be done, before a patient can be accepted into the clinic program. Myrtle Ave Free Diabetic Education and Monitoring Blood Sugar Monday-Thursday 8am 5pm Friday 8am-2:30pm Walk-ins are welcome You are required to attend 3 free diabetic classes. They will monitor your blood sugar for free. Upon completion of the class should you require assistance or a referral for supplies and a monitor, the Clinic will work with you and your health insurance carrier. If you do not have insurance they will refer you to the Shands Financial Screening unit for a possible Clinic card. To use the Health Department on 6 th Street and the Boulevard to obtain your insulin, you must have your written prescription from your medical doctor. To use the I.M. Sulzbacher Center for assistance with syringes and strips, you will need to call the Clinic first at 904/ ext. 23 for hours of operation. The clinic days and hours vary from week to week. If you are a Medicaid beneficiary and your eligibility status is active, you can call the AHCA Medicaid program office at 904/ (Mon-Fri 8am-5pm) to solicit provider participation for Pharmacy and DME (Durable Medical Equipment). Project management personnel at the property will determine your eligibility and advise you when an apartment will be available. This list is subject to correction. The website for the national list of public housing authorities is We fund money to them to subsidize the rent through our Public Housing Program. Subsidized housing is also available through the United States Department of Agriculture (USDA), Rural Development in Gainesville, Florida. Multi-Family Housing - Rental Assistance Program (Section 515 and 514) are for individual residents of multi-family dwellings primarily in the form of rental assistance. Rent subsidies under the Rental Assistance Program ensure that elderly, disabled, and low-income residents of multi-family housing complexes financed by Rural Housing Service (RHS) are able to afford rent payments. With the help of the Rental Assistance Program, a qualified applicant pays no more than 30 percent of his or her income for housing. Please note that not all residents of RHS-financed housing developments receive rental assistance.
