4 MEDICAL CLINIC RESOURCES OU Physicians Community Health in partnership with Oklahoma City County Health Department Walk-in patients welcome, or schedule an appointment by calling (405) or (405) Most insurance accepted. There is a sliding-fee scale for uninsured patients based on poverty guidelines (household income and number of people in the household). Patients are required to complete a financial assistance form and need to bring a monthly pay stub. Co-pays range from $10 - $60 based on income.
