2 Medical Definitions Acute Care- A disease that begins suddenly and usually runs a short course. Chronic Condition- An illness that persists for a long period of time usually more than 3 months. A condition that can be controlled, but not cured. Eligibility- This may include proof of residence and income. Common items requested include paycheck stub, housing receipt, power bill, tax return. Low-Income Status- To qualify for low-income status, household incomes are usually 200% of the federal poverty level or below. Consult clinics to determine status. Primary Care- The "medical home" for a patient. Primary care provides the patient with a broad range of care, both prevention and treatment. Sliding-fee scale- Fee charged for a service based on income. 1 Special Note: The following clinics do NOT provide emergency care, or write prescriptions for narcotic medications. Walk-ins are seen on a first-come/firstserved basis.

By Appointment only. Not a Walk- In Clinic. If approved, patients are given an identification card and a medical record number that they should bring on every visit. When you call to make an appointment, please

Eligibility Screenings: Tuesday & Wednesday 1:30pm 3:30 pm Eligibility Requirements: Must provide proof of low- income status. Must have lived in Forsyth, Stokes, or Davie counties for at least the last three months. Cannot be eligible for state or federal sponsored healthcare insurance, including Veterans Administration, Medicaid, or Medicare. Cost: Free Services: Basic Health Care, Cardiology, Dental, Dermatology, Endocrinology Gastroenterology, Gynecology, and Neurology services. Note: Does NOT provide emergency care. Does NOT provide care for pregnant women. Community Mosque Clinic 1419 Waughtown Street Winston-Salem, NC (Next to Family Dollar) Phone: (only answered during clinic hours) Hours: The first and second Saturday of each Month 10:00am - 2:00pm Appointments are available one month in advance: Walk-ins welcome. Green Street United Methodist Church Clinic 639 South Green Street Winston-Salem, NC 2710 (West Salem) Phone: Hours: Wednesdays 6:00-8:00pm from September through May. (Every other week in the summer). Call for clinic hours if not sure. Doors open at 5:30 and close at 7PM. Walk-ins only. Appointments not available. Cost: Small co-pays for services. No insurance.
