2 Medical Definitions Acute Care- A disease that begins suddenly and usually runs a short course. Chronic Condition- An illness that persists for a long period of time usually more than 3 months. A condition that can be controlled, but not cured. Eligibility- This may include proof of residence and income. Common items requested include paycheck stub, housing receipt, power bill, tax return. Low-Income Status- To qualify for low-income status, household incomes are usually 200% of the federal poverty level or below. Consult clinics to determine status. Primary Care- The "medical home" for a patient. Primary care provides the patient with a broad range of care, both prevention and treatment. Sliding-fee scale- Fee charged for a service based on income. 1 Special Note: The following clinics do NOT provide emergency care, or write prescriptions for narcotic medications. Walk-ins are seen on a first-come/firstserved basis.

Must have lived in Forsyth, Stokes, or Davie counties for at least the last three months. Cannot be eligible for state or federal sponsored healthcare insurance, including Veterans Administration, Medicaid, or Medicare. Cost: Free Services: Basic Health Care, Cardiology, Dental, Dermatology, Endocrinology Gastroenterology, Gynecology, and Neurology services. Note: Does NOT provide emergency care. Does NOT provide care for pregnant women.
